[Two cases of perioperative coronary vasospasm following ASD closure and OMC].

Two cases of perioperative coronary spasm following ASD closure and OMC are reported. A 36-year-old female had been diagnosed of vasospastic angina and another 47-year-old female patient was diagnosed of effort angina for 99% stenosis of the right ventricular branch preoperatively. Both patients suffered from coronary vasospastic attack and ventricular fibrillation several times within 17 hours postoperatively. Although the coronary dissolution was obtained finally following aggressive cardiac massage, administration of spasmolytic agents, such as NTG, lidocaine, DBcAMP and the start of IABP, the resolution was stormy due to the hemodynamic derangement. To prevent the coronary spastic episode, it is suggested, for patients of possible coronary spasm, to avoid arousal stimulation and to administer spasmolytic agents prophylactically within the first postoperative day.

[A successful treatment of spontaneous carotid cavernous fistula causing hemorrhagic infarction treated by detachable balloon].

A rare case of hemorrhagic infarction associated with carotid-cavernous fistula is reported. The patient was a 74-year-old female. CT scan showed hemorrhagic infarction of the left superior temporal gyrus, irregular vascular enhancement of the bilateral front-temporal lobe, and dilatation of the cavernous sinus. Left carotid angiogram revealed a high-flow left-sided CCF, cross filling to the right cavernous sinus, and intracavernous aneurysm. Bilateral front-temporal cortical veins were visualized early in the arterial phase. In this case, there was a direct shunt between the internal carotid artery and the cavernous sinus, and venous hypertension. Therefore urgent treatment using detachable balloon catheters was performed. Postoperative digital subtraction angiography revealed the disappearance of CCF. In conclusion, in CCF with cortical venous drainage there is a high possibility of developing hemorrhagic infarction. Only immediate detachable balloon occlusion can improve the outcome in elderly patients.

Enzymic conversion of 11,12-leukotriene A4 to 11,12-dihydroxy-5,14-cis-7,9-trans-eicosatetraenoic acid. Purification of an epoxide hydrolase from the guinea pig liver cytosol.

(11S,12S)-Epoxy-5,14-cis-7,9-trans-eicosatetraenoic acid (11,12-leukotriene A4) was nonenzymically converted to seven compounds: two diastereomers of (12S)-hydroxyeicosatetraeno-delta-lactones (major products), two diastereomers of (5,12S)-dihydroxyeicosatetraenoic acid and three stereoisomers of (11,12S)-dihydroxyeicosatetraenoic acid. Among these compounds, (11R,12S)-dihydroxy-5,14-cis-7,9-trans-eicosatetraenoic acid proved to be the only enzymic product. This hydrolysis activity was present in the cytosol fractions of various tissues of guinea pig such as liver, adrenal gland, small intestine, and brain. We purified the epoxide hydrolase to an apparent homogeneity from the guinea pig liver. The enzyme had a molecular weight of 60,000 as determined by sodium dodecyl sulfate-polyacrylamide gel electrophoresis, and an isoelectric point of 7.3. The partial amino acid sequence was different from that of the microsomal enzyme. Km and Vmax values for 11,12-leukotriene A4 were 18 microM and 2.4 mumol/min/mg protein, respectively. These results indicate that 11,12-dihydroxyeicosatetraenoic acid is enzymically synthesized from 11,12-leukotriene A4 by the action of the cytosolic epoxide hydrolase in vitro.

Capacities of a newly established thymic stromal cell clone to express Ia antigens and to produce interleukin-6, colony-stimulating factor, and thymic stroma-derived T-cell growth factor.

Thymic stromal cell lines, termed MRL104 and MRL28, have been isolated from long-term liquid cultures of thymic stromal cells from MRL/l mice. The capacities of these parental lines and derived clones to express Ia antigens and to produce cytokines involved in T-cell proliferation and/or differentiation were investigated. Parental lines and their clones did not exhibit a typical fibroblastic, macrophage-like, or epithelial appearance in electron as well as phase-contrast micrographs. These thymic stromal cells seemed to differ from established fibroblast lines in that these thymic stromal cells expressed Ia antigens after exposure to interferon-gamma (IFN-gamma), whereas fibroblast lines did not. They also appeared to differ from macrophage cell lines in that they lacked the expression of Mac-1 antigens on their cell surface and produced no detectable level of interleukin-1 (IL1) before or even after exposure to lipopolysaccharide. When these parental lines and its clones were tested for their ability to produce various types of cytokines, it was revealed that they were capable of producing colony-stimulating factor (CSF), IL6, and thymic stroma-derived T cell growth factor (TSTGF), which was recently described, but were unable to generate other lymphokines and IFNs. Thus these cell lines and clones represent unique features in that they have potentials to express Ia antigens and to produce CSF, IL6, and TSTGF. The biological significance for the expression of these features is discussed in the context of intrathymic T-cell maturation and T-cell repertoire selection.

