Ultrasonographic and endocrinological studies of ovarian function.

Ovarian morphological alterations in 6 physiological menstrual cycles were studied by sonography and compared to 13 inadequate cycles with a short or missing luteal phase. In addition, basal body temperature (BBT) and 17-beta-estradiol (E2), luteinizing hormone (LH), progesterone (P), testosterone (T) and dehydro-epiandrosterone-sulfate (DHEA-S) levels in serum were measured. Maximal follicle were significantly smaller in insufficient cycles (17.7 +/- 2.9 mm) than in physiological cycles (23 +/- 2.3 mm). Corpus luteum (CL) structure was visualized in 5 of the 6 physiological cycles but was not detected in insufficient cycles. Persistent poly-follicular reaction (greater than 3 follicles/ovary) without a dominant follicle larger than 10 mm was detected in the ovaries of 3 patients with clinical and hormonal signs of polycystic ovarian disease (PCOD).

[Comparative sonographic and endocrine studies of gonadotropin-stimulated menstrual cycles].

In 18 cycles of 16 patients the morphological changes following stimulation by human gonadotrophin were followed by ultra-sonography and radio-immunoassays of 17 beta-estradiol and progesterone (HMG). In 9 of 18 cycles hyperstimulation with more than 3 follicles on the day of chorionic gonadotrophin administration were found. When the human menopausal gonadotrophin treatment (HCG) results in the maturation of more than 10 follicles per ovary the estradiol level reaches more than 2000 pg/ml. In 4 of 8 cycles the administration of HCG triggered the hyperstimulation syndrome. In 5 of 18 cycles pregnancy was induced after a follicle of at least 17 mm diameter was found independant of the number and size of concommitant follicles. Because of the high incidence of HMG hyperstimulation ultra-sonographic prediction of a multiple pregnancy is of no practical value. In 6 of 10 treated cycles without success the deformation or unclear margins of the predominant follicle suggested an insufficient stimulation with HMG. For the monitoring of follicular maturation ultrasonography is superior to estradiol determination since only a large number of follicles showed correlation to elevated estradiol levels.

[Ultrasound and semiquantitative chorionic gonadotropin secretin in the diagnosis of abortion].

191 patients with signs of threatened abortion were assessed by means of ultrasound (Kretz-Technik, Vidoson, Siemens) and semiquantitative assay of urinary HCG-excretion. Sonar examination was able to establish an accurate diagnosis of fetal life (heart action and/or movements) in all pregnancies showing a gestation sac suitable to the 11th week. The remainder cases required a repeated investigation including HCG-levels and growth rate of the gestational sac. Thus the diagnosis of blighted ovum could be made within one or occassionally two weeks.

[Ultrasonic study of fetal dysmaturity: hypertension, diabetes and Rhesus-immunization (author's transl)].

Using ultrasonic A- and B-scan equipment (Kretz-Technik), 80 pregnancies with chronic placental dysfunction were investigated: 34 cases with hypertension, 24 cases with diabetes and 22 cases with rhesus immunization. In addition to biparietal diameter (A-scan), the direct and transverse diameter, the circumference and the area were estimated on B-scan displays of the occipito-frontal plane and the largest cross-sectional plane of the abdomen. 15 parameters of fetal growth and body composition were compared with normal standards. The significance of the different methods in the diagnosis of fetal dysmaturity was demonstrated.

[Estimation of the fetal head-trunk proportion by ultrasound (author's transl)].

Using ultrasonic A- and B-scan equipment, cross-sections of the fetal head and abdomen were recorded in 219 pregnancies of the last trimester. Normal values of the body composition were established by several diameters, the circumferences and the areas. The diagnostic reliability of the different methods is demonstrated in a case report.

[Characterization of active fetal movements in early pregnancy using ultrasound (time-motion technic)].

