Incidence of myocardial infarction in the Danish MONICA population 1982-1991.

BACKGROUND: Cardiovascular mortality has been declining in Denmark over the past 20 years. Trends in incidence of myocardial infarction (MI) over the period 1982-1991 are described within the framework of the World Health Organization MONICA Project. METHODS: The DAN-MONICA heart register included all cases of MI in 25-74-year-old men and women living in 11 municipalities around Glostrup County Hospital evolving over a period of 10 years. They were identified retrospectively based mainly on relevant ICD diagnoses in death certificates and hospital discharge reports. Cases meeting WHO-MONICA criteria for definite or possible MI, recurrent as well as first-ever MI, were registered. Subsequent tracing of cases through national registers on deaths and hospitalizations by means of the patient's civil registration number ensured the completeness of the registration. RESULTS: A total of 6025 cases of MI occurred in the period, 4532 among men and 1493 among women. A total of 2923 men and 1047 women had a first-ever MI in the period. The age-standardized rates show a definite decline over the registration period for men and a less distinct decline for women. CONCLUSIONS: The DAN-MONICA heart register meets the requirements for completeness and uniformity throughout the registration period. Causes and magnitude of bias are well described. Even when possible sources of bias are taken into account, the incidence of MI decreased significantly over the 10-year-period 1982-1991 by an average of 5.0% per year for men and 3.5% per year for women.

Partial volume decrease of the thalamus in relatives of patients with schizophrenia.

OBJECTIVE: The authors' goal was to compare the thalamic, total brain, and intracranial volumes of patients with schizophrenia, their healthy siblings, and normal comparison subjects. METHOD: Magnetic resonance imaging (MRI) brain scans were obtained for 32 same-sex siblings who were discordant for schizophrenia and 32 matched normal comparison subjects. RESULTS: Mean total thalamic volume, corrected for total brain volume, was significantly different among affected siblings, unaffected siblings, and comparison subjects. Thalamic volume was smallest in the patients; thalamic volume in their siblings was smaller than that of comparison subjects but larger than that of the patients with schizophrenia. CONCLUSIONS: These results suggest that healthy siblings of patients with schizophrenia partially share the thalamic abnormalities of their affected relatives.

[Incidence of myocardial infarction in Denmark. Results from Dan-Monica 1982-1984].

The mortality from ischemic heart disease in Denmark has changed from a steady increase towards a decrease also in younger males during the 1980s. During this period, the Glostrup Population Studies have registered the incidence of myocardial infarction, risk factor changes and changes in medical care in Copenhagen county (320,000 inhabitants) as part of WHO MONICA, which compares 39 centres in 26 countries. The incidence of myocardial infarction during the first three years, 1982-1984, of the Heart Register, DAN-MONICA at Glostrup Hospital, is reported. The register is complete and valid. If the age specific incidence of myocardial infarction in Denmark was as in this survey, the results indicate an age standardized incidence for the Danish population including all age groups over the age of 25 of 5.58 per 1000 per year (7.48 per 1000 adult males and 3.99 per 1000 adult females).

[Total cholesterol findings in Austria--an overview of epidemiologic studies].

Hyperlipoproteinaemia is the most important public health risk factor for ischaemic heart disease. This publication gives a review of six Austrian epidemiological studies covering more than 87,000 people in all. The results are not representative for Austria, but give a picture of the epidemiology in Austria. Mean values were calculated age-standardized (age-adjusted when ten-year age groups are registered) to the Austrian population of 1985, for the age group 20 to 79 years. This age range covers 99.77% of the adult male population aged 20 or more, and 95.70% of the women. Mean total cholesterol level for the six studies is 224.2 mg/dl in men and 227.3 mg/dl in women. These results are similar to those of a representative study in the province of Vorarlberg in western Austria (220.0 mg/dl, and 227.4 mg/dl respectively). Variation range is 20.4 mg/dl in men, and 22.5 mg/dl in women; the coefficient of variation is 3.9% in both, men and women.

[The prevalence of hyperlipidemia and obesity in Vienna health examinations 1986].

A random sample of 20% of people taking part in the free health check-ups provided by the city of Vienna in 1986 (707 men and 1022 women), aged 20 to 79 years (mean age men 48.1 +/- 15.8, women 49.6 +/- 15.1 respectively) were retrospectively evaluated to determine coronary heart disease risk according to the recommendations of the Austrian Cholesterol Consensus Conference. Age-standardized mean total-cholesterol is 230.0 mg/dl in men and 235.8 mg/dl in women. Age-standardized percentage of normal lipid levels (i.e. total-cholesterol below 200 mg/dl) is 29.7% in men, 25.7% in women respectively. The high-risk group (i.e. total-cholesterol more than 250 mg/dl) has an age-standardized prevalence of 28.5% in men and 35.8% in women. The age-standardized percentage of obesity according to the guidelines of the National Institutes of Health is 14.2 in men, 17.2 in women respectively. The comparison of age-standardized men-values of total-cholesterol, LDL-cholesterol, triglycerides and Quetelet-index, as well as normal lipid levels according to the Austrian recommendations and percentage of obesity, in the age groups 20 to 39, 40 to 59 and 60 to 79 years shows a statistical significant difference of each parameter between age-group 20 to 39 and age-group 40 to 59 in men, but no significant difference in any parameter between age-group 40 to 59 and 60 to 79 in men. On the contrary in women differences are significant between age-group 20 to 39 and 40 to 59, as well as between age-group 40 to 59 and 60 to 79 in each parameter.(ABSTRACT TRUNCATED AT 250 WORDS)

