Computer-aided individual prognoses of squamous cell carcinomas of the lips, oral cavity and oropharynx.

Computer-aided individual prognoses (CIP) is a software-package developed on the basis of an empirical study and can be installed on any IBM-compatible personal computer. The project which went into the making of CIP was called "Prospective DOSAK-study on squamous cell carcinomas of the lips, oral cavity and oropharynx". In the course of the study 1485 patients were treated between 1977 and 1982, and followed up through 1985. CIP facilitates individual prognoses and comparisons of independent patient groups with parallel groups from the data of the above-mentioned study. In practical clinical work individual prognoses allow exact and reliable judgements on individual patients. In clinical cancer research it provides the information about prognostic factors required for controlled clinical studies. The comparison of independent patient populations allows for an ongoing qualitative control of the patients in each clinical institution. In clinical cancer research such a comparison means that certain characteristics of the patient, the tumor and of the disease can be given higher prognostic value. The same is true of the clinical testing of therapeutic measures which is typically carried out during phase-II-studies. Due to its menu-based organization CIP does not presuppose any specialist knowledge on the part of its users and can be regarded as particularly user-friendly.

Biostatistical aspects of adjuvant therapy assay.

The specific problems of biostatistical evaluation of adjuvant therapy are discussed. These problems arise from the fact that the effect of adjuvant therapy is usually rather small; in particular, for many kinds of cancer it is smaller than the variability in prognosis among patients due to different prognostic factors. Knowledge about prognostic factors for a particular type of cancer is therefore an essential precondition for the correct planning of controlled clinical trials. The importance of prognostic factor studies is stressed and it is shown how results of such studies can be utilized for the design of efficient trials for adjuvant therapy assay. The results of the retrospective DOSAK study on carcinomas of the oral cavity are reported. Based on a sample of 1021 patients in this study, numerous prognostic factors were analysed by both univariate and multivariate statistical techniques. On the basis of these analyses, a prognostic index was developed which should be a valuable aid for the design of clinical trials.

Retrospective DOSAK Study on carcinomas of the oral cavity: results and consequences.

In a multicentre, retrospective observational study on carcinomas of the oral cavity, including the lips and oropharynx, data material of 1021 patients has been analysed. The specific goals of this study were: Review of existing proposals for classification. Analysis of prognostically relevant factors of the tumour disease. Construction of a prognostic index for the determination of individual and collective prognoses. The following results were achieved: ad 1: All existing TNM-Classifications of oral cavity carcinomas so far fail to meet the requirements of the necessary criteria. ad 2: The multivariate analyses of prognostically relevant factors were performed with and without taking therapeutic factors into account. The results show unequivocally that reliable prognoses are only possible if various treatment modalities are considered. ad 3: This led to the construction of the treatment-dependent prognostic index TPI, which will be eligible for use in clinical-therapeutic cancer research and in clinical practice.

The doubtful relevance of nonspecific immune reactivity in patients with squamous cell carcinoma of the head and neck region.

Nonspecific immune reactivity (lymphocyte stimulation with PHA, PWM; % T-cells; absolute T-cell levels; skin reactivity to DNCB) was determined in 30 patients with squamous cell carcinoma of the head and neck region and in 30 age- and sex-matched healthy controls. The tests were carried out in each patient at 4-week intervals for at least 1 year. The tumor patients were regularly controlled over a period of 5 years. A reduced nonspecific immune reactivity was detected for tumor patients as compared with healthy controls. However, there was no correlation between the follow-up tests on unspecific immune reactivity and the clinical course of the disease. Moreover it was not possible--on the basis of pretherapeutic unspecific immune reactivity and 5 years' clinical follow-up--to make any prognostic statement for the tumor patients tested.

The prognostic relevance of various factors at the time of the first admission of the patient. Retrospective DOSAK study on carcinoma of the oral cavity.

In a retrospective study, on a sample of 1021 patients with carcinoma of the lips, oral cavity and oropharynx, the clinically available factors at the time of the patient's first admission are analysed with reference to their prognostic relevance. Prognostically relevant factors are determined by both univariate and multivariate analyses. Of the 18 factors analysed, the following seven finally proved to be prognostically relevant: tumour size, degree of infiltration, degree of histological differentiation and site by organ of the primary tumour, the combination of evidence + clinical appearance + degree of fixation of the regional lymph nodes, age of the patient, and evidence of distant metastases. On the basis of these results it will be possible to create a prognostic index. This prognostic index should be eligible for use in clinical practice, as opposed to usual classification models.

[Retrospective DOSAK study of oral cavity carcinomas. Results and consequences].

