The role of chloride on deoxycorticosterone acetate-salt hypertension.

Selective sodium loading attenuated the development of hypertension in the deoxycorticosterone acetate (DOCA) treated rat. The DOCA treated rat fed a diet equimolar in sodium to a 7% sodium chloride diet and in chloride to a standard diet, differed in various parameters from the DOCA treated rat fed a 7% sodium chloride diet: it had higher sodium concentration in both erythrocytes and muscles, a higher erythrocyte ouabain sensitive 22Na efflux rate constant (Kos), and a lower norepinephrine turnover rate in the heart and the spleen. These results suggest that the suppressed sympathetic nervous system activity and the activated cell membrane sodium pump contribute in part to the mechanism for the suppression of the development of hypertension in the DOCA-selective sodium loaded rat.

Tumor-specific immunotherapy by active immunization with haptenic muramyl dipeptide derivative-coupled tumor cells.

The theoretical basis and practical manipulation of cellular collaboration mechanism responsible for enhanced induction of tumor-specific immunotherapeutic potential were reviewed. Enhanced generation of tumor-specific in vivo protective immunity could be obtained by immunizing tumor cells coupling highly immunogenic additional antigens to hosts in which helper T cell activity against these antigens is already induced. MDP haptens newly synthesized exhibited cross-reactivity with BCG and functioned as appropriate tumor cell-modifying reagents in BCG-sensitized individuals. With the use of these new MDP haptens, the above approach for enhanced tumor immunity was successfully applied to tumor-specific immunotherapy models. When tumor-bearing hosts had been sensitized to BCG, the growth of tumor cells in a solid tumor mass or in metastasized or disseminated form were effectively inhibited by active immunization with MDP hapten-modified tumor cells. Thus, these results reinforce the theoretical framework of the approach for enhanced induction of tumor immunity. The protocols which were developed in these studies also provide an effective maneuver for tumor-specific immunotherapy.

[Reduction surgery combined with chemotherapy of liver cancer].

Effects of reduction surgery combined with chemotherapy on DAB induced hepatoma were investigated. DAB hepatoma was transplanted in the liver and subcutaneous tissue of rats. The results were as follows; 1. Reduction surgery prolonged the survival time of rats with DAB induced hepatoma, either in liver (5, 7 days, p less than 0.001) and subcutaneous tissue(15.9 days, p less than 0.01). 2. Synchronization chemotherapy with MMC and ADM showed inhibitory effect on cell growth in the cultured human hepatoma cells. (p less than 0.01) 3. Reduction surgery with combined chemotherapy prolonged the survival time 3.9 days compared with reduction surgery only. (p less than 0.05) In conclusion, it was suggested that reduction surgery with synchronization chemotherapy is beneficial for improvement of the prognosis in patients with unresectable hepatic tumors.

Idiopathic portal hypertension associated with polyclonal hyperimmunoglobulinemia. A case report and review of the literature.

We report a case of idiopathic portal hypertension associated with prominent polyclonal hyperimmunoglobulinemia and plasmacytosis in the bone marrow and the spleen. Microscopic examination of the liver showed chronic inflammation in the portal area and abnormal vasculature adjacent to the portal tracts. In the spleen, polyclonal plasma cell proliferation was demonstrated histochemically in addition to the presence of remarkable sinus hyperplasia and periarterial fibrosis. In the present case, a chronic inflammatory state such as connective tissue disease was strongly suspected to exist in the liver, since pancytopenia and hyperimmunoglobulinemia persisted even after splenectomy. It is suggested that the pathogenesis in the present case may have been due to this chronic inflammatory state.

[A personal point of view to EC-IC bypass surgery in completed stroke after the co-operative study].

The role of EC-IC bypass surgery has become a very controversial issue recently. EC-IC anastomosis was performed in 132 patients of stroke in our institute. Thirty six of them which could be followed up completely had a completed stroke and ten of these showed a significant clinical improvement by the next day of surgery. We studied the angiograms, CT scans and dynamic CT studies of all 36 patients to determine the common factors responsible for a significant clinical improvement in these ten patients. It was found that patients with occlusion of middle cerebral arteries and with good collateral circulation fared better than those with other lesions and good cross circulation. Site of low density on CT revealed patient with watershed low density did well. Characteristic of low density area was found to be a significant factors. If the low density area was homogeneous and well defined in an arterial territory, it was invariably associated with no improvement after surgery. Patients showing an improvement had scattered or irregular low density areas. Dynamic CT studies were not found to be significant.

A human T cell line established from a patient with Sézary syndrome. Application for assay of human interleukin 2 (IL-2).

