More frequently after oncological resection, the mandibular reconstruction is done with microsurgically anastomosed free flaps. The titanium implants propose to the patients an oral rehabilitation with best functional and aesthetic results. It is very true in oncological surgery with or without radiotherapy.
Odontogenic fibroma is an infrequent benign tumor, originating in the mesenchyma and including an inactive odontogenic epithelial component. The central form, of which one case is reported here, is expressed by a relatively well-delineated radiolucent area, whose interest and difficulty lie in the differential diagnosis. On the basis of a review of the literature, details are given on the clinical, clinicopathologic, diagnostic and therapeutic data.
Diagnosis of aetiology of facial asymmetry is dependent on results of clinical and complementary examinations. Its most frequent causes are illustrated by several case reports.
The solitary plasmocytoma of the mandible is a rare affection. This article describes the case of a man, 42 years old. A short review of the pathology is done. The treatment (surgery and radiotherapy) with their failures and complications is exposed.
The authors report their experience with the free osseous and osteocutaneous fibular flap used in the complex mandibular reconstruction. After a short review of the mandibular reconstruction possibilities and description of the fibular flap characteristics, the clinical cases are reported and the operative technique used by these authors is detailed. The discussion of the advantages and the disadvantages of this flap in the complex mandibular reconstruction ends the paper.
Clinical and radiologic examinations were carried out after a mean 7 year follow up of functional conservative treatment of a mandibular condyle fracture in 16 patients. Anatomic and functional recovery was excellent in children and was often very good in adolescents. Signs of dysfunction were a frequent finding in adults, but these were not considered as important by the patients and their specific treatment was not required.
This paper present 2 different cases of extra-articular ankylosis of the temporo-mandibular joint by a maxillo-facial presentation of localized and generalized ossifying myositis. A review of the literature about this subject allow the AA, to describe the clinical, diagnostical and therapeutic features of this unusual pathology.
This study, based on the authors' experience, intends to determine the specific indications of mandibular reconstruction after oncological maxillofacial resections. Mandibular defects without osseous interruption do not necessarily need an osseous reconstruction: the esthetic and functional problems can be resolved using dental osseointegrated implants. On the contrary, mandibular defects with osseous interruption need imperatively an osseous and/or composite reconstruction: the best esthetic and functional results are obtained with an immediate reconstruction by an osseous or osteocutaneous free flap, either from the fibula or the iliac crest. The advantages and the disadvantages of all these reconstruction technics are detailed and largely illustrated.
Adenomatoid odontogenic tumor is observed most frequently in male patients aged between 10 and 20 years. Occurring usually in the anterior part of the upper jaw, its discovery is often fortuitous during routine radiological examinations, which show a well defined clear zone 1 to 3 cm in diameter. Treatment by curettage results in healing by secondary bone cicatrization. A case is reported and epidemiological, clinical, diagnostic, therapeutic and prognostic features reviewed.
The AA. report their experience about 42 of latissimus dorsi flaps, used in the maxillofacial reconstruction of 40 patients, during the last 4 years. After a brief historic and anatomic descriptions, the operative technique used for them is detailed with the solution for the encountered problems. The discussion of the advantages and drawbacks of this flap which should deserve a preferential place in the reconstructive surgery of large losses of the maxillofacial sphere (besides the microsurgical methods) ends the paper.
Two patients with heterotopic gastric cysts are presented and the literature is briefly reviewed. These cysts are histologically characterized by a gastric mucosa with parietal and chief cells, gastric glands and sometimes even have a muscularis mucosae.
This review systematically analyses all of the current data concerning the treatment of oral cancer. Each therapeutic modality involving surgery, radiotherapy, chemotherapy and immunotherapy is detailed and an exhaustive review of the literature about each therapeutic strategy is presented.
The clinical observation of an oral pulse granuloma allows to detail the clinical and pathological characteristics of this lesion. The etiology of this particular granuloma is explained by the introduction of alimentary particles into maxillary bone. A review of the literature about this subject is also made.
Every dentist can meet peculiar radiographic images that require a further investigation by a specialist in buccal pathology. This one can ensure the way to determine for example the tumoral or systemic character of the suspected lesion. Other morphological radiographic appearances include a risk of misinterpretation and thus inopportune, inadequate or hazardous operative procedures. This limited review with selected examples presents some occurrences the practitioner must keep in mind. Finally, this review demonstrates the need of correct radiographic knowledges and applications.
This paper gives a brief description of the aetiology, pathophysiology, diagnosis and treatment of traumatic arteriovenous communications. Traumatic arteriovenous communications of the face and scalp are rare lesions characterized by multiple endothelial-lined channels between the arterial and venous system. A swelling of the face with pulsatile tinnitus comprise the main presenting symptoms. Lesions should be delimited by arteriography unless small and localized. They are managed by complete excision and ligation of arterial feeding vessels. One case of traumatic arteriovenous communication of the face is reported.
The authors report their experience about 42 latissimus dorsi used in the maxillo-facial reconstruction of 40 patients. After a brief historic and anatomic description, the operative technique used by the authors is detailed with the solution for the presented problems. The discussion of the advantages and the disadvantages of this flap ends the paper.
This paper is a review of cranio-maxillo-facial bone malformations, characterized by an abnormal cortical bone density. For each lesion, clinical and radiological cranio-maxillo-facial features are described. Whenever possible, recent etiopathogenic and differential diagnostic elements are included, on the basis of a review of the literature.
