[Human acid secretion during daily administration of H2-blockers].

The potency and duration of a single dose of ranitidine and famotidine were compared in placebo-controlled studies. In addition, the effect of a single night-time dose of famotidine (40 mg) on gastric acid secretion and basal hormone levels was assessed before, during and after a 28-day treatment. Basal acid secretion was depressed by about 73% and 76% 12 hours after 300 mg ranitidine and 40 mg famotidine respectively. Pentagastrin-stimulated acid output was reduced by about 26% and 29%. 20 hours after both drugs, basal secretion was still inhibited by about 60%, whereas no effect on stimulated acid secretion could be detected. Nocturnal gastric acidity (23.00-07.00) was inhibited from 35.8 +/- 4.6 mmol/l to 1.8 +/- 0.5 mmol/l (94% inhibition) by 40 mg bedtime famotidine , and to 1.7 +/- 0.5 mmol/l (95% inhibition) by 300 mg ranitidine. Both drugs significantly reduced H+ concentrations during the following day. On day 29, i.e. 12 hours after the last famotidine dose, basal and stimulated acid secretion was reduced by some 50% and 26% respectively. On day 35, gastric acid output had returned to pretreatment values. Basal levels of prolactin, testosterone etc. were unchanged by 28-day famotidine treatment. Rantidine and famotidine may therefore be used as a single night-time dose in the acute treatment of peptic ulcer disease.

Radioimmunoassay for serum thyroglobulin designed for early detection of metastases and recurrencies in the follow-up of patients with differentiated thyroid carcinoma.

A radioimmunoassay (RIA) for the measurement of thyroglobulin in human serum was developed and factors that influence sensitivity were investigated. In a comparison of 3 different labeling procedures (chloramine T, iodogen, lactoperoxidase) iodogen-prepared tracer proved to be slightly superior with respect to sensitivity and stability. The shelf life of the tracer was improved by a protein-enriched buffer, which serves as a radical scavenger. The binding kinetics of tracer to antibody at different temperature ranges were examined, and the most rapid and complete binding was found at room temperature. For the preparation of standard curves, several artificial media were compared with thyroglobulin-free serum. Second antibody separation was investigated and optimized. By employing sequential saturation, sensitivity of 0.75 microgram/1 (B0-3 SD) and 50% intercept of less than 5 micrograms/l were achieved. The results of RIA measurements of thyroglobulin in 142 patients with papillary and follicular thyroid carcinoma after thyroidectomy and 131I treatment were compared with 131I whole-body scans. The results confirmed that serum thyroglobulin is an early indicator of recurrency.

Seasonal variations of serum 1,25-dihydroxyvitamin D3 and alkaline phosphatase in relation to the antler formation in the fallow deer (Dama dama L.).

Serum 1,25-dihydroxyvitamin D3 and serum alkaline phosphatase increased several fold during the antler formation period in July. Both maxima were observed in the second half of the antler formation period, where the mineralization of the antler takes place. In contrast serum levels of calcium and 25-hydroxyvitamin D3 showed no alternation or seasonal variation.

Interlaboratory comparison of 25-hydroxyvitamin D determination.

This interlaboratory study on determination of 25-hydroxyvitamin D (25-OH-D) in serum involved 15 laboratories in eight European countries. All could distinguish between normal (50 +/- 31 nmol/L, mean +/- SD) and grossly increased concentrations, but for eight laboratories the results for serum samples with low and normal 25-OH-D content overlapped. In general, values were well reproducible, but interlaboratory variation in 25-OH-D measurement was large, 24,25(OH)2D3 interfering in most of the assays. We present evidence in favor of chromatography before assay, as opposed to nonchromatographic methods. Liquid chromatography with ultraviolet detection for quantifying 25-OH-D2 and 25-OH-D3 appears to be an appropriate reference method, whereas competitive protein binding assay is the method of choice for routine determinations. Control sera with subnormal, normal, and above-normal concentrations of 25-OH-D3 are needed for use in standardization of 25-OH-D assays.

[Left ventricular function and activity of vasoconstrictor systems in patients with chronic heart insufficiency].

