[Functional and morphologic studies on the reinnervation of the replanted rabbit external ear].

Disturbances observed int he thermoregulation of replanted extremities were the stimulus for this experimental study. We chose the auricular conchae of rabbits as an experimental model, as these show--as do the fingers--innervation with the sympathic nervous system and fulfil thermoregulatoric functions. Twenty-five of thirty auricular conchae of rabbits took completely after amputation and replantation. Blood flow measurements with 133 Xenon, thermographic and histologic examinations were performed. Thermography showed dysfunction with temperature provocation even one year after replantation. Histological examinations revealed partly deficient reinnervation in vessels and also in cutaneous appendages.

[Functionally satisfying reconstruction of a combined abdominal, chest wall and diaphragmatic defect with corium following sternum tumor resection].

In a patient, suffering from a chondrosarcoma in the lower sternum, and having undergone radical tumor resection, we had to reconstruct the thoracic wall, the abdominal wall, and the diaphragm. This was done with autologous corium. We report on the advantages of this material and at the same time refer to that of a complete physiologic reconstruction of the diaphragmatic dome seems impossible if accompanied by defects of the thorax and the abdominal wall. Fitting a corset appears necessary as an external support.

[Preservation and reconstruction of the thumb with the aid of microvascular surgery (author's transl)].

Report about two cases of severe injuries to the thumb. One severe crush injury was treated primarily; in the other case of complete amputation a late reconstruction was performed. Methods and modern concepts for these problems are discussed. The importance of long vein grafts in microsurgery is pointed out. The advantage of reconstructing all structures primarily is stressed. Good functional results are presented to support these concepts.

[Partial reconstruction of the lower eyelid].

Reconstruction of the lower eyelid after a full-thickness loss can be accomplished with flaps and free grafts. Results of the use of pedicle flaps from the upper eyelid to rebuild the lower eyelid are described.

[Reconstruction of abdominal wall defects using corium. Surgical procedure, clinical results and manometric examination of postoperative abdominal wall function].

Extensive resection of the abdominal wall was performed on six patients to reconstruct defects caused by tumors or necroses. Autologous dermis covered by a flap plasty produced good clinical and functional results, documented by the measurement of intraabdominal pressures in response to coughing, pressing, and lifting of the legs. The autologous dermis graft represents a valuable tool for the closure of large defects of the abdominal wall.

Scanning electron microscopic study of hyperacute rejection in the inbred rat.

Twenty skin-presensitised Lewis rats received kidney transplants from (Lewis X BN)f1 rats. Two grafts each were withdrawn at intervals from 1--120 min and examined using a scanning electron microscopic (SEM). A series of Lewis to Lewis isografts served as control. In hyperacute rejection at just 1 min spider-like fibrin fibres and platelets could be observed in small arteries, where the endothelium was severely altered. In these regions at 2 and particularly 5 min a fibrin network often contained platelet aggregates, mechanically altered erythrocytes as well as different kinds of leucocytes. This coagulation process progressed with time and resulted in a complete vascular occlusion at 30--60 min.

[Free transplantation of a fibula with epiphysis].

Problems of free bone transplantation, as well as those of bone transplantation by microvascular anastomoses are exemplified by a case of free fibula transplantation--the epiphysis being conserved--by microvascular anastomoses. If revascularisation of the bone transplant is immediately achieved, the chance for survival of bone tissue is improved and there is a chance that the epiphysis will retain the capacity to grow. By multiple surgical interventions the stability of the abnormally mobile forearm was reestablished and considerable lengthening of the shortened skeleton was achieved. By tendon and nerve transplantations simple grasp could be restored to the hand which originally was without function.

[Impairment of the skeletal growth of the hands after electrical burns in childhood].

Fourteen children with electrical burns involving 33 fingers were followed for a period of more than 8 years. Primary treatment was conservative in 17 fingers, of which 11 required surgery at a later date. Adequate function was restored in 12 fingers after completion of this treatment and persisted for 8 years and more. In 5 fingers substantial joint deformities developed. For 16 fingers surgery was required as primary treatment. Bone involvement was present in 10 of these. Secondary surgery proved to be necessary in 2 fingers, on one aseptic necrosis necessitated replacement by a bone graft. All of the 16 fingers showed good function in the period under review. There was no radiological evidence of deformities, but growth was substantially slower than normal. Apparently coverage of exposed bone with well vascularized tissue improved local blood flow so that damage other than that originally produced by the electrical burns was prevented.

[Familial occurence of Martin-Gruber anastomosis].

1. A Case of MARTIN-GRUBER anastomosis detected intraoperatively is reported. 2. The two children of the above patient also showed a MARTIN-GRUBER anastomosis. The phenomenon was proved by EMG and local anaesthesia.

[Critical considerations of various surgical methods for correction of the syndactylia (report on an observation period of 10-20 years)].

During the postoperative period of 10 to 21 years 58 syndactylies in 45 hands of 30 patients were followed-up with special attention to alternations due to growth. These were prevented in the majority of the patients, if the skin cover for the commissure was done by both a dorsal and palmar triangular skin flap and for the side areas of the fingers by several transversely oriented full thickness skin grafts, separated by bridges of the finger skin. In none of the 37 syndactylies in which this procedure was performed, was a corrective operation necessary at the end of the growth period. These results harmonize with previous follow-up studies (MILLESI 1964). The observations of these patients for an additional 10 years have not shown any alterations due to growth.

Histomorphologic and volumetric analysis of implanted autologous preadipocyte cultures suspended in fibrin glue: a potential new source for tissue augmentation.

Previous efforts to use adipocyte transplants for tissue augmentation have been limited by high and unpredictable resorption rates. Preadipocytes are precursor cells that are capable of replication and differentiation into mature adipocytes. Furthermore, they are more resilient to ischemia, making them a desirable transplant media. Utilizing fibrin glue as a transport vehicle and a prefabricated intramuscular capsule pouch as the recipient site, we have demonstrated the successful transplantation of cultured preadipocytes without the previously presented resorption sequelae. Histological analysis at 2 weeks has demonstrated establishment of vascular supply and the complete resorption of fibrin glue. Most importantly, using planimetric analysis, volume retention has been demonstrated in implanted areas up to 1 year following implantation. Finally, BrdU labeling has been utilized to demonstrate the lack of increased and uncontrolled replication rate, an index of potentially tumorigenic tissue. In conclusion, we have demonstrated a potentially new and safe source of tissue augmentation in the rat model.
