[Localized Caroli's disease associated with mucosecreting papillary adenocarcinoma. Value of hepatic resection].

A new case of carcinoma arising in Caroli's disease in a 48-year old male patient is reported. Since dilatation of the bile ducts was localized to the left lobe, left hepatectomy was performed. Histological examination showed that the tumour was a papillary mucoid adenocarcinoma. There was no recurrence of cholangitis while the patient survived. This case and a review of the literature shows that left hepatectomy suppresses repeated attacks of cholangitis in patients with Caroli's disease restricted to the left lobe, and that the incidence of carcinoma in Caroli's disease is higher than was formerly believed, which should encourage early surgical treatment when the disease is localized in the left lobe.

Immunohistochemical study of pheochromocytomas. An investigation of methionine-enkephalin, vasoactive intestinal peptide, somatostatin, corticotropin, beta-endorphin, and calcitonin in 16 tumors.

Immunohistochemical investigations were carried out on 16 pheochromocytomas for a study of their immunoreactivity to methionine-enkephalin, vasoactive intestinal peptide, somatostatin, corticotropin, beta-endorphin, and calcitonin on serial semithin araldite sections. All antiserums except anti-beta-endorphin, selectively stained a variable number of distinct tumor cells. Methionine-enkephalin-immunoreactive cells were the most frequent. The neuropeptide content of pheochromocytomas appears highly diverse and unpredictable. These findings are supportive of the concept of multisecretory APUD cells of neural crest origin and rule out any of these neuropeptides as reliable immunohistochemical markers for tumor chromaffin cells.

[Gastric pseudolymphomas. Apropos of 3 cases. Review of the literature].

The authors report three cases of gastric pseudolymphoma, before going on to review the literature on the subject. The pseudolymphoma has only recently been identified in relation to other primitive gastric lymphoid growths, on the basis of histological and evolutional criteria, which show it to be benign. It raises two problems. Firstly, a practical problem of diagnosis, as it often visualizes radiologically and endoscopically as a malignant tumor, and this diagnostic uncertainty persists, even after endoscopic biopsy. This necessarily implies surgical management of the case, normally leading to a clear diagnosis based on study of the gastrectomy specimen. Histologically, a growth of this nature consists of a very large lymphoid hyperplasia, often of an organoid type, strictly confined to the mucosa, which, incidentally, shows no real recent or ancient ulceration. Cytologically, this lymphoid infiltrate is benign and the neighbouring nodes are always normal. Point by point, therefore, these characteristics distinguish the pseudolymphoma from the real lymphoma and the lymphoid hyperplasia associated with ulcers. Fluorescence microscopic study of the intracytoplasmic immunoglobulin, which was carried out in the cases treated by the authors, and also in another recent case reported in the literature, shows a polyclonal type immunofluorescence. The pseudolymphoma is therefore, a benign form of lymphomatosis without ulceration. The second problem concerns the real nature of the lesion: a local immunosuppressive pathology? An atypical lymphoid presarcomatous hyperplasia? In order to support these hypotheses, fresh tissue lymphocyte typing tests covering the entire field of digestive lymph pathology should be carried out to complete the intracytoplasmic immunofluorescent study of the immunologically competent cells.

[Electron microscopic study of the sera of hepatitis B surface antigen (HBs Ag) subjects (author's transl)].

Electron microscopic observation of sera from 59 subjects(HBs Ag-negative controls, HBs Ag-positive asymptomatic blood donors, and HBs Ag-positive patients with acute viral hepatitis, chronic persistant or active hepatitis and alcohol-induced liver damage) was capable of specifying the morphology of the HB virus (spherical particles, rod-shaped particles and Dane particles) and of correlating the aspect of the observed viral material with the nature and evolution of the liver damage. The viral particles are observed in small amounts in asymptomatic blood donors and in patients with acute viral hepatitis, but are present in very significant quantities in chronic hepatitis. Dane particles and viral particle aggregates are more numerous in chronic active hepatitis than in chronic persistant hepatitis. The detection of HBeAg in the serum, especially found in chronic active hepatitis, correlates with the presence of Dane particles. In most cases, the observed viral particle aggregates do not seem to be associated with an immunological process.