An increased activity of vasoconstrictor mechanisms may play an important role in circulatory adjustments to heart failure. Thus, hemodynamic data and the plasma hormones epinephrine (E), norepinephrine (NE) and arginin vasopressin (AVP) as well as the plasma renin activity (PRA) were assessed in 50 patients undergoing coronary angiography and right heart catheterization. Patients were classified into three groups according to severity of left ventricular (LV) dysfunction as assessed by ejection fraction (LVEF): those with normal left ventricular function (group 1 (n = 12): LVEF greater than or equal to 55%, mean 70 +/- 3%) and those with moderate (group 2 (n = 16): LVEF 54-35%, mean 43 +/- 2%) or severe LV dysfunction (group 3 (n = 22): LVEF less than 35%, mean 22 +/- 1%). At rest plasma NE concentrations in patients with heart failure (group 2: 187 +/- 17 pg/ml; group 3: 299 +/- 27 pg/ml) did not differ significantly from control values (199 +/- 26 pg/ml). During exercise, NE concentrations increased in all patients (p less than 0.001). This increase in plasma NE was more pronounced in group 3 (753 +/- 71 pg/ml) than in group 1 (262 +/- 37) and group 2 (388 +/- 64). A significant inverse correlation was found between plasma NE and stroke index at rest (r = -0.592, p less than 0.001) as well as during exercise (r = -0.659, p less than 0.001). PRA was elevated at rest and during exercise in patients of group 3 but not of group 2 as compared with control patients (p less than 0.05). Plasma E and AVP were similar in all groups. Patients of group 3 were subdivided according to exercise capacity into patients who tolerated a maximum work load of 50 watts or more (group 3A) and those who did not tolerate a work load exceeding 25 watts (group 3B). At rest and during exercise, patients of group 3A had a higher stroke index than patients of group 3B. In contrast, there was no significant difference in LVEF between group 3A and 3B (22 +/- 2 vs 20 +/- 1%). During exercise patients with low exercise capacity (group 3B) had higher NE levels than patients with less impaired exercise capacity (group 3A) (948 +/- 86 vs 590 +/- 65 pg/ml, p less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

[4 weeks' administration of omeprazole: effect on acid behavior and basal hormone levels].

We have assessed the effect of omeprazole (30 mg daily) on gastric acid secretion as well as on basal hormone levels (fasting gastrin; TSH, T3, T4, TBG; insulin, glucagon, C-peptide; prolactin, testosterone, 17-beta-oestradiol, dihydroepiandrosterone, cortisol and PTH) in 8 healthy volunteers before and after a 28 day treatment. On day 29, i. e. one day after the last omeprazole dose, mean stimulated acid output was still reduced from 27.4 +/- 3.5 mmol H+/h (+/- SEM) to 7.8 +/- 1.4 mmol H+/h (72% inhibition). Fasting gastrin levels were raised from 55.5 +/- 6.8 pg/ml to 80.9 +/- 6.7 pg/ml (33% increase). On day 39, stimulated gastric acid secretion and fasting gastrin levels have been returned to pretreatment values. Basal levels of prolactin, testosterone, TSH, T3, T4, TBG, cortisol, PTH, 17-beta-oestradiol, insulin, glucagon, c-peptide, dihydroepiandrosterone remained unchanged by a 28-day omeprazole treatment. Omeprazole is a highly effective antisecretory compound without any effect on the basal hormone levels tested. Even after 28 days its effect on acid secretion and fasting gastrin levels was fully reversible.

[Tumor markers in C-cell cancer].

C-cell carcinoma (medullary thyroid carcinoma) is associated with increased serum calcitonin levels. In addition, increased serum levels of other tumour markers are also found: carcinoembryonic antigen (CEA), adrenocorticotrophic hormone (ACTH) and tissue polypeptide antigen (TPA). The relevance of these tumour markers as compared to calcitonin was evaluated in 46 patients with C-cell carcinoma. With decreasing frequency pathologically increased levels were found: calcitonin 93%, CEA 73%, TPA 17% and ACTH 8%. There was a positive correlation between calcitonin and CEA values. Calcitonin and CEA levels correlated with the clinical tumour stage. In selective venous catheterisation estimation of calcitonin is superior to estimation of CEA. Assessment of CEA supplements calcitonin measurements in follow-up controls and evaluation of success of treatment. As regards specificity and sensitivity calcitonin is superior to all other tested tumour markers in C-cell carcinoma.

Electrolyte and glucose metabolism in VX-2 carcinoma of the rabbit.

Biochemical and other parameters in VX-2 carcinoma in rabbits were evaluated. VX-2 carcinoma not only produced hypercalcemia but also hypophosphatemia and 25-OH-vitamin D deficiency. An increased turnover of 25-OH-vitamin D seems likely. Serum parathyroid hormone and urinary cyclic AMP did not increase. Hypokalemia occurred in association with hypophosphatemia and lowered blood glucose within 1 week after tumor transplantation. At the end of the experiment glucose and insulin were both below the control range. It is concluded that VX-2 carcinoma in rabbits yields much more complex biochemical alterations than reported before on calcium metabolism.

[Prevalence of pathologic vitamin D and parathyroid hormone levels in geriatric patients].

Diseases of the bone have always to be considered in geriatric patients, especially because of the therapeutic consequences in osteomalacia (3/100) and hyperparathyreodism (1/100). Therefore calcium, phosphate and alkaline phosphatase should be measured in geriatric patients. The high prevalence of pathologic vitamin D measurements (78/100) in spring points to the importance of outdoor activities.

Different effects of oral glycine and methionine on urinary lithogenic substances.