In 82 pregnancies between 9 and 14 weeks of amenorrhea, 120 registrations of active movements of the embryo were recorded by the ultrasonic time motion technique. Four types of early motility could be observed: Ia = single convulsions of the body, Ib = series of body convulsions, II = rapid movements of the extremities, III = combined complexe movements. The frequency and complexity of the movements increased with progressing gestational age.

[Diagnostic and therapeutic problems in retarded intrauterine development].

The present paper reports on the deliveries of 16 infants exhibiting chronic intrauterine growth retardation, reflected by a birth weight between 900 and 2000 gm. All babies were delivered by caesarean section. The validity of prenatal supervising methods is demonstrated with reference to termination of pregnancy.

[Determination of the gestational age in late pregnancy by sonography (author's transl)].

In 219 normal pregnancies between 28 and 42 weeks the relationships of different ultrasound measurements of the fetal head and the fetal body to the gestational age were determined in order to improve the accurate diagnosis of fetal maturity during the last trimester of pregnancy. With the A and B ultrasound apparatus of Kretz, the biparietal diameter was determined by the A mode, the fronto occipital plane and the largest abdominal diamter were determined by the B scan. The following ultrasound measurements were taken: the two horizontal and transverse diameters, the circumferences and the planes. The abdominal measurements showed a higher rate of growth, compared to the cranial measurements a lesser rate of growth retardation and a higher variation of the normals. The correlation of the abdominal measurements with the gestational age was worse than that of the cranial measurements. Wth the exception of the biparietal diameter in the A mode, the mean diameters of the cranial and abdominal measurements gave better results than the single diameters. Circumference and surface measurements showed no advantages. The best results were obtained by combined fetal head and body measurements using the summation of the biparietal diameter in the A mode and the mean abdominal diameter in the B scan.

[The prognosis of intrauterine fetal growth retardation (author's transl)].

Intrauterine growth retardation is described and 232 cases observed between 1970 and 1973 are reported. The prognosis for the infant is dependant upon the birth weight. Dysmature infants with a birth weight above 2000 grams show relatively few signs of antenatal and postnatal pathology. With a birth weight between 1000 and 2000 grams a perinatal mortality of 50% must be expected unless delivery is performed by primary Caesarean Section. The available methods of observation of high risk pregnancies permit an accurate determination of the best time to terminate the pregnancy. Under optimal conditions the long term prognosis for dysmature infants around 1500 grams birth weight is good.

[The value of ultrasonography in the diagnosis of female genital tract tumors (author's transl)].

In 199 patients with female genital tract tumors the preoperative diagnosis recorded by palpation and ultrasound examination with the A and B apparatus of Kretz were compared with the microscopic findings at laparotomy. The sonographic description of the findings was accurate in 60% of the cases, largely correct in 33% of the cases and mostly erroneous in 7% of the cases. By palpation a correct diagnosis was made in 38% of the cases, largely correct diagnosis was established in 45% of the cases and a mostly erroneous diagnosis was made in 17% of the cases. With tumors up to 4 cm. in diameter, both methods were of approximately equal value. A prognosis as to the likely histology was not possible with the ultrasound technique. A prediction as to malignancy was possible in many cases. Only 4% of the homogenous tumors were malignant whereas 27% of the inhomogenous tumors were malignant tumors.

Diagnostic Errors↗

[Estimation of fetal weight by ultrasound (author's transl)].

In attempt to estimate birth weight in utero several dimensions of fetal head and trunk cross-sections, obtained by ultrasonic A- and B-scan techniques (Kretz-Technik), were related to postnatal weights in 108 normal and abnormal pregnancies. Abdominal parameters showed better correlation with weights than did head parameters. Evaluations derived from mean diameter, circumference or area yielded more accurate estimations than those derived from single diameter. In addition, the combination of both head and abdomen measurements increased accuracy. The best results were obtained by planimetric determination of head and trunk cross-sectional areas. Using this method the standard error of estimates was 233 gm. as compared with 423 gm. employing biparietal diameter alone.