A human T cell line, designated Sez 627, was established from a patient with Sézary syndrome. These clonal T cells have been cultured for more than 2 years in the presence of IL-2 without any antigen stimulation. The surface phenotype of Sez 627 was OKT3+, OKT4-, OKT8+, and Tac+, and infection with human T lymphotrophic virus type I (HTLV-I) was demonstrated by Southern blot hybridization analysis. In human IL-2 assay, Sez 627 cells were found to be superior to murine CTLL-2 cells with respect to their unresponsiveness to phorbol 12-myristate 13-acetate (PMA), and species specificity for human IL-2, as well as better cryopreservation, although they were less sensitive to IL-2 than CTLL-2 cells. Using Sez 627 cells, IL-2 production by lymphocytes from patients with systemic lupus erythematosus (SLE) was examined. Decreased IL-2 production was observed in the patients with active SLE but not in most patients with inactive SLE. These findings suggest that Sez 627 is a useful human T cell line for human IL-2 assay.

Clinical evaluation of palliative therapy for unresectable primary liver cancer.

Ninety-nine patients with unresectable primary liver cancer were treated with ligation of the hepatic artery (LHA), intra-arterial infusion chemotherapy (IAIC) or transcatheter arterial embolization (TAE) between 1960 and 1983. A statistical study was made of therapeutic effects and prognosis. The results are as follows: The mean survival time of 14 patients treated with LHA was about 5 months. The survival time of the LHA + IAIC with mitomycin C group was not prolonged as compared with that of the LHA group, but LHA + IAIC with Adriamycin (doxorubicin) group showed a significant prolongation of survival time over the LHA group. In the LHA + IAIC with Adriamycin group, the mean survival time was 12.8 months and the 1-year survival rate was 37.5%. Factors including age, presence of liver cirrhosis, number of involved segments, and degree of anaplasia graded on Edmondson's criteria did not influence survival time.

Cross-reactivity between haptenic muramyl di- or tripeptide derivatives and Mycobacterium bovis BCG: potential application for enhancing tumor immunity.

Muramyl di- or tripeptide (MDP or MTP) hapten derivatives bearing various structures were synthesized, and the correlation of these structures with cross-reactivity with Mycobacterium bovis BCG and their applicability to enhance induction of syngeneic tumor immunity were investigated. The cross-reactivity of MDP or MTP haptens to BCG was examined by T-cell proliferation responses of lymph node cells from BCG-primed C3H/He mice in the stimulation with MDP- or MTP-coupled syngeneic cells. A haptenic MDP derivative (designated L4-MDP) stimulated proliferative responses appreciably. Derivatives in which alanine in the peptide portion of L4-MDP was replaced by methylalanine or valine failed to induce stimulation. However, the cross-reactivity with BCG was regained in the MTP derivative that was formed by adding lysine to dipeptide containing methylalanine or valine. Whether this cross-reactive pattern was correlated with enhanced induction of tumor immunity was further investigated. According to the established protocol for the augmented induction of tumor immunity, BCG-primed C3H/He mice were immunized with various haptenic MDP-coupled syngeneic X5563 tumor cells. Immunization with tumor cells conjugating BCG-cross-reactive haptens resulted in enhanced tumor immunity, whereas immunization with tumor cells coupling non-cross-reactive haptens failed to produce anti-X5563 tumor immunity. These results indicate that the peptide portion in these haptenic structures is critical in the generation of BCG cross-reactivity leading to enhanced tumor immunity.

Enhanced red cell sodium permeability in patients with borderline hypertension.

Red cell membrane sodium permeability was studied in 41 untreated patients with essential hypertension (20 borderline hypertensives and 21 established hypertensives) and 21 age matched normotensive subjects by means of the measurement of unidirectional passive influx of 22Na+ into ouabain-treated erythrocytes. The mean value (+/- SD) of 22Na+ influx was greater in the hypertensives than in the normotensives (0.183 +/- 0.047 vs 0.152 +/- 0.047 mmol/l . cells/hr, respectively, p less than 0.02). Among the patients with essential hypertension, the borderline hypertensives demonstrated a higher 22Na+ influx than the established hypertensives (0.207 +/- 0.043 vs 0.160 +/- 0.038 mmol/l . cells/hr, respectively, p less than 0.001), and 22Na+ influx positively correlated with plasma renin activity (r = 0.44, p less than 0.005). In 16 of 20 borderline hypertensives, 5 year blood pressure changes were examined retrospectively, and a positive correlation was observed between mean blood pressure increase and 22Na+ influx value in these subjects (r = 0.64, p less than 0.01). These results suggest that passive sodium influx may be altered in the course of the development of hypertension in relation to the changes in blood pressure level and that enhanced sodium permeability may be a characteristic of the early stage of essential hypertension.