Treatment of malignant ascites and pleurisy by a streptococcal preparation OK-432 with fresh frozen plasma--a mechanism of polymorphonuclear leukocyte (PMN) accumulation.

A single injection of a streptococcal preparation, OK-432, with fresh frozen plasma (FFP) (or fresh human serum) into the peritoneal or pleural cavity for the treatment of malignant ascites or pleurisy resulted in a complete reduction of ascitic fluid or pleural effusion in 5 out of 11 patients. FFP was used a further source of complement for the effective accumulation of antitumor polymorphonuclear leukocytes (PMNs) by complement-derived chemotactic factors in the cavity. C5a increased in the fluids 3-9 h after the injection and preceded a massive increase in PMNs. C1 inhibitor (C1INH) and C3b inactivator (C3bINA) decreased in several cases 6 h after the treatment. Chemotactic arachidonic acid metabolites, thromboxane B2(TXB2) as a characteristics of TXA2, and leukotriene B4(LTB4) also increased at the same time even in cases where C5a changed only minimally, and may play a role in accumulating antitumor PMNs in the cavity.

DNA methylation in xeroderma pigmentosum.

DNA methylation was examined in xeroderma pigmentosum (XP) cells. The amount of 5-methylcytosine (mC) in DNA from XP cells was about 70% of that in DNA from normal controls. Southern hybridization analysis showed that the HLA-DR alpha gene in XP lymphocyte B cells was differently methylated from normals, but its expression was apparently unaffected. The methylation of dihydrofolate reductase, a housekeeping gene, was the same as in controls. The revertants to UV resistance from XP fibroblasts recovered a methylation level close to that of normal cells. Results suggested that XP DNA was undermethylated non-randomly, and that DNA methylation might be associated with DNA repair function.

Effect of chloride on blood pressure and sympathetic nervous system activity in deoxycorticosterone acetate-treated rats.

The effect of selective chloride loading on blood pressure and sympathetic nervous system activity was studied in deoxycorticosterone acetate (DOCA)-treated rats. The rats fed a normal sodium-high chloride and fed a high sodium chloride diet had higher blood pressure and lower cardiac norepinephrine concentrations than those fed a normal sodium chloride diet. Furthermore, cardiac norepinephrine concentrations were significantly correlated with systolic blood pressure. These data suggest that selective chloride loading can raise blood pressure in DOCA-treated rats due to augmented sympathetic nervous system activity.

Acute effects of exposure to cold on blood pressure, platelet function and sympathetic nervous activity in humans.

To clarify the mechanism for cold-related thrombosis, we evaluated responses of blood pressure, platelet function, and sympathetic nervous activity after cold exposure in ten healthy male volunteers (33 +/- 2 years old). Mean blood pressure, beta-thromboglobulin, platelet factor 4, and plasma noradrenaline were increased after cold exposure associated with significant falls in skin, oral, and urine temperature. The increase in plasma noradrenaline significantly correlated with the change in platelet aggregation (3 microM ADP: r = 0.73, P less than .02, 3.0 micrograms/mL epinephrine: r = 0.65, P less than .05), and with mean blood pressure in the warn environment (r = 0.76, P less than .02). These results suggest that the cold-related increase in sympathetic nervous activity may contribute to enhancement of platelet function. This provides a possible explanation for the risk of thrombosis in cold weather in essential hypertension.

The X gene of hepatitis B virus induced growth stimulation and tumorigenic transformation of mouse NIH3T3 cells.

To examine the transforming potential of the X gene product of hepatitis B virus (HBV), the X-gene-containing region (referred to as the HBx region) was introduced into mouse NIH3T3 cells. Each transformed cell line expressed X-coding mRNA at a different level. A positive correlation was found between the level of X-coding mRNA and the saturation density of the cells. The HBx-transformed cell lines exhibited X protein production and tumor formation in nude mice. The function of HBV in oncogenesis may involve the continuous expression of the X-gene-coded product in the HBV DNA-integrated cells.

