[Eosinophilic esophagitis associated with recurrent urticaria: is the worm Anisakis simplex involved?].

Anisakis simplex, a fish parasite of the nematode family, typically infects marine mammals such as whales, dolphins and seals. Human anisakiasis, which is acquired by eating raw or insufficiently heated fish or squid, has gained world-wide importance. Infestation with living larvae caused by eating parasitised fish results in acute upper abdominal pain, nausea and vomiting and may be confused with acute abdomen due to appendicitis and other inflammatory abdominal disorders. Extraintestinal organ manifestations are rare. Endoscopically, inflammation, oedema, erosions and ulcerations may be found. The parasite can been found in up to 50% of patients. Histologically, an eosinophilic inflammation is typical. Acute anisakiasis may be prevented by thorough cooking or deep-freezing the parasitised fish for at least 48 h. IgG-antibodies specific for Anisakis simplex are thought to represent an immunological host reaction against parasitic antigens. More recently, allergic reactions to Anisakis ingestion or exposure, such as urticaria, anaphylaxis and even occupational asthma, have been reported. These allergic reactions may also occur when the fish has been properly cooked, and hence these allergens are thought to be heat-stable. Such cases may be diagnosed by skin tests and the determination of specific Anisakis-IgE. However, the specificity of IgE is low, since they may also be present in exposed asymptomatic individuals. Since the eliciting allergens are temperature-stable, prophylactic dietetic measures are indicated. We report a case from Switzerland acquired during a holiday in Portugal. The patient suffered from recurrent dysphagia and urticaria, and histologically eosinophilic oesophagitis was found. IgG-antibodies and a positive skin prick test to Anisakis simplex support its aetiologic role for the symptoms.

Cardiac metastasis of an epitheloid leiomyoblastoma of the lung.

A 73-year-old patient underwent an extended pneumonectomy on the left side on account of a central malignant epitheloid leiomyoblastoma of the upper lobe. During this operation a tumor invasion of the pulmonary vein was discovered by chance. During the postoperative course the patient suffered from a tumor-caused morbus embolicus which led to his death. The autopsy showed cardiac metastasis. Four aspects are discussed: the morphology and biology of the malignant leiomyoblastoma, the incidence of cardiac metastases and their clinical diagnosis as well as the intraoperative embolism of tumor masses during lung resections.

[Unusual intestinal manifestations of tuberculosis].

In an 87-year-old Swiss female referred with complaints of bloody diarrhea and weight loss, colonoscopy revealed three ulcers in the rectum and colon. Cultures from the colonic ulcers were positive for Mycobacterium tuberculosis. There was no evidence of pulmonary infection. One week after adequate therapy was begun, a perforation occurred at the rectosigmoid junction. The sigmoid was resected and left-sided colostomy was performed. Seven days after surgery the patient died. Clinical features, diagnosis and morphological changes of intestinal tuberculosis are discussed.

Carcinosarcoma of the prostate in combination with adenocarcinoma of the prostate and adenocarcinoma of the seminal vesicles. A case report with immunocytochemical analysis and review of the literature.

A unique triad of a carcinosarcoma and an adenocarcinoma of the prostate as well as an adenocarcinoma of the seminal vesicles in a 67-year-old man is reported. The carcinosarcoma was investigated by immunohistochemical methods. The mesenchymal parts of the sarcoma showed chondromatous differentiation expressed by S-100 protein and vimentin. The undifferentiated epithelial elements of the carcinosarcoma reacted positively to the panepithelial antibody lu-5 and to carcinoembryonic antigen (CEA). The adenocarcinoma of the prostate stained with antibodies to panepithelial antigen (lu-5), prostatic acid phosphatase, prostate specific antigen and CEA, whereas the adenocarcinoma of the seminal vesicles reacted only to lu-5 and CEA. The literature is reviewed.

Primary duodenal carcinoma arising in a non-vaterian tubulo-villous adenoma. A case report with immunocytochemical analysis and review of the literature.

