DecisionNET: database/network support for clinical decision analysts.

DecisionNET is a proposed remote database/wide-area computer network that should cultivate quantitative approaches to medicine by providing rapid access to medical facts, literature citations, decision models, and commentary of specific use to those involved with quantitative clinical decision making. Increasing accessibility to data and providing a forum for exchange of ideas should prove to be a significant asset to clinicians, either as modelers or as clients. Further work will address specific database implementation, development of a query engine, and coordination of the communications infrastructure. DecisionNET provides a unique model for the growth and maintenance of other medical knowledge bases.

Effect of lung volume history on rate of edema formation in isolated canine lobe.

The effect of lung volume history and prior accumulation of interstitial fluid on rate of edema formation in isolated canine lobes was investigated. Mean pulmonary artery pressure and mean pulmonary venous pressure were kept constant at 40 and 30 cmH2O, respectively. Transpulmonary pressure (Ptp) was varied among 5, 15, and 25 cmH2O by progressive stepwise inflation and deflation. Rate of fluid accumulation was estimated by changes in slow weight gain after a change in Ptp. Although there is continuous interstitial fluid accumulation over the course of the experiment the results indicate that interstitial hydrostatic pressure around leaky vessels at Ptp of 15 cmH2O is reduced by prior lung inflation to Ptp of 25 cmH2O and increased by prior deflation to Ptp of 5 cmH2O. These results suggest that the distribution of interstitial fluid may vary as a function of lung volume history.

Allergic contact dermatitis to 1,4-bis(isopropylamino)anthraquinone. Caused by a felt-tip marker pen.

Adult allergic contact dermatitis to the black ink of a felt-tip marker pen occurred in two adult subjects. In both subjects the cutaneous reaction did not require photoactivation. Patch testing with the ingredients in the black ink demonstrated cutaneous sensitivity to the black dye. Further patch testing with the components of the black dye revealed cutaneous sensitivity in both subjects to the blue component, 1,4-bis(isopropylamino)anthraquinone, commonly known as Colour Index Solvent Blue 36.

Effect of transpulmonary and vascular pressures on rate of pulmonary edema formation.

The effect of transpulmonary pressure (Ptp) on rate of extravascular fluid accumulation in isolated canine left lower lobes perfused at constant vascular pressures was investigated. Changes in rate of extravascular fluid accumulation were estimated by changes in rate of slow weight gain. Mean inflow pressure (Ppa) was constant at 34 cmH2O. Mean outflow pressure (Ppv) was constant at either 27.2 cmH2O or 13.6 cmH2O. When increasing Ptp is associated with a derecruitment of vascular beds as the lung changes from predominantly zone III toward zone II, or zone I, there is a decrease in rate of weight gain. When increasing Ptp is not associated with a derecruitment of vascular beds, it is impossible on the basis of these experiments to predict the change in rate of weight gain.

Effect of intrathoracic pressure on pressure-volume characteristics of the lung in man.

Quasi-static pressure-volume (P-V) curves in normal seated human subjects were determined with pressure at the airway opening (Pa0) set below (negative pressure), above (positive pressure), or equal to ambient pressure. Dynamic compliance (Cdyn) during controlled continuous negative pressure breathing (CNPB) was also studied. Quasi-static P-V curves at negative pressure were decreased in slope, reflected a decrease in total lung capacity, and intersected the P-V curve obtained at ambient Pa0. At positive pressure the P-V curves showed an increase in slope and an increase in total lung capacity. During CNPB a fall in Cdyn was found. The fall in Cdyn was rapid and persisted for the duration of CNPB. Cdyn promptly returned to control levels when Pa0 was adjusted to ambient pressure.
