This study reports subcortical lesions in 7 newborn babies after transient circulatory arrest and/or asphyxia. Basal ganglia, diencephalon, tegmentum of the brain stem and spinal grey matter exhibited extensive necroses in a columnar pattern. The lesions of the telencephalic and cerebellar cortex are less prominent. The spinal cord, available in 3 children, revealed subtotal neuronal loss in all segments. The lesions represent the extreme anoxic damage of the CNS in the perinatal period, comparable with brain death in the adulthood. Moreover, the pattern with predominating subcortical lesions indicates that anoxia affects the grey matter in all levels of the CNS.

This study reports clinical and neuropathological findings in six premature infants dying after prolonged assisted ventilation (IPPB and CPPB) due to pulmonary insufficiency (gestational age: 26.--32. week; birth weight 820--1400 Gm; respirator therapy 41--143 days; survival 104--263 days). During the rspirator therapy the children developed marked extensor rigidity of the trunc and spasticity of the extremities. Postmortem examination revealed cor pulmonale and right ventricular failure. Signs of marked perinatal brain damage were missed. The slightly atrophic brains showed predominant damage to the telencephalic white matter of varying intensity ranging from focal necroses to gliosis and retarded myelination. Constant findings were increased vascularisation and transformation of the premyelinating glia into astrocytes in the deep and subcortical white matter. This form of telencephalic leucoencephalopathy indicates the particular vulnerability of the developing white matter in conditions with chronic hypoxia, hypercapnia, acidosis and vascular congestion. The varying intensity of the lesions suggests that, in principle, minor lesions are either reversible or may be compensated in surviving children.
A sample extraction chamber for the determination of O2- CO2- and N2O-concentrations in 50 microliters blood samples by gas chromatography is described. These blood samples can be transferred anaerobically from the ear lobe of a patient or other donor by a doctor or medical auxillary. It is shown that N2O, O2 and CO2 are completely extracted from blood, red cell suspension and HCO3-solutions respectively. When the concentrations of the same sample was repeatedly measured the coefficient of variation was 1.3% for O2 and N2O and 1.2% for CO2.

Blood gas and acid base values in cord blood are not predictive for neonatal cardiopulmonary adaptation on the extrauterine life. The oxygen-cardiorespirography shows objectively the state of adaptation. By this monitoring beat-to-beat heart rate, respiratory rate, thoracic impedance and the transcutaneous PO2 are measured. 16 of 337 newborns had severe cardiopulmonary problems (11 respiratory distress, 5 congenital heart disease), which was seen in at least one of the parameters. In 10 of 16 infants a pathological pattern was recognized by oxygen-cardiorespirography before clinical symptoms appeared. Additional information can be given by the hyperoxia test to differentiate healthy and sick newborns (respiratory problems and congenital heart disease). With two tcPO2-electrodes fixed on the thorax and abdomen, the shunt through an open ductus arteriosus can be estimated by different values.
