Oral inflammatory pseudotumour: immunohistochemical investigation of a case involving the submandibular gland and review of the literature.

Inflammatory pseudotumours (IPs) are rare lesions. Most commonly reported in the lung, they are almost ubiquitous, but few oral cases have been described. Their rapid growth, local invasiveness and recurrence, and their ultrasound, computed tomography (CT) and magnetic resonance imaging (MRI) aspects are confusing and mimic benign or malignant neoplasms. Their recognition and distinction from malignant tumors is of importance but their histopathological diagnosis may represent a challenge. In the case reported involving the submandibular gland, the spindle cells had the immunohistochemical profile of myofibroblasts, broader cells with a larger nucleus were CD68 and/or Mac387 positive and the dense plasmacytic infiltrate was polyclonal. Histopathology of IPs covers a spectrum of appearances according to the cellularity and the degree of fibrosis. The recognition of a variable mixture of three main cell types: histiocytes or macrophages, myofibroblasts or fibroblasts and abundant plasma cells, with low mitotic activity and absence of cytological abnormalities in an ill circumscribed and rather fibrous lesion is recommended for the diagnosis of oral IP.

[Video-assisted endoscopic lifting: development].

Now the Endoscope is used differently in video-assisted Face-lifts. Authors describe their evolution for 2 years and a half. In the beginning. It was used to correct glabellar frowns. Then, the endoscope served to elevate the forehead by a subperiosteal approach. Sometime it was combined with a subperiosteal malar dissection in an Endoscopic full face-lift. Bad results on the eyebrows and eyelids complications involved the authors to modify their strategy: By changing the dynamic of muscles: lateral section of the orbicularis occuli muscle arises the frontal muscle to definitely lift up the eyebrow: this is called the "myotomy box"; by preferring a subgaleal, retro-orbicularis dissection, under the malar fat pad and the platysma muscle in a deep plane but always above the zygomaticus major muscle, protector of the facial nerve in the buccal space; by developing new endodissectors efficient and less aggressive to spread the deep planes without damage; by an acute control of the fronto-temporal and cervico-facial deep dissection using new endoscopic valves; by treating precise damaged areas with Limited Endoscopic Face-Lifts in a same harmonious deep dissection instead of large multiplanar underminnings.

[Bacillary angiomatosis of the oral cavity in AIDS. A differential diagnosis of mucosal Kaposi's sarcoma].

The diagnosis of bacillary angiomatosis (BA) was performed on a biopsy of a reddish nodular lesion of the gingival mucosa, in a patient seropositive for the H.I.V. It was based on the histopathological features, and by the demonstration of bacilli both on the Warthin-Starry stain and the electron microscopic study. A treatment by erythromycin was quickly effective. BA is an opportunistic infection seen essentially in patients with A.I.D.S., most often causing cutaneous involvement. The causative pathogen is Rochalimaea henselae. BA of the oral cavity is very rare. This infection can be mistaken with Kaposi sarcoma on its clinical appearance.

[Extemporaneous anatomopathologic test of the thyroid body. Value, limitations and practical implications. Report of 1,680 thyroidectomies].

The analysis of the results of 1,680 immediate histopathological studies of thyroidectomy specimens allows assessing the value, limitations and pitfalls of this method. Although its specificity for the diagnosis of cancer is 100%, its sensitivity is 70.5% only. Misinterpretation is accounted for by the difficulty to identify the criteria of malignancy in an immediate study. The experience of a pathologist is essential and it ensures the reliability of the method, which is aimed at allowing an optimum one-stage surgical treatment.

[Fractures of the mandible. A critical study of repair procedures].

Based on their personal experience of 110 patients operated on for mandibular trauma, the authors analyse the results of the various techniques used. Screwed micro-plates may be very useful but only complete the therapeutic arsenal of the surgeon. In fact classical methods continue to provide satisfactory results. Poorer results may occur with fractures which combine involvement of the articular surfaces with a lesion of the body of the mandible.

[Gardner's syndrome. Review of the literature. Report of a familial form].

Through a family form study and a literature review, the authors define the symptoms of the Gardner's disease and the checkup to be carried out. Sometimes, the surgical treatment, proposed in an aesthetical purpose, is difficult to carry out due to the tumor location and consistency. Incomplete forms are likely to be more frequent but the risk of colic degeneration remains the same as for the usual form. Examination and multi-departmental care are required.

[Fractures of the floor of the orbit. A critical study of methods of repair].

Based on a personal series of 160 cases of surgically treated closed orbital floor fractures, the authors discuss the various reconstruction procedures and their respective indications. Emphasis is placed on surgical access and osteoplasty techniques. In this series, both cutaneous upper eyelid (127 patients) and transconjunctival approaches (33 cases) were used. Access through the upper eyelid gave the fewest complications; transconjunctival surgery was reserved for pure "blow-out" fractures. Several materials were tested for osteoplasty: conventional techniques included Silastic sheets (112 cases), corticospongiosal bone grafts (16 cases), and lyophilized dura mater (9 cases); new techniques included use of resorbable thermoplastic sheets (23 cases). Choice of a material depends on the size of the defect to be filled. Thin Silastic sheets are the ideal solution for small defects. Bone grafts are reserved for extensive losses of substance.

[Histological aspects and prognostic significance of the regional immune reaction in carcinomas of the upper respiratory and digestive tracts].

A prospective study of histopathologic data from 195 cervical evidements performed in the Centre Antoine-Lacassagne between 1979 and 1983 evaluated the prognostic value of morphologic criteria reflecting the regional immunity reaction of lymph nodes. Results failed to demonstrate any statistically significant correlation between the various groups, particularly with respect to localization, tumor extension and degree of histologic differentiation. These data are in line with other immunologic criteria, particularly measurement of lymphocyte subpopulations.
