[20 years of subcapital osteotomy of slipped upper femoral epiphysis. Indications and results].

Between 1945 and 1985, 570 patients with a slipped upper femoral epiphysis were treated at our hospital. During the last two decades a subcapital osteotomy was performed if the degree of slip exceeded 50 degrees. Twenty two such operations were carried out in 21 patients. In five hips complete avascular necrosis of the epiphysis occurred, and in another hip there was partial necrosis. The success of the operation depends upon careful preoperative planning and a meticulous surgical technique.

[Arthrolysis of the knee joint].

The surgical treatment of the extensor rigidity of the knee joint described by Payr was performed in fourteen cases at the Orthopedic University Hospital of Giessen. A flexion of more than 90 degrees was achieved in all patients. In case of correct indication, this method has still its value in orthopedia even regarding the changed patient categories.

[Shaft fractures in the growth stage--surgical indications or conservative therapy].

The treatment of fractures in growing children, especially shaft fractures, is subject to special rules compared to the treatment of such injuries in adult persons. Osteosynthesis is only rarely indicated. Marrow nailing involves great risks at the point where the nail is driven in. Therefore it is not indicated in case of open epiphyseal cartilages.

[Femoral fractures near the knee in the growth period].

Fractures of the distal part of the femur in children are difficult to classify as suggested by Aitken, Harris and Salter if the epiphyseal cartilage is injured. Such injuries are rare if seen in relation to the grand total of paediatric lesions. There are late complications, however. In unilateral closure of the epiphyseal cartilage growth will be irregular and there will be differences in leg length. The sequels to knee joint distortions with ruptures of the ligaments are significant. These ruptures will also result in leg malpositioning due to epiphyseal closure. Corrective measures are often necessary to straighten out the axial deviation and to compensate the differences in leg length. The authors' own patient material comprises besides epiphyseal fractures eight such ligamentous lesions with late complications.

[Liability problems in endoprosthetics].

The alloarthroplasty is a successful operation in coxarthrosis. It is a procedure not including alone the possibility of alternative treatment but also many certain and uncertain risks. Patients wishes are not always to realise so increased number of demands are in this way to expect.

Female

[Femoral neck fractures in the growing skeleton].

Prognosis and therapy of fractures of the neck of the femur are identical in children and adults. The results of injuries to the neck of the femur in children are determined by the path of vessels to the femoral head and the neck of the femur. 28 cases have been followed up, partly for 25 years. These observations allow to draw the conclusion that fractures of the neck of the femur in children have to be treated as acute lesions in order to avoid severe late consequences for the hip joint. The fractures of the neck of the femur are classified according to Ratliff. The transcervical fracture is the most frequent injury. Late consequences are treated by correction osteotomy, chip implantation, and boring, according to the extent of necrosis and deformity.

[Differentiated indication for arthrolysis, arthrodesis, arthroplasty].

Actually, arthrodesis and arthrolysis are still valuable as alternative surgical methods in order to improve the functions of damaged joints. Their indication depends upon the local conditions in the traumatized region and the extent of the disease, i.e. uni-or bilateral joint lesion.

Carbon as an implant material in orthopaedic surgery.

Carbon is a material of proven value in many fields of medicine. It has only minor reactions compared with other materials, but does have unfavourable mechanical characteristics. New kinds of carbon with similar physical characteristics to other materials, e.g. steel and ceramic, have now been developed.

Animals
