[Results of cataract extraction and lens implantation].

98 patients were invited for a follow-up examination 2 years after cataract extraction. They then completed a questionnaire dealing with their daily functioning, comparing the situations prior to and 2 years after the operation, and were asked as to their satisfaction with the results of the operation. Statistically significant improvement was found in measurements of both near and distance vision and in performance of most aspects of daily functioning. Most patients were pleased with results of the operation.

Cataract Extraction

[Coincidence of uveitis, dental pulpitis and gingivitis].

The coincidence of uveitis, dental pulpitis and gingivitis is reported. The patients were 2 women, 21 and 50 years old, respectively, and a man aged 36. All were cured by systemic antibiotic treatment. As uveitis is often found in otherwise healthy patients with no apparent focus of infection, it has been suggested that such a focus might be of dental origin. In a tooth abscess the seat of infection in the bone tissue is subject to pressure and irritation from chewing. This can cause bacteria from infected root canals to be pumped into the blood stream and lymphatics. The causes we describe demonstrate the importance of systemic, wide-spectrum antibiotic treatment in such cases.

Adult

An atypical fulminant course of choroidal osteoma in two siblings.

We studied two cases of bilateral choroidal osteoma in an otherwise healthy 5-year-old boy and his only sibling, a 7-year-old sister. Both children were known to have normal fundus appearances at younger ages. The tumor showed slow growth in all four eyes, but severe visual acuity loss developed in three eyes because of neovascular complications that could not be effectively treated by photocoagulation. Secondary retinal cysts developed in three eyes.

Child

Autosomal recessive blepharophimosis, ptosis, V-esotropia, syndactyly and short stature.

A recessively inherited syndrome of blepharophimosis and ptosis with weakness of extraocular and frontal muscles is reported in six members of three related kindreds. Prognathism, synophrys and thick eyebrows added to a typical facial appearance. Additional findings included short stature, borderline head circumference and toe syndactyly. Borderline mental retardation and anosmia were found in one patient. The clinical features and the mode of inheritance distinguish this syndrome from other blepharophimosis and ptosis syndromes.

Adolescent

Autosomal recessive Peters anomaly, typical facial appearance, failure to thrive, hydrocephalus, and other anomalies: further delineation of the Krause-Kivlin syndrome.

Two cousins and an unrelated patient, all offspring of consanguineous parents, presented with Peters anomaly, unusual facial appearance, disproportionate short stature, retarded skeletal maturation, and a variable degree of mental retardation. Variable digital, cardiac, CNS, and urogenital anomalies were present. The inheritance is probably autosomal recessive. The condition is a distinct clinical entity for which we suggest the eponym Krause-Kivlin syndrome. Peters anomaly is thought to result from abnormal migration of neural crest cells. A similar mechanism was implicated in the pathogenesis of other disorders of the anterior chamber. The presence of Peters anomaly, and possibly of other corneal endothelial disorders in a newborn infant, should alert the clinician to the possibility of this syndrome. Communicating hydrocephalus (or brain atrophy) and polyhydramnios were documented in two patients, potentially allowing prenatal diagnosis in secondary familial cases.

Effect of I-Scrub on signs and symptoms of chronic blepharitis.

To assess the effect of I-Scrub (Spectra Pharmaceutical Services), a new detergent eyelid scrub, on the subjective symptoms and objective clinical signs of chronic blepharitis, 20 patients with this disease were treated for three weeks. None of them yielded positive cultures for virulent bacteria from the eyelids and conjunctiva. Most of the patients reported significant reduction in the subjective symptoms and in the severity of the clinical signs as well. No adverse effects were observed. The results of our study suggest that this product is of value as a detergent eyelid scrub in the treatment of chronic blepharitis.

Aged

Unilateral congenital ptosis due to plexiform neurofibroma, causing refraction error and secondary amblyopia.

