Comparison of salivary miconazole concentrations after administration of a bioadhesive slow-release buccal tablet and an oral gel.

The salivary miconazole concentrations after administration of a bioadhesive slow-release buccal tablet and an oral gel have been compared. The bioadhesive tablet consisted of a mixture of thermally modified starch and 5% polyacrylic acid. Although the amount of drug administered via the bioadhesive tablet was sixfold lower than when the gel was used, the salivary miconazole levels were higher and remained above the MIC value of Candida albicans for more than 10 hours. The mean adhesion time of the tablet was 586 min. The bioadhesive tablet appears to be a promising drug delivery system for the buccal administration of drugs for local therapy.

[Oroantral communication and its treatment].

The most frequent cause of an oroantral perforation is the extraction of an upper molar. In this article, this for a dental practitioner important entity is discussed in detail. After the etiopathology, clinical and roentgenological diagnosis is discussed. As 6.5% of perforations closes spontaneously, the question is raised wether all perforations should be surgically treated. Those different surgical techniques are then described and emphasis is thereby laid on the difference between a mere perforation and a fistula.

[Subcutaneous emphysema of the oral floor during cleaning of a bridge fixed on an IMZ implant. Case report].

A case of subcutaneous emphysema following intraoral elimination of plaque and tooth shading, on a bridge fixed on implants, is presented. An instrument that produces a cleaning spray with a water-air-powder mixture was used. A few minutes after this cleaning procedure there was facial swelling. The patient experienced difficulties in breathing and in swallowing; symptoms of unilateral hearing loss were also noticed. The emphysema lasted for three days. Etiology, diagnosis, differential diagnosis, complications and management are discussed.

Revascularization of gingival autografts placed on different receptor beds. A fluoroangiographic study.

The revascularization of gingival autografts, placed either on fenestrated periosteum or on non-fenestrated periosteum, has been compared by means of 12 fluorescein angiographs, taken 8, 10 and 12 days postoperatively on 12 patients, 18-32 years of age. The grafts placed on periosteum (P-zone) showed a mean increase of fluorescence of 10.6% between the 8th and 10th day and of 19.4% between the 10th and 12th day. At that time the circulatory system in the graft seemed to be established for 97.7%. The grafts placed on fenestrated periosteum (B-zone) showed a mean increase of fluorescence of 13.2% between the 8th and the 10th day and of 13.5% between the 10th and 12th day. The revascularization reached 96.1% at that time. Between both groups no statistically significant difference has been found. It can be concluded that a periosteal receptor side is not a prerequisite for the formation of an even circulatory pattern in gingival autografts.

[Basal cell nevus syndrome with sinus involvement].

The characteristics of the basal cell naevus syndrome are multiple basal cell naevi and basal cell carcinomas, cysts of the jaws, anomalies of ribs and vertebra, intracranial calcification and striking deformities of the skull, usually with a positive family anamnesis. We will discuss symptomatology, histopathology and therapy. A flemish family, where the syndrome occurs with the mother and her four children, three of whom are admitted in hospital nearly simultaneously with sinusitis maxillaris, are discussed in extenso. Also, the occurrence of the syndrome's stigmata in other members of the family is noted.

[Osseous desmoid fibroma of the mandible. An unusual case in a 15-month-old infant].

The authors present a rare case, unique in the literature, of an osseous desmoid fibroma in which the histological diagnosis was made at the age of fifteen months. The signs were of a slight swelling with complete trismus of the mandible. After excision of the tumour, with a resultant defect in the superior ramus of the mandible, the trismus disappeared spontaneously after two weeks.
