[Family support and quality of life among hemodialysis patients].

OBJECTIVE: Sixty chronic renal failure patients who had received hemodialysis for 3 months or longer were interviewed with the Quality of Life Questionnaire and Social Support from Family Scale. The results were that the hemodialysis patients had a lower of quality of life and there was a positive correlation between family support and the patients' quality of life. The results suggest that both the patients' condition and their family background should be estimated so as to improve the quality of life of the chronic renal failure patients who had been received hemodialysis.

Screening of serum autoantibodies to cardiac beta1-adrenoceptors and M2-muscarinic acetylcholine receptors in 408 healthy subjects of varying ages.

Autoantibodies to cardiac beta1-adrenoceptors and M2-muscarinic receptors have mainly been found in the sera of patients with idiopathic dilated cardiomyopathy (DCM). In order to elucidate the pathological significance of these autoantibodies in DCM, it is necessary to understand their characteristic distribution in a healthy population of different genders and ages. The peptides corresponding to the sequences of the second extracellular loops of the human beta1-adrenoceptor and M2-muscarinic receptors were therefore used as antigens to screen the sera of 408 healthy subjects of different ages (ranging from 0.5 to 85 years). Of 408 sera, 41 (10.0%) and 46 (11.3%) recognized the beta1-adrenoceptor and M2-muscarinic receptor peptides respectively. Of the positive sera for beta1-adrenoceptors and M2-muscarinic receptors, up to 63.4% and 56.5% had both anti-beta1-adrenoceptor and anti-M2-muscarinic receptor autoantibodies respectively. The antibody titres of the positive sera of healthy subjects were all of a low level, with a geometric mean titre of 1:42+/-1.9 for anti-beta1-adrenoceptor antibodies and 1:51+/-1.7 for anti-M2-muscarinic receptor antibodies. The frequency of occurrence of autoantibodies to both receptors in the sera of healthy subjects increased significantly with age. In conclusion, the autoantibodies to beta1-adrenoceptors and M2-muscarinic receptors in the sera of healthy subjects are characterized by a low frequency of occurrence and low titre, with the frequency of occurrence increasing with age.

[Pathological study on thoracic carcinoids accompanied with Cushing's syndrome].

Eight patients with carcinoid tumors (5 from the lungs and 3 from the thymus) associated with Cushing's syndrome were studied. Among them, 5 were male and 3 female with a mean age of 32.5 years. Histologically, all were typical carcinoids. Immunohistochemically, tumor cells were strongly positive of NSE, chromogranin A and ACTH in all 8 cases. Some tumors also positive for S-100, calcitonin, HCG alpha and HCG beta. Various amounts of neurosecretory granules were found in the tumors by electron microscopy and ACTH positive dots were also noticed in some of the neurosecretory granules in the tumor cells by immunoelectron microscopical colloidal gold labelling technique. By in situ hybridization technique, overexpression of chromogranin A mRNA was detected in all of the tumors reported.

[Analysis of the characteristics of human papilloma virus infection in 85 neoplasms of the respiratory system in adult patients].

Eighty five neoplastic tissues of respiratory system in Chinese adult patients were searched for HPV by multiple PCR and in situ hybridization. In 13 of the specimens examined, HPV DNA was identified (15.3%), including 6 of 36 cases of pharyngolaryngeal neoplasms (16.6%) and 7 of 49 cases of lung squamous cell carcinomas (14.2%). Among the 13 cases tumor specimen, most of them were positive for HPV6/11 types and a few were double positive for HPV 6/11 and 16 types. Many tumor cells positive for HPV DNA on in situ hybridization were morphologically similar to those koilocytes in genital condylomas. HPV infection in 85 neoplasms was characterized by the fact that the same genotypes of HPV were detected in upper and lower respiratory tract and that the dominant virus types detected were HPV 6/11 types. The detection rate of HPV was a little higher in the upper than that in the lower respiratory tract and the HPV DNA positive tumors had similar histological appearance. The results suggest that HPV infection in the neoplasms of the upper and the lower airways is related to each other with similar mode of transmission.

[A study of human papillary virus infection by in situ hybridization and histopathology in squamous cell carcinoma of the lung].

Paraffin embedded archival tissue from 7/49 carcinomas of the lung which were positive for HPV DNA examined by multiple PCR were again searched for HPV DNA by in situ hybridization technique. Five cases were found positive for HPV DNA by in situ hybridization, of which three cases were positive for HPV 11, One was positive for HPV 16 and the other was positive for both HPV 11 and 16. The hybridization signal was mainly located in the nuclei of the koilocytotic tumor cells. These results suggest that HPV infection may have some role in the development of squamous cell carcinoma of the lung.

[Detection of human papillomavirus DNA in squamous cell carcinomas of the lung by multiple polymerase chain reaction].

49 cases of formalin-fixed paraffin-embedded tissues of squamous cell carcinomas of the lung were searched for the presence of human papillomavirus (HPV) DNA by multiple polymerase chain reaction (PCR). Totally seven cases were positive for HPV DNA (14.2%), Among them four cases were positive for HPV6/11, one case was positive for HPV16 and two other cases were double positive for HPV6/11 and 16. These results suggest that HPV infection might play some role in development of squamous cell carcinomas of the lung.

Thoracic ectopic ACTH-producing tumors with Cushing's syndrome.

During a 5-year period (1986 to 1990), 7 patients with thoracic ectopic ACTH-producing tumors (4 pulmonary and 3 thymic) associated with Cushing's syndrome were diagnosed and treated in PUMC Hospital, Beijing. Among the 7 patients, 4 were male and 3 were female, with a mean age of 35.7 years. The duration of Cushing's syndrome before operation ranged from 6 months to 72 months. Histologically all tumors were typical carcinoids. Various amounts of neurosecretory granules were found in the tumor cells of all tumors by electron microscopy. Immunohistochemically all tumors were strongly positive for chromogranin A and NSE. The immunopositivity for ACTH was variable in different tumors. Some tumors were also positive for S-100, calcitonin, HCG alpha and HCG beta. By in situ hybridization technique, overexpression of chromogranin A mRNA was detected in all tumors.

The role of flow cytometry and morphometry in discriminating benign from malignant nonfunctioning pancreatic endocrine tumors.

Nonfunctioning pancreatic endocrine tumor (NFPET) is a kind of tumor with high malignancy but bland in morphological features. It is impossible to predict the malignancy solely by gross and light microscopic pictures of the tumor. For the purpose of assessing the value of flow cytometry and morphometry in predicting the prognosis of NFPET, the DNA content and several morphological parameters of 26 NFPETs were determined by appropriate techniques. The results show that abnormal DNA content has no practical value in discriminating benign from malignant NFPETs; whereas the "score" obtained by combining the morphological measurements of the number of nuclei/mm2 and N/C, is of certain referential significance in predicting the malignancy of NFPET.

Vacuole sign and nodule sign in early peripheral bronchogenic carcinoma. Diagnostic value and pathologic correlation.

We analyzed the radiological and pathological characteristics of 30 peripheral lung masses 1.5-3 cm in diameter. We describe the vacuole and the nodule sign. Retrospective examination of 100 conventional tomograms of peripheral pulmonary masses and comparison with the histological diagnoses allowed us to determine the usefulness of these two signs in differentiating small peripheral bronchogenic carcinomas from tuberculomas.