Binding properties of human anti-DNA antibodies to cloned human DNA fragments.

The DNA-anti-DNA antibody immune complexes were isolated from plasma of systemic lupus erythematosus (SLE) patients and DNA fragments separated from immune complexes were subjected to molecular cloning. The resulting recombinant DNA clones showed a molecular size of 37-79 base pairs, a high guanine and cytosine content, high frequencies of CpG dinucleotides, and palindromic sequences, and also clusters of G + C- and A + T-rich segments. These clones hybridized randomly to total human DNA. The reactivity of dsDNA antibodies, both monoclonal and polyclonal, from SLE was examined with a cloned SLE antigen DNA. A competitive inhibition assay showed that human monoclonal antibodies had at least one magnitude higher affinity to the cloned DNA than to the native DNA fragments. In order to characterize the factors that were recognized by antibodies, human G + C-rich and also A + T-rich 100 bp DNA fragments were cloned, and their base sequences determined. The antibody showed a higher affinity to the G + C-rich DNA fragment (71% G + C) than to the A + T-rich DNA fragment (46% G + C). When cytosines in CpG doublets in G + C-rich fragments were methylated (mCpG), the reactivity increased up to 100-fold. The native anti-DNA antibodies from SLE patients also showed preferential binding to G + C-rich fragments. These observations suggested that human anti-dsDNA antibodies may recognize some unique structures around the G + C regions or G + C clusters of DNA.

Effect of oral calcium on blood pressure response in salt-loaded borderline hypertensive patients.

To clarify the mechanism of the antihypertensive effect of oral calcium loading, we studied the effect of low versus high calcium intake on salt-induced blood pressure elevations in patients with borderline hypertension. After a 7-day period of dietary salt restriction (50 meq/day), 27 patients were placed on a high salt (300 meq/day), low calcium (250 mg/day) diet for 7 days; 14 of these patients were given 2,160 mg/day of supplementary calcium (Ca group), and 13 patients were given placebo (non-Ca group). With a high salt intake, the percent increase in mean blood pressure was smaller in the Ca group than in the non-Ca group (+2.85 +/- 1.22% vs. +8.63 +/- 1.66%, respectively, p less than 0.01). The Ca group showed a smaller weight gain (p less than 0.05) and a greater urinary excretion of sodium (p less than 0.005) than the non-Ca group. In the Ca group, but not in the non-Ca group, high salt intake resulted in an increase in intraerythrocyte magnesium content (p less than 0.01), which was correlated inversely with the salt-induced changes in mean blood pressure (r = -0.54, p less than 0.05). While the increase in cellular magnesium was greater in the Ca group, the changes in red blood cell sodium and sodium/potassium ratio were not different between the two groups. The results suggest that oral calcium supplementation may prevent a rise in blood pressure in patients on a high salt, low calcium diet by attenuating the sodium retention.(ABSTRACT TRUNCATED AT 250 WORDS)

Oral magnesium supplementation in patients with essential hypertension.

To elucidate the effects of magnesium on high blood pressure, a 4-week study of oral magnesium supplementation (MgO 1 g/day) was conducted in 21 outpatients with uncomplicated essential hypertension. During the study, blood pressure and intraerythrocyte sodium concentration decreased significantly, and the erythrocyte ouabain-sensitive 22Na efflux rate constant (Kos) and intraerythrocyte magnesium concentration both increased. Serum triglyceride and free fatty acid concentrations were reduced. Furthermore, the elevation in Kos significantly and positively correlated with both the increase in intraerythrocyte magnesium concentration and the decrease in mean blood pressure. There was a significant inverse correlation between the prestudy Kos and the decrease in mean blood pressure. In addition, when patients were divided according to their overall decrease in mean blood pressure, the prestudy intraerythrocyte sodium concentration was significantly higher in patients with a mean blood pressure decrease of more than 7 mm Hg than that of patients whose mean blood pressure decrease was less than 7 mm Hg. These results suggest that oral magnesium supplementation may lower blood pressure through the activation of a cell membrane sodium pump and may reduce serum lipid concentration. It also suggests that the lower the prestudy Kos or the higher the prestudy intraerythrocyte sodium concentration, the more effective the oral magnesium treatment is in lowering blood pressure. Therefore, we concluded that appropriate oral magnesium intake might be effective as a nonpharmacological treatment for essential hypertension.

