Long-term follow-up of chimerical state of the patients transplanted for different haematologic diseases.

Long-term follow-up of peripheral cellular chimerism in patients treated with BMT or PBSCT revealed the usefulness of their continuous monitoring at molecular level. Our results are based on monitoring of 120 patients, who were followed for at least 24 months. Comparison of the patients treated for chronic myelogenous leukemia (CML), acute myelocytic leukemia (AML), acute lymphocytic leukemia (ALL), myelodysplastic syndromes (MDS) and aplastic anaemia (AA) revealed that mixed chimerism was practically absent in MDS and relatively long-lasting in ALL and AA (regardless to substantially different post-transplantation treatment). The first disease relapses signalized by molecular checking of mixed peripheral chimerism were observed also after a period of remission lasting for several years. Molecular watching enables us to detect relapses at their very beginning that would remain hidden to less sensitive methods. We believe that all of the transplanted patients ought to be monitored for residual disease i.e. cellular chimerism using molecular methods without time limits. On the other hand low level of mixed cellular chimerism is not necessarily a sign of disease progression and can remain unchanged as "status quo" for a very long period.

Successful treatment of a critically burned patient (case report).

The objective of this study was to document that treatment of a critically burned patient could be, although with some problems, ultimately very successful. The commonplace of such successful therapy is certainly not only current, up-to-date and most intensive care, but also good teamwork and professionalism of the whole team in the specialized workplace--Burn and Reconstructive Surgery Departments of the Faculty Hospital in Brno.

Unusual course of treatment of a patient diagnosed with Pemphigus Vulgaris (case report).

The case report is describing patient with an autoimmune disease, Pemphigus Vulgaris. This patient arrived to the general practitioner with Pemphigus Vulgaris after 20 days from the first manifestation of the disease. Despite intensive care, patient dies forty-seventh day after first clinical manifestation of the illness of candidemia. Candidiasis was diagnosed only in postmortem examination.

Reconstructive surgeries after extensive burns in children.

The authors' aim is to summarize and generalize the knowledge acquired in the course of the last eight years of work with children with extensive burns. Reconstructive surgeries show a certain specific feature consisting in the considerable extent of the scarred area often exceeding 50% of the body surface, and with the gradual increase in number of operations they are gaining importance.

Successful treatment of a critically burnt neonate (case-report).

In a health institution in 2000, a premature neonate suffered critical burns. In the submitted case-report the authors describe the successful treatment of this quite exceptional case. Indirectly, they thus also draw attention to the very positive role played by interdisciplinary collaboration under such unusual circumstances.

Quality of prehospital management of patients with burn injuries--a retrospective study.

The authors evaluate the quality of burn patient management prior to admission to a specialized department. The most frequent mistake was found to be inadequate airway management (no intubation) and a lack of an i.v. line and volume resuscitation. These problems could be seen especially in the patients admitted to the specialized department as secondary transfers, following initial treatment in another healthcare facility. Prehospital care of the patients admitted as primary transfers was found to be satisfactory in the majority of cases. The authors conclude that some mistakes in diagnosis may be caused by the development of the burn over time as well as by the fact that certain clinical experience is necessary for determining the correct diagnosis. The predominant cause of inadequate management in patients with severe thermal injuries is underestimating the severity of the injury and ignorance of the possible consequences of inadequate management of the patients for transfer.

The most frequent problems in the initial stage of burn management.

On the basis of three case reports discussed in the article, the authors demonstrate the most frequent serious drawbacks occurring in the initial stage of care of severely burned patients. The first case report highlights the importance of correct determination of the extent of the burn trauma, which should be correctly assessed by every first contact physician. The second case report demonstrates the need for adequate management of patients with severe burn trauma (especially airway management and i.v. establishment). In the third case (a little baby) again the need for correct patient assessment, timely airway management and beginning of resuscitation and especially of appropriate and immediate transfer of the burned patient to the specialized burn facility are highlighted.

Special features of burn injuries in elderly patients.

Types of burns and other aspects of burn injuries and case outcomes were assessed in a group of geriatric patients (> 60 years) and a younger group of patients (40-59 years). Between 1990 and 1999, 137 geriatric patients (47 [34%] males and 90 [66%] females) were admitted to the Burn Centre and Reconstructive Surgery Centre at University Hospital in Brno. We compared findings in this elderly group to those in 176 younger burn patients (126 [72%] males and 50 [28%] females) who were treated at the centre during the same time period. Age and concomitant chronic disease contribute to the high mortality and a higher frequency of complications in geriatric patients who suffer burn injuries. In this study, the complication rates for geriatrics during hospitalization (44% in males and 32% in females) and the elderly patients' mortality rates (26% in males and 17% in females) differed statistically from the corresponding rates in the younger patient group. It is important to know the special needs of elderly burn patients because this patient group is expected to grow in parallel with the rising average age of the Czech Republic's population.

Inhalation injury.

Inhalation injury is an acute insult of the respiratory tract, caused by steam or toxic inhalants. A suspicion of inhalation trauma (closed-space exposure, facial burns, etc.) is an indication for an immediate endotracheal intubation. Precise objective case history is also very important point for making the diagnosis. Up-to-date methods of examination in case suspicion of inhalation injury are described in our contribution. The main therapeutical points are mentioned as well.

Cultured epidermal allografts for the treatment of deep dermal burns.

The paper summarizes the experience with the application of cultured epidermal allografts in the treatment of deep dermal burns. In a series of patients, including 10 adults and 32 children this method allowed to attain an epithelialization in 90% of deep dermal burns. The advantages of this therapeutic method consist particularly in the early epithelialization of the burnt area and the maintenance of all vital layers of the corium. The risk of a transmission of infection by the transplantation of allografts of live human cells can be minimalized by keeping all directions prescribed for the examination of donors. The possibility of cryopreservation and the easy transport of cultured epidermal allografts renders them available for the use as a new therapeutic method in the treatment of deep dermal burns.

[Use of cultured epidermal autotransplantation in a severely burned patient].

In the submitted paper the authors describe in detail the administration of cultivated epidermal autotransplants (CEA in a patient with severe burns (80% of the body surface). They draw attention to several problems which occurred during treatment: a) a relatively small overall increase of CEA and the long time required for their cultivation, b) the very difficult preparation of infected wound surfaces for transplantation, c) secondary healing of CEA. The solution of these problems is according to the authors optimization of the laboratory technique of CEA cultivation and substitution of the dermal component of skin.