Primary duodenal carcinoma and duodenal adenoma are rare tumours. Duodenal carcinoma makes up about 0.3% of all malignant tumours of the gastrointestinal tract (Alwmark et al. 1980; Spira et al. 1977). The present paper describes a duodenal carcinoma arising in a mixed tubulo-villous non-Vaterian adenoma in a 68 year old male. Immunocytochemical analysis revealed evidence of neuroendocrine differentiation in both adenoma and carcinoma. In a review of the literature a correlation between the size of adenoma and the probability of concomitant carcinoma is demonstrated. Duodenal adenoma measuring more than 4 cm in diameter should be considered potentially malignant.

The immunohistochemical reactivity of a new anti-epithelial monoclonal antibody (MAb b-12) against breast carcinoma and other normal and neoplastic human tissues.

A mouse monoclonal antibody, MAb b-12, has been described previously (Stähli et al. 1985) which reacts with a Mr 350 kD glycoprotein with mucin-like characteristics (Stähli et al. 1987) expressed in cytoplasm and on the surface of human breast carcinoma cell lines (MCF-7 and ZR-75-1). In the present report the immunohistochemical reactivity of this MAb with normal and malignant human tissues is analyzed. Pre-experiments showed that the epitope b-12 is resistant to formalin treatment allowing the use of tissue processed by standard paraffin embedding methods. 167 normal and 408 neoplastic tissues were tested by indirect immunofluorescence or the avidin-biotin complex method. MAb b-12 stained the apical cytoplasm of secretory epithelia and their secretions including the acinar and ductular epithelia of the breast. It reacted with all breast carcinomas independent of their histological type or stage, frequently with all but in some cases with a fraction of the tumour cells. Some other carcinomas, primarily those of adenomatous differentiation, were also reactive. In these, however, the fraction of positive tumour cells was usually lower. The b-12 epitope is thus a marker for normal and neoplastic epithelia with secretory functions, particularly for breast carcinomas of all histological types and stages, and perhaps a differentiation marker for abortive adenomatous differentiation in solid carcinomas of the gastro-intestinal, uro-genital or respiratory tract.

Production of the alpha-subunit of glycoprotein hormones by pituitary adenomas.

Positive immunoreactions with alpha-subunit antibodies were present in 43% of biopsy samples obtained from 147 subsequently operated pituitary adenomas representing all major endocrine types (57 endocrine inactive adenomas, 43 prolactinomas, 30 adenomas causing acromegaly, and 17 adenomas causing Cushing's disease or Nelson's syndrome). Marked variations of the incidence, however, were found among the individual endocrine groups. Positive reactions were present in 63% of endocrine inactive adenomas, 57% of adenomas causing acromegaly, 35% of ACTH-secreting adenomas, and 9% of prolactinomas. A positive alpha-subunit reaction was accompanied in a minority of cases only with positive glycoprotein hormone-beta-subunit reactions. There were 21 pure alpha-subunit adenomas in the group of endocrine inactive adenomas.

Immunocytochemistry of pituitary tumors.

Pituitary tumors from 376 patients were investigated, using immunocytochemical techniques at the light and electron microscopic level, and autoradiography combined with immunocytochemistry for localizing somatostatin (SRIH) receptors. Prolactinomas, growth hormone-secreting adenomas causing acromegaly, and hormonally inactive adenomas were most frequently observed (153, 86, and 90 tumors, respectively). Among the latter, we could distinguish "alpha-only adenomas," many of which were oncocytomas. At the light and electron microscopic levels, cells containing (and presumably producing) simultaneously both prolactin and growth hormone, and cells containing exclusively either prolactin or growth hormone, could be demonstrated. In addition, a highly variable number and distribution of SRIH receptors could be shown in tumors secreting prolactin, growth hormone, and in tumors not associated with symptoms caused by inappropriate hormone secretion. The systematic combination of clinical, radiological, and biological techniques has currently brought great progress in the behavior and therapeutic concepts of pituitary lesions, and promises new achievements in the near future.