Transin/stromelysin expression in the synovium of rats with experimental erosive arthritis. In situ localization and kinetics of expression of the transformation-associated metalloproteinase in euthymic and athymic Lewis rats.

Transin is a neutral metalloproteinase initially isolated from malignantly transformed rat fibroblasts and subsequently shown to be homologous to human stromelysin. We performed Northern blot analysis on synovial tissue specimens from Lewis rats with proliferative and invasive streptococcal cell wall (SCW) arthritis. Transin mRNA was present in abundance, as was the mRNA of the c-myc oncogene, which is associated with cellular proliferation. Immunohistochemical staining of synovia from rats with chronic SCW arthritis showed high-level transin expression in the cells of the lining layer and underlying stroma, as well as in chondrocytes and osteoclasts in subchondral bone. Intense nuclear staining for the Myc oncoprotein was also detected with a cross-reactive antibody to v-Myc. Transin stained similarly in the early, rapid-onset, thymus-independent, acute phase of SCW arthritis. In the T cell-dependent adjuvant arthritis, transin expression was noted by day 4, 6 d before the influx of mononuclear cells and the onset of clinical disease. Athymic rats did not express transin. We concluded that transin is a marker of proliferative, invasive arthritis in rats and appears early in the course of disease development, but requires a competent immune system to sustain its expression in these model arthropathies.

Mechanism of development of asymmetric septal hypertrophy in patients with essential systemic hypertension.

In order to understand the mechanism of development of asymmetric septal hypertrophy (ASH) in hypertension, 290 patients with essential hypertension (HT) were examined echocardiographically. Out of them 84 cases of advanced left ventricular hypertrophy (LVH) [37 cases of symmetric hypertrophy (HT-SH group) and 47 cases of ASH (HT-ASH group)] were compared in their clinical and echocardiographic findings with hypertrophic cardiomyopathy (HCM). In the 290 HT cases, the highest systolic pressure in each patient's history was found to correlate with left ventricular (LV) posterior wall thickness (PWT), but not with the septal wall thickness (IVST). There were no differences in LV thickness (IVST + PWT) among patients in the HT-ASH, HT-SH and HCM groups. While the HCM group patients showed no significant differences in IVST and PWT from those in the HT-ASH group, they did have greater IVST and smaller PWT than HT-SH group patients. The rapid filling rate (RFR) was also not much different in the HCM and HT-ASH groups, but was significantly lower in the HCM group than in the HT-SH group. Furthermore, HT-ASH group patients has a milder degree of hypertension and a higher incidence of familial occurrence of HCM than did those in the HT-SH group. After treatment for HT, the HT-SH group showed a significant decrease in wall thickness during long-term observation, while the HT-ASH and HCM groups, failed to exhibit such changes. Moreover, the degree of myocardial disarrangement in the HT-ASH group did not differ significantly from that in the HCM group. These results suggested that LVH in HT is related not only to pressure load but also to genetic factors similar to that in HCM.

Decreased cytosolic free calcium concentration in lymphocytes of magnesium-supplemented DOCA-salt hypertensive rats.

The effect of magnesium supplementation on blood pressure and lymphocyte cytosolic free calcium concentration (L-[Ca2+]i; nM) was examined in DOCA salt hypertensive rats (DSR). Magnesium was administered for 4 weeks with a standard diet in the form of 1% MgO (850 mg magnesium/100 g). In magnesium-supplemented DSR (DS-Mg), the elevation of systolic blood pressure (SBP; mmHg) was significantly attenuated (136.7 +/- 23.0 vs 180.6 +/- 23.4, p less than 0.005) and the L-[Ca2+]i was significantly lower (70.5 +/- 7.0 vs 84.5 +/- 7.1, p less than 0.005) than in DSR which were fed the standard rat diet. Lymphocyte magnesium content (nEq/mg protein) in DSR was lower than in control rats (160.6 +/- 18.3 vs 182.3 +/- 13.9, p less than 0.05) and approximately the same as in DS-Mg (164.9 +/- 6.9, p less than 0.05 vs control). Plasma magnesium concentration was slightly higher in DS-Mg than DSR (2.53 +/- 0.35 vs 2.21 +/- 0.26). We observed a significant positive correlation between SBP and L-[Ca2+]i in DS-Mg and in DSR as a whole. These findings suggest that oral magnesium supplementation can attenuate the elevation of blood pressure and simultaneously decrease the cytosolic free calcium concentration in DOCA salt hypertensive rats.
