Dual genotype in cutaneous T-cell lymphomas and pseudolymphomas.

Genomic DNA digests of skin biopsies from 20 patients with cutaneous T-cell lymphomas and pseudolymphomas were studied by hybridization, using probes for the constant region of the T-cell receptor beta chain and the joining region of the immunoglobin heavy chain gene. Skin biopsies from all 20 patients contained a monoclonal T-cell population. In addition, DNA from 5 patients contained an immunoglobulin gene rearrangement. These results demonstrate that cutaneous T-cell lymphomas are clonal T-cell malignancies that frequently express a dual genotype, which may sometimes reflect the clonotypic heterogeneity of these disorders.

Four cases of vesicular lesions in adults caused by enterovirus infections.

Echovirus types 4 and 33 and coxsackievirus type B1 were recovered from vesicular lesions in four adults. Patient 1 had cutaneous localized vesicles, patient 2 had a recurrent cutaneous vesicle, and patients 3 and 4 had mucosal vesicles. Three of the patients were suspected of having herpesvirus lesions. One of the patients was a human immunodeficiency virus type 1-seropositive man, and the enterovirus infection was the first clinical manifestation. Our results underline the importance of virological diagnosis before treatment with acyclovir, especially for immunocompromised patients.

Cutaneous angiomas in POEMS syndrome. An ultrastructural and immunohistochemical study.

Cutaneous manifestations of POEMS (polyneuropathy, organomegaly, endocrinopathies, M-protein, and skin changes) syndrome in a 51-year-old white man included two types of hemangiomas, ie, cherry-type and subcutaneous hemangiomas. By light microscopy and immunohistochemistry, both types of tumors showed features of capillary hemangiomas. By electron microscopy, a peculiar finding of subcutaneous hemangiomas was the presence of slender abluminal processes of endothelial cells that were frequently anastomosing to form electron-lucent blebs. Although the precise etiopathogenesis of the vascular proliferations remains speculative, these lesions merit study since they constitute an easily recognizable marker of POEMS syndrome.

[Onychoheterotopia with polyonychia associated with Pierre Robin syndrome: apropos of a new case. (Trial of classifying nail ectopia)].

The authors report the second case in the English language literature of a rare abnormality of the fingers and nails associated with Pierre Robin syndrome. The patient was born at term to non-consanguineous parents. The only potentially toxic drug taken by the mother during the first months of pregnancy was Sympathyl, a combined preparation of phenobarbitone, methenamine and plant extracts (boldo, crataegus) used as sedative. The child presented at birth with typical Pierre Robin syndrome responsible for respiratory distress, associated with hypotonia, dysplasia of the right hip and cardiac murmur. His development was marked by pronounced psychomotor retardation. At the age of four the patient was referred for congenital malformations of the hands, and notably bilateral and symmetrical abnormalities of the 5th finger consisting of inflammatory-like tapering, soft tissue hypotrophy, sucked appearance of the ungual phalanx, absence of palmar flexion folds and skin ridges and above all, presence of an ectopic supernumerary nail on the volar aspect of the ungual phalanx. This extra nail was not fused with the dorsal nail, although both met at their lateral edges. Other abnormalities were camptodactylia of the 4th fingers, clinodactylia of the 4th and 5th fingers and irreducible extension of the 5th fingers contrasting with hyperlaxity of their metacarpophalangeal joint. The karyotype (46 XY) was normal. Avulsion of the ectopic nails confirmed that they were distinct from the dorsal nails, having their own matrix.(ABSTRACT TRUNCATED AT 250 WORDS)
