Supportive care of cancer patients in Dubai.

Supportive care of cancer patients starts from the first day of diagnosis. It is a comprehensive approach involving psychosocial, physical and spiritual supports. Maximum efforts are put in to keep the patients as comfortable as possible during diagnosis, during treatment and during the terminal stages of their disease.

Three-dimensional fetal cephalometry.

Craniofacial growth has been the subject of numerous studies in which different techniques have been elaborated aiming to model this dynamic phenomenon in a rational manner. One of the methods employed is cephalometric analysis applied to the fetus. Generally, however, these studies are confined to the exploration of a single spatial plane (sagittal plane), whose orientation is never defined in a rigorous and perfectly reproducible manner. Thus, none of these analyses offers a formal growth model. This has led us to propose a new method of fetal cephalometric study taking into account criteria for proper reproducible analysis: spatial exploration of the head performed through three-dimensional tomodensitometric images and precise location of landmarks and reproducibility of the orientation of each image, which is assured by reference to the vestibular orientation (based on the external semicircular canals), as has been described by Girard and Perez and further developed by Fenart. When the labyrinth is developed, this orientation does not change during the growth stages of the head, even with craniofacial deformities. This permits application of this orientation on fetuses and the superposition of images of different subjects. The methodology is presented using two normal human fetuses, and the advantages of this computerized tool are discussed.

Cephalometry

[Mountain sports: their role in 2200 facial injuries occurring over 4 years at the University Hospital Center in Grenoble].

Injuries caused by mountain sports account for many of the injuries admitted to the University Hospital of Grenoble. Out of 4,490 traumas, 470 were injuries sustained during the practice of mountain sports. While the frequency of these accidents does not evolve much, the etiological distribution depends on fashion. Thus an increasing number of lesions caused by cross-country biking has been noted during the past two years. We find it urgent to propose protective measures adapted to this new sport.

Treatment of advanced squamous cell carcinoma of the skin with cisplatin, 5-fluorouracil, and bleomycin.

The authors treated 14 patients with advanced squamous cell carcinoma (SCC) of the skin or lip with one to four cycles of combination chemotherapy consisting of cisplatin by bolus injection, and 5-fluorouracil (5-FU) and bleomycin by continuous 5-day infusion. Objective responses were seen in 11 of the 13 evaluable patients (84%). Four patients had a complete remission (30%) and seven patients, a partial remission (54%). Local control after definitive complementary radiation and/or surgical treatment was achieved in seven patients. Toxic side effects was acceptable; they consisted of nausea and vomiting in all patients, transient skin changes, hematologic (Grade 3/4) abnormalities in four patients, and pulmonary fibrosis in one elderly patient. These results show that this chemotherapy combination could play a role in reducing the tumor mass and in facilitating definitive treatment to obtain better functional and cosmetic results in advanced SCC of the skin.

[Role of micro-anastomosed muscular transplantation in corrective surgery of permanent facial paralysis].

Vascularized and reinnervated transfer of a muscle from the body to the face is never a simple procedure. It is important to evaluate the pre-operative state of facial muscles. It is difficult to define the correct indications for the various surgical techniques proposed: muscle selection, selection of recipient vessels, selection of a reinnervation method, the positioning of the transferred muscle. The authors present a series of 12 cases of long standing facial paralysis for more than 3 years. Results are presented. The authors think that the ideal muscular transfer is still unknown. In principle, nothing can replace facial muscles except facial muscles. Currently, the authors prefer the latissimus dorsi flap with partial transfer. For reinnervation, the facial nerve must take the priority, while all other reinnervation technics should take second place. In all cases, great attention must be paid to the positioning of the muscle, the usefulness of conventional palliative procedures and the continuous moral support to all of these patients.

Facial Paralysis

Small sucrose density gradients: reproducible and automatic fractionation using a commercial fraction collector (Pharmacia).

A simple, inexpensive modification of a programmable commercial fraction collector (Frac 100; Pharmacia, Sweden) was described. This modification allowed reproducible automatic fractionation of small sucrose gradients by rinsing the hanging drop at the delivery head of the fraction collector into the sampling tube, using signals from the fraction collector to control this specialized function. The described principle may also be useful whenever fraction size is near to drop volume and high resolution and reproducibility are desired.

Animals

The spectrophotometric determination of streptomycins.

The spectra produced by the acid treatment and alkaline treatment of streptomycin and dihydrostreptomycin have been investigated to obtain suitable methods for their determination. Mixtures of the two antibiotics have been successfully determined in the UV region. Thus, using the orthogonal function method, dihydrostreptomycin has been determined after acid treatment whereas streptomycin has been determined after alkaline treatment followed by immediate neutralization.