[Immunofluorescence of the synovial membrane. Study of 100 cases].

A study of the use of immunofluorescence of synovial membranes in one hundred cases of various arthropathies reveals the value of the cytoplasmic localization of immune deposits and their type (Ig) in orienting the diagnosis, the influence of therapy, the lack of correlation with optical anatomopathology of synovial tissue (performed on another tissue fragment). Certain recurrent images such as fluorescence of blood vessel walls or the presence of fibrin are of little interest; other such as the cytoplasmic fluorescence of infiltrating cells and particularly, IgG-IgM mixed cellular fluorescence or coupled fluorescence-cellular with Ig and interstitial granular with Ig and C3 orientate the diagnosis to RA. Immunofluorescence appears to be of interest for inflammatory monoarthritis and probable RA. It is less interesting when the etiological context is limited. In addition, a negative immunofluorescent study does not rule out any diagnosis.

[Characteristic ultrastructural features of parathyroid gland cancer. A report on 9 cases (author's transl)].

Authors have studied 9 parathyroid carcinomas under electron microscope (3% of their parathyroid lesions). Two types of the ultrastructural features were significant: 1) architectural: basal membrane abnormalities, sparse desmosomes, absence of nerve fiber; 2) cytological: nuclei atypism and abnormal mitochondria in tumors cells resembling dark chief cells. Also noticed were signs of cell hyperactivity and/or trouble in parathormon production or regulation.

French mesothelioma register.

A total of 197 definite cases of mesothelioma (mainly of the pleura) have been reported by pathologists to the French Mesothelioma Register for the period 1965--78. A moderate linear increase in the annual incidence of mesothelioma cases since 1965 was noted. In men and women over the ages of 47 and 41 years, respectively, the annual death rate was higher in those with mesothelioma than in those of the general population. The proportions of mesothelioma patients who had experienced occupational, para-occupational, and unknown asbestos exposures were 77%, 3%, and 20% respectively. The number of cases reported to the register is obviously an underestimate of the true number of mesothelima cases in France, because such cases are not reported to the register systematically. Mesothelioma registers cannot provide information about dose-response relationships; however, with good management of the register, such relationships could be obtained, since a good assessment of dose can be accomplished by biologic monitoring.

Polyvinyl chloride pneumoconiosis.

A 53-year-old man, who had been exposed for 23 years to polyvinyl chloride (PVC) in the bagging area of a vinyl chloride polymerisation plant, presented with a diffuse micronodular infiltrate on his chest radiograph. Light microscopy of lung obtained by drill biopsy showed a diffuse infiltration with histiocytes and multinucleated giant cells, with some collagen formation. Ultrastructural studies showed foreign particles in the macrophages, which were identical with PVC powder viewed under the electron microscope. Incubation of PVC powder with human lung macrophages in vitro showed that the macrophages englufed the powder to give a similar ultrastructural appearance.

Electron-microscopic study of the anaphylactic degranulation of human basophils from atopic subjects.

Leukocytes from subjects allergic to Dermatophagoides pteronyssinus were incubated for 20 min with a solution of D. pteronyssinus extracts. Histamine release was measured at 0, 3, 10 and 20 min. Simultaneously, samples were treated for electron microscopy in such a way as to correlate histamine release and the morphological aspects of basophil leukocytes. The principal features accompanying histamine release were: a progressive activation of the cytoplasmic membrane which showed long processes, densification of the mitochondria, fusion of granulations, progressive dissolution and exocytosis of the contents of the granulations, short segments of rough endoplasmic reticulum, active Golgi apparatus, and thin membrane-bound granules suggesting resynthesis of mediators.
