[Dacarbazine in malignant melanoma. Rare, severe side-effect: Budd-Chiari-syndrome (author's transl)].

Dacarbazine (DTIC) alone or in combination with other cytostatic drugs may lead to severe liver disease with acute thrombosis of intrahepatic veins and parenchymal cell necrosis. 6 such cases were described in the literature, an additional case is described here, all with fatal outcome. 6 of these 7 patients had malignant melanoma. The disease has been partly designed as Budd-Chiari-Syndrome (BCS), partly as veno-occlusive disease (VOD).-The pathoanatomical findings and the appearance of the clinical symptoms exclusively during the 2nd injections series suggest a Budd-Chiari-Syndrome, in terms of an allergic-hyperergic reaction of the liver as target organ. For future administration of Dacarbazine in malignant melanoma (a) patients with pre-existent liver disease should be excluded from DTIC-treatment, (b) the prodromi and cardinal symptoms should be carefully registered and (c) high dosage corticosteroid therapy with fibrinolytic measures should be immediately administered.

XY gonadal dysgenesis: aberrant testicular differentiation in the presence of H-Y antigen.

Six patients with pure nonmosaic 46,XY gonadal dysgenesis (XY GD) and histocompatibility H-Y antigen titers in the normal male range (H-Y+) are presented. Clinical characteristics included a female phenotype, masculine skeletal characteristics, signs of virilization, and primary amenorrhea. All individuals had unambiguous female external genitalia, a hypoplastic uterus, bilateral tubes, and streak gonads. Histomorphologic evaluation of the gonads revealed various abortive testicular and ovarian elements capable of steroid production. Gonadal tumors were found in 4 patients (gonadoblastoma, dysgerminoma, granulosa cell tumor, myxofibroma). Plasma and urinary androgens and basal and stimulated gonadotropin levels were elevated prior to gonadectomy. Data show that the presence of the H-Y antigen per se does not guarantee normal testicular organogenesis. It is hypothesized that defective H-Y antigen binding to its gonadal receptors triggers aberrant testicular differentiation in 46,XY H-Y+ GD.

Vascular tumors in the region of the breast.

Three vascular tumors in the breast region with different degrees of differentiation are presented. The first neoplasm is a haemangiosarcoma (of the vascular neoplasms, these tumors are the type which occur most frequently in the breast). Haemangiosarcomas show an infiltrative growth of atypical blood capillaries, frequently with formation of highly cellular and solid capillary sprouts. Ultrastructurally, the tumor cells are characterized as endothelial, also in the region of the capillary sprouts. The second tumor (an angiosarcoma in Stewart-Treves-syndrome, STS) is characterized by an intensive endothelial proliferation. Solid spindle-celled regions are also found in which the tumor cells correspond to undifferentiated mesenchymal cells, but other cells possessing properties of smooth muscle cells and pericytes may be found. The third tumor corresponds light and electron microscopically to a haemangiopericytoma of the soft tissue. The pericytic character of the tumor cells is most clearly seen in the immediate vicinity of the vessels. With increasing distance from the capillaries, the tumor cells take on the characteristics of fibroblasts. The tumors reflect the diversity of the angioplastic differentiation potential of the mesenchyme.

XY (h-y+) gonadal dysgenesis. Morphological examination of 4 cases by light and electron microscopy.

The gonads of 4 phenotypically female individuals with XY chromosomal constitution and signs of virilisation were examined by light microscopy. Electron microscopic examination was also performed in two cases. Serological analysis of H-Y antigen titer yielded positive results. The matrix of the gonads is shown to be ovarian stroma, in which tubular and follicular structures are embedded. The epithelia of the follicles resemble granulosa cells of the ovary, the tubular epithelia are resemble Sertoli cells. Tubules and follicles both show extensive regressive changes. A varying number of Leyding cells/stroma lutein cells were found in each gonad. The different degree of development of testicular and ovarian structures in the dysgenetic gonads might be explained by a defect of the gonadal specific receptor for the H-Y antigen, this defect varying in time of occurrence, duration and severity.

[Quantitative shifting of intra and peritumoral cell populations after intratumoral BCG-cell-wall therapy in squamous epithelial mouth neoplasms].

A qualitative histologic examination of squamous cell carcinoma of the buccal cavity was made before and after the injection of BCG cell walls directly into the tumor. The density of the inflammatory infiltrate in and at the tumor site differed significantly. The distribution pattern of the cells involved in the infiltrate showed an increased number of lymphocytes and histiocytes. Lymphocytes, the largest cell group represented, were more frequently demonstrated in the tumor than outside it. The injection of BCG cell walls directly into the tumor primarily causes a shift in the total number of cells and only secondarily an alteration in the distribution pattern of the cells involved in the infiltrate.

[The ultrastructure of ganglia and newly formed tendon sheaths].

Comparative electron microscopic examination of ganglions (from the tendon sheaths and joint capsules of the hand) as well as that of newly formed tendon sheaths (after resection of digital flexors and their tendon sheaths) indicate that both are the result of an adaptive effect of the mesenchyma which enables the formation of a new synovial organ in the presence of an adequate stimulus.

[Peritoneoscopic diagnosis of M. Whipple--making an indirect diagnosis (author's transl)].

A case report is given of a patient with a malabsorption syndrome. Primary diagnostic procedures including x-ray examinations and endoscopy yielded no results. Alterations of the abdominal lymph vessel system were found during peritoneoscopy giving rise to the hypothetical diagnosis of Whipple's disease. This diagnosis was confirmed by intestinal biopsy yielding macrophages with PAS-positive inclusion bodies. It is pointed out, that peritoneoscopy is a good means to evaluate the abdominal lymph vessel system.
