[Brain death and organ circulation].

Onset of brain death is accompanied by dramatic circulatory and metabolic reactions. The following period is marked by hypothermia and hypotension. Little is known about the fate of organs under these circumstances. Circulation, ultrastructure and biochemical parameters of the liver were investigated in dogs and pigs by means of a brain death model. During the occurrence of brain death, liver cell hypoxia could be observed, though flow rates in the portal vein and hepatic artery increased remarkably. After brain death has occurred, metabolic and ultrastructural signs of hypoxia diminish while the liver flow is reduced to 53% of the recordings before cerebral damage.

[Biochemical and ultrastructural changes of the liver during the loss of cerebral function].

Biochemical and electron optical changes in the liver cell were recorded at the experimental brain death in dogs. The investigation results refer to a hypoxic lesion at the beginning of the brain death despite of a strong flow in the hepatic artery and the portal vein. An enlarged shunt flow is discussed to be the cause.--A distinct recovery tendency shows the morphological picture at the state of the brain death despite reducing the hepatic blood-flow and without aimed therapeutic measures. This recovery tendency appears better in the electron microscopical finding than in the behaviour of the cell metabolites.

[Effect of cerebral circulation standstill on liver blood flow].

Arterial pressure, heart-rate, cardiac output, and blood-flow in the hepatic artery and the portal vein were recorded during the experimental death of the cerebrum in dogs. An increase of the hepatic blood-flow with a consistent portion in the heart time volume was to be seen in the beginning of the death of the cerebrum. A decrease of the hepatic blood-flow with a diminished portion in the heart time volume was detected in the state of brain death after 3 hours.

[Experience obtained from segmental epidural analgesia in obstetrics (author's transl)].

Lumbar epidural anaesthesia was applied for obstetric analgesia to 103 women, most of them primiparae. Three different concentrations of Bupivacain (Markaine) were used for local anaesthesia, percentages being 0.5, 0.375, and 0.25. Effects and side-effects were evaluated. -- The rate of vacuum extraction was slightly increased under epidural analgesia (24.3 per cent), with ejection periods being enlarged to average values between 82 and 63 minutes. Frequency of caesarean sections as well as both the acid-base status and Apgar scores of the newborns were similar to those recordable from comparison groups. -- Advantages were clearly visible and side-effects of minor importance. Catheterisation for lumbar epidural anaesthesia, consequently, is recommended as an optional approach to obstetric analgesia.

[Experimental brain death].

There is described a method for cerebral circulatory standstill by means of an isolated intra vital sink of cerebral vessels with Kallocryl C and it is compared with other methods. This methods is the most suitable model. Indeed it is connected with only a single response of the circulatory system in contrast to the most frequent clinical progresses. The final condition, however, corresponds to the isolated brain death of the organ donor. Long time examinations are possible.

[Anaesthetic block close to spinal cord of gynaecological high-risk patients (author's transl)].

Conduction anaesthesia was applied close to the spinal cord of 91 gynaecological patients whose risk in the context of surgery had been found to be increased on account of advanced age or for severe secondary internal diseases. The rate of complications of these patients with regional anaesthesia was compared to that of another group of 91 patients on whom operations were performed under general anaesthesia. The group with anaesthetic block close to the spinal cord exhibited less intra-operative variation in arterial pressure and less postoperative disorders of the respiratory organs. The metabolism of diabetic patients was more easily controlled and managed under regional anaesthesia. Conduction anaesthesia close to the spinal cord is recommended for patients in geriatric condition or with concomitant internal disorders.

[Comparative measurements of the brain and internal body temperature in cerebral circulatory stop in dogs].

A report is given about comparison measurements of the intracerebral temperature and the temperature of the interior of body in form of the blood temperature in the arteria pulmonalis in dogs with cerebral circulation stop. The decrease of the brain temperature of 0.03 K per minute takes place long before the decrease of the blood temperature. It coincides clearly with other signs of the deficiency of all brain functions. The decrease of the brain temperature can be considered as one of the early symptoms of the cerebral circulation stop.