An 8-year-old boy with congenital ptosis of the right upper eyelid due to plexiform neurofibroma was operated on because of a rapidly worsening of his ptosis. Only partial removal of the tumor was possible. A hypermetropic astigmatism of the right eye was caused by the condition of the upper lid, with secondary amblyopia. This finding suggests that in cases of congenital ptosis the presence of a lid tumor must be suspected. Such a tumor causes ptosis and this can lead to refraction error and amblyopia.

Amblyopia

[Ocular injury by artificial snow spray].

We treated the eyes of 12 children, aged 2.5-16 years, which were injured by artificial snow-spray during Israel's Independence Day festivities in 1987 and 1989. There was chemical damage to the conjunctiva and cornea which took 1-3 weeks to heal.

[Hyperviscosity syndrome and eye involvement].

10 patients with serum hyperviscosity syndrome of different etiologies were examined by fluorescein angiography of the ocular fundus. There was no exact correlation between the pathological findings in the fundus of cases with plasma cell dyscrasia and blood viscosity. Only papilledema and retinal venous engorgement were improved by lowering serum viscosity by plasmapheresis. The abnormal circulating immunoglobulins are an important factor in the genesis of the pathological changes in the retina of patients with plasma cell dyscrasia.

[Ocular injuries from seashore racketball].

A 3-year prospective study revealed 39 hospitalized cases of ocular injuries from seashore racketball sports. Most were children and young adults and most had internal injuries of the globe. Over a third had retinal edema, 2 eyes were blinded, and at least 4 are subject to serious future complications.

Eye injuries in children in Israel. A nationwide collaborative study.

A nationwide prospective collaborative study on ocular trauma was performed in Israel during a period of 3 years (1981 through 1983). Almost half of the traumas (1127 [47%] of 2416 eyes) were sustained by children younger than age 17 years, and mainly between the ages of 6 and 12 years. Most of the injuries happened at home (38.1%) or in the street (26.8%) and during play and sport (65.1%). The male-to-female ratio among the children was 4:1. Blunt injuries accounted for 59.2%; 30.5% were perforating injuries and the rest were chemical and radiation injuries. The visual acuity at the time of hospital discharge was better than 6/30 in the majority of the eyes but 122 patients (11%) had a visual acuity of less than 6/60, and no light perception was the result in 40 eyes (3%). The reasons for the severe outcome were as follows: 23 eyes (2%) owing to phthisis, 22 eyes (1.9%) were enucleated, and 16 eyes (1.4%) suffered from endophthalmitis.

Adolescent

[Metastatic Serratia marcescens endophthalmitis].

A case of metastatic Serratia marcescens (SM) endophthalmitis is described in a 57-year-old diabetic woman, after amputation of her leg above the knee because of peripheral vascular disease. SM cultured from the infected surgical stump was the source of septic emboli to her right eye and lungs, causing endophthalmitis and pneumonia. The ocular infection did not respond to appropriate antibiotic therapy and evisceration was required. SM infection can cause endophthalmitis refractory to antibiotics, and it should be aggressively treated when SM is cultured from any infected site.

[Epiphora as an expression of dry eye].

One of the most common ophthalmic disorders in the elderly is epiphora, a watery eye with overflow of tears. In some patients it appears in a paradoxical form: dry eye indoors and tearing outdoors. We present a 76-year-old man and a 58-year-old woman with this disorder. Despite diagnosis of dry eye, confirmed by several tests, they complained of epiphora. This ceased after treatment with tear substitutes, bromhexine and occlusion of the lacrimal puncti. The possibility that tearing may be an expression of the dry eye state should always be considered.

Aged

Methazolamide-associated temporary leukopenia and thrombocytopenia.

A three-week course of methazolamide therapy for chronic open-angle glaucoma resulted in marked leukopenia and thrombocytopenia. Prompt discontinuation of therapy resulted in complete recovery of white blood cell and platelet counts within five days. The recommendations for patients receiving carbonic anhydrase inhibitors for blood count control at six-month intervals are discussed.