In a multicentric, retrospective observational study on carcinomas of the oral cavity including the lips and the oropharynx a data material of 1021 patients has been analyzed. The specific goals of this study were: Review of existing proposals for classification; Analysis of prognostically relevant factors of the tumor disease; Construction of a prognostic index for the determination of individual and collective prognoses. The following results were gained: None of the TNM-classifications of oral cavity carcinomas does the necessary criteria justice not even approximately. The multivariate analyses of prognostically relevant factors were performed with and without taking therapeutical factors into account. The results unequivocally show that reliable prognoses are only possible under consideration of various treatment modalities. This led to the construction of the treatment-dependent prognostic index TPI, which will be eligible for the use within the clinical-therapeutical cancer research and within the clinical practice.

The use of the fibrin adhesion system for local hemostasis in oral surgery.

In 144 patients with bleeding disorders and in 11 patients with normal coagulation, 517 wound sealings were performed using the fibrin adhesion system (FAS) with the primary object of local hemostasis. Excellent results were obtained in patients with bleeding disorders caused by impaired thrombocyte function or anticoagulant therapy. Replacement therapy or the withdrawal of anticoagulant medication was thereby avoided. Ultimate hemostasis in patients with hemophilia was achieved by fibrin sealing in combination with low-dose replacement therapy with clotting factor concentrates. Though the risk of hepatitis transmission cannot be completely ruled out, the advantages, such as excellent tissue tolerance, complete resorption, and a wide spectrum of practical uses, speak in favor of the use of this physiologic tissue adhesive.

[A retrospective study of carcinomas of the oral cavity: a preliminary report (author's transl)].

UNLABELLED: The German-Austrian-Swiss Research Team on Maxillofacial Tumours is investigating a series of 585 carcinomas of the oral cavity, lips and oropharynx by biostatistical methods. The aim of the study is the determination of relevant prognostic symptoms of oral cavity carcinomas, classification of the series on the basis of these symptoms, the determination of therapeutic efficacy and the assessment of collective and individual prognosis. PRELIMINARY RESULTS: 1. Factors of prognostic relevance are probably only tumour size and intensity of regional metastases. 2. The classification of the series becomes problematic due to the lack of relevant prognostic factors.

[Animal experiments for intra-arterial chemotherapy with bleomycin (author's transl)].

Intra-arterial chemotherapy for treatment of head and neck carcinomas is performed on the basis of clinical reports, which postulate higher effective concentrations at the tumor for this therapeutic method than after systemic administration. This assumption, which has so far not yet been confirmed experimentally, has led to the present study. The study is concerned with different modes of application of bleomycin with the aim to achieve high cytostatic concentrations at the site of action required. In sheep the tissue concentration of 57Co-bleomycin was determined in a hypothetical tumor area, the planum buccale, and in the lymph nodes draining this area, after submucous administration (hypothetical tumor area), intra-arterial administration (A. transversa faciei, A. temporalis superficialis, A. carotis externa) and i.v. administration (V. saphena). Only submucous injection led to significantly increased 57Co-bleomycin activities in the hypothetical tumor area and in the corresponding primary draining lymph nodes. The intra-arterial administration into the A. temporalis superficialis and A. carotis externa did not show any differences compared to the i.v. injection of 57Co-bleomycin. Compared to i.v. administration, injection into the isolated A. transversa faciei led to a slightly increased concentration of radioactively labeled bleomycin, restricted to the hypothetical tumor area. The findings made by animals experiments print out that the clinical application of intra-arterial is not justified and favor intratumoral treatment of head and neck carcinomas with bleomycin.

Carcinoma of the oral cavity: on the prognostic significance of the primary tumour site (by organs) in the oral cavity.

The prognostic significance of so-called organ sites was investigated in 585 cases with carcinoma of the oral cavity and lips. For the subsamples studied the numerical distribution of TNM categories, life tables and life table comparisons were computed. This produced the following results: 1. There is no demonstrable difference in prognosis between identical T catagories in the organs of the oral cavity. 2. In some cases there is a significant difference between identical N categories in organs of the oral cavity. In the No category this is, however, attributable to the substantial differences in the numerical distribution of T categories. By contrast, a logical explanation for the computationally demonstrable significant differences in the Nx category is not available. The problem is currently being investigated. 3. An assessment of identical TN combinations in the "organs" of the oral cavity proved to be impossible on account of the inadequate number of cases available. The so-called "organ localization" of primary tumours in the oral cavity need not - at least for the time being - be accorded any prognostic relevance. The findings should, however, be re-examined on the basis of greater numbers.

Carcinoma of the oral cavity: on the prognostic significance of the primary tumour site (by levels and areas).

The prognostic significance of primary tumour site in carcinomas of the oral cavity was investigated in a series of 585 cases. For the subsamples (levels and areas of oral cavity) studied, the numerical distribution of TNM categories, life tables and life table comparisons were computed. This produced the following results: Given an identical extension and analogous metastazising rate, there is no computationally demonstrable difference in prognosis between primary tumours sited at different levels and areas of the oral cavity. The exception are the T1Nx categories, for which a difference exists between precanine and retromolar sites. These results should be re-examined on the basis of a larger series.