Nine male healthy volunteers were examined during a control period, during an oral glycine load (45 g/day, 600 mmol) and oral methionine (6 g/day, 40 mmol). Glycine caused a significant increase of urinary oxalate above baseline (from 644 to 797 mumol/day) without change in calciuria (4.74 vs 4.84 mmol/day). In contrast methionine caused no change of oxaluria, but a significant increase in calciuria (from 4.74 to 6.9 mmol/day). Alterations of lithogenic ions in urine after protein ingestion are mediated by different amino acids. The particular lithogenic risk of animal protein may be related to its high methionine/cystine and glycine content.

Changes in gastrointestinal DNA synthesis produced by acute and chronic ethanol consumption in the rat: a biochemical study.

The effect of acute and chronic ethanol administration on DNA synthesis in the gastrointestinal tract of the rat was investigated. Acute intragastric ethanol administration (3 g/kg; 50%) decreased significantly in vivo DNA synthesis when measured 1 hour after alcohol application in the stomach and in the upper small intestine, whereas acute intravenous ethanol administration had no significant effect. In contrast, chronic ethanol ingestion resulted in a significant increase of in vivo and in vitro DNA synthesis in the upper gastrointestinal tract. In addition, even a more enhanced stimulation of DNA synthesis after chronic ethanol consumption was found in isolated intestinal cells. These results indicate an inhibition of gastrointestinal cell regeneration directly after the oral application of ethanol. The enhanced cellular regenerativity observed after chronic ethanol consumption may be secondary to the ethanol induced damage of the gastrointestinal tract.

[The C-cell-tumours of the thyroid gland in childhood (author's transl)].

C-cell tumours are rare in children. They must be classified as hereditary types of the C-cell carcinomas. C-cell hyperplasias are considered as obligatory precanceroses of this malignoma. The article reports on two cases of C-cell hyperplasia discovered within the framework of a family screening study. Attention is drawn to the importance of screening for diagnosis of early tumour stages resulting in improved prognosis.

The effect of high level CaCi2 injection on parathyroid hormone dependent cAMP in stone formation patients.

Patients with a history of idiopathic calcium oxalate stones but without current stone formation do not react to stimulation of parathyroid extract as expected after high dose calcium pretreatment. With Ca2+ pretreatment, these patients show higher serum and also renally generated levels of cAMP after rapid injection of extreme parathyroid extract-concentrations. Healthy controls show a modest increase of serum levels cAMP after similar stimulation with parathyroid extracts. In contrast to patients, calcium pretreatment of controls significantly decreases the renal cAMP response to parathyroid extract stimulation. Without Ca2+ pretreatment, both patients and controls show a strong increase in urinary and also in renally generated cAMP levels following parathyroid extract stimulation.

Homologous radioimmunoassay for human parathyrin (residues 53-84).

We describe a sequential saturation double-antibody radioimmunoassay for carboxyl-terminal fragments of human parathyrin (hPTH) in serum. Standards are prepared with synthetic hPTH (residues 53-84) in hPTH-free serum. Antisera are obtained by immunizing guinea pigs with partly purified hPTH extracted from adenomatous glands. Tracer is prepared by labeling hPTH (53-84), presumably at the histidine residue, with 125I by the Chloramine T method at pH 8.6. Dilution curves for hPTH extracted from adenomas are superimposable on dilution curves for the synthetic 53-84 fragment. Dilution of sera from hyperparathyroid patients showed linearity of response with concentration in the present assay, but non-linearity in the heterologous radioimmunoassay. In contrast to the heterologous system, which discriminated 28 of 32 patients with primary hyperparathyroidism from 32 normals (normal range: undetectable to 54 pmol/L, omitting the highest and lowest values from controls), the present assay separated these groups without overlap.

Vitamin status in patients on chronic anticonvulsant therapy.

Vitamin levels in blood were estimated in 146 epileptics, aged 20-40 years. Compared to healthy subjects, no higher risk rates of a vitamin deficiency were found in epileptics for vitamins B1, B12, A, C, E and beta-carotene. alpha EGR was elevated only in epileptic females, alpha EGOT in epileptic males, indicating a higher risk of a vitamin B2 and B6 deficiency in these groups. Markedly reduced levels of folate, 25-hydroxy-cholecalciferol, and biotin were found in the epileptics; folate and biotin levels showed a significant negative correlation with the total amount and the average daily dose of anticonvulsants administered.

[Early recognition of C-cell carcinoma by family screening (author's transl)].

All available family members of five patients with C-cell carcinoma (medullary carcinoma of the thyroid) were tested for their basal serum-calcitonin level and after pentagastrin stimulation. In two females, aged 12 and 25 years, from two different families, serum-calcitonin was elevated, but only after repeated tests or after pentagastrin stimulation, to definitely abnormal levels. Neither had palpable thyroid nodules, lymph-adenopathy or diarrhoea. The thyroid scan was normal and there were no storage defects. But because of the family history and the elevated serum-calcitonin levels total thyroidectomy was performed in both. The specimens revealed multicentric, bilateral C-cell carcinomas. The serum-calcitonin concentration became completely normal postoperatively, so that one may justifiably speak of a curative effect of the thyroidectomy in these two instances of early diagnosis.
