Spinal cord compression due to intradural extramedullary aspergilloma and cyst: a case report.

A case of thoracic spinal cord compression caused by intradural extramedullary cyst associated with aspergilloma is presented. The specific diagnosis was extremely difficult and required specialized tissue studies. Clinical suspicion of fungal infection, surgical intervention for spinal cord decompression, and tissue biopsy and relentless diagnostic laboratory studies are particular features of this case.

The tip of the iceberg: a validation study of the Victorian Congenital Malformations/Birth Defects Register.

Among 1500 children under 4 years of age admitted consecutively to two paediatric teaching hospitals in Victoria in 1985 the prevalence of birth defects was 20%. Fewer than half of these defects had been notified to the Victorian Congenital Malformations Register. Only chromosomal anomalies and, to a lesser extent, major structural malformations present at birth were adequately ascertained. The implications for birth defects monitoring and for service planning are discussed.

2-substituted tetralin derivatives as conformationally restricted butyrophenone analogs.

Five related derivatives of 2-(2-piperidinoethyl)tetralin have been synthesized as partially rigid analogs of the neuroleptic butyrophenones. Their in vitro dopamine receptor binding properties and in vivo inhibition of dexamphetamine-induced locomotor activity are described. 1-Phenyl-2-(2-piperidinoethyl)-3,4-dihydronaphthalene hydrochloride (8) is about 1% as potent as haloperidol.

Community psychiatric nursing: a survey of patients and problems.

The work of a community psychiatric nursing service for acute psychiatric conditions was reviewed for the period 1980-82. Five community psychiatric nurses operated in 10 health centres. General practitioners were the most frequent users of the service and a wide range of psychiatric problems were encountered. The age structure of all referrals was found to be significantly related to sex, with the proportion of females in all age groups at least twice that for males. The most frequent reason for referral was mood/affect disturbance; female patients referred for the first time were predominant. Over half of all problems resolved within three months of referral and a limited number of patients were sent for further psychiatric investigation.

Roentgenologic changes in pulmonary echinococcosis.

Radiologic changes seen in 420 cases of pulmonary echinococcosis are discussed. Criteria for early diagnosis of complicated, infected or ruptured cysts are described. The role and limitations of radiology in approaching the diagnosis of advanced stages of infected hydatid cysts of the lung are emphasized.

Arterial occlusion secondary to methylmethacrylate use.

We are reporting a new complication of the use of methylmethacrylate, namely occlusion of the iliac artery with embolization. We believe that the occlusion is related to trauma to the intima by the heat of polymerization created by methylmethacrylate, with resultant thrombosis. In view of the initial failure of embolectomy alone, we suggest direct attack on the involved artery with prosthetic replacement, and management of the distal embolus by embolectomy with Fogarty catheters.

Spontaneous hemorrhage in previously ischemic (pale) cerebral infarcts.

Spontaneous bleeding into a pale cerebral infarct is documented in 2 patients. Neither patient had received anticoagulants or was hypertensive. The development of the hemorrhage was associated with neurological worsening. Initial CT scans showed a hypodense lesion in an arterial distribution representing infarction; subsequent CT scans showed multiple areas of hemorrhage within the hypodense lesion. The size of the hemorrhages and their location in gray and white matter differ from the pattern of hemorrhagic infarction and primary intracerebral hemorrhage as visualized on CT scan and at autopsy.