[Epidemiology of hypercholesterolemia in the Vienna area].

Hypercholesterolaemia is the most important risk factor in coronary heart disease (CHD). Epidemiological data are not available for the region of Vienna and the aim of this investigation was to obtain them. The municipal health investigation centres are attended by 6000 to 8000 people from the region of Vienna each year, who may voluntarily use these services once a year without any cost. We analysed total cholesterol, HDL cholesterol and the ratio of total to HDL cholesterol of 733 unselected men and women aged 20 to 79 years, who visited one of the centres in May 1986. The classification was carried out according to the recommendations of the National Institutes of Health (Bethesda, USA, 1984). Using these guidelines 26.0% of the men and 26.6% of the women have normal total cholesterol levels. With regard to total cholesterol 55.0% men and 44.3% women have elevated levels and 19.0% of men and 29.2% of women are at high risk of developing CHD. The mean ratios of total to HDL cholesterol, which is closely related to CHD risk, and the proportion with a value above 4.5, which is associated with elevated CHD risk, are closely related to Austrian trends in ischaemic heart disease, which are unfavourable, especially in middle aged men, the so-called "young myocardial infarctions".

[Results of screening for diabetes within the health-care program of vienna (author's transl)].

Within the health-care program of Vienna 1577 volunteers (1203 female, 374 male persons) were screened for diabetes mellitus during 1975/76. Besides measurement of glucose tolerance one hour after the ingestion of 100 g glucose the following parameters were analyzed: cholesterol, triglycerides, uric acid and blood pressure. Male persons had a slightly decreased glucose tolerance; a distinct relationship with age was found in both sexes. After elimination of volunteers in whom pathological results were obtained with respect to other examined parameters, a significantly greater glucose tolerance was found in the remaining "normal" group. There are no parameters which are closely correlated with glucose tolerance although it was possible to reject the null-hypothesis in several cases due to the large sample involved. For statistical reasons (test sensitivity, test specificity, prevalence of disease) single glucose-tolerance measurements do not give enough evidence for diagnosing diabetes mellitus. Depending on the aims of the screening program one has to define the detection limits as well as frequency of retesting.

[The therapeutic management of consumption coagulopathy in progressive liver cirrhosis: low-dosage heparin therapy (author's transl)].

The high incidence of consumption coagulopathy in active liver cirrhosis prompted us to introduce low-dosage heparin therapy (LDHT) in the management of this condition. An investigation was carried out on 109 patients with clinical and biochemical evidence of progressive liver cirrhosis, which was designed to evaluate whether in addition to basic LDHT, the administration of either vitamin K1, human fibrinogen or partial prothrombin complex (Prothromplex 500) enhanced the results obtained with LDHT alone. The normotest, PTT, thrombin coagulase activity, fibrinogen and platelet count were determined at regular intervals. A significant increase in fibrinogen and platelet count was obtained within 14 days of LDHT in about 75% of the patients and the consumption coagulopathy was halted. Additional treatment with vitamin K1 did not bring about any further increase in the prothrombin complex. Substitution therapy with factors II, IX, X and fibrinogen combined with LDHT brought the expected results. The results reported in the literature and the aims of, and indications for LDHT are discussed.

[Technique of submucous sclerosing in patients with bleeding esophageal varices -- therapeutic results (author's transl)].

Acute hemorrhage from esophageal varices is becoming more and more frequent, implying a poor prognosis for the patient and necessitating dramatic therapeutic procedures by the physician or the surgeon. Regular endoscopic controls of patients with liver cirrhosis and portal hypertension are necessary before the first bleeding occurs in spite of negative X-ray findings of the esophagus, since long-term therapeutic results may be inproved by such controls. Sclerosing the esophageal wall does not occlude the esophageal veins, which do remain open consequently; they are just "displaced" deeper into the mucosa. There are no risks or complications of this procedure especially when esophageal varices are just beginning to form and when the esophageal mucosa is still being intact. Therapeutic results are encouraging, and they are better as compared to the results of emergency shunt or elective shunt surgery. Until short time ago sclerosing the esophageal wall was sort of a preliminary preoperative therapeutic procedure. Today it is a well established therapy in its own right, although it is, as is shunt surgery, symptomatic therapy. It does seem to be superior however to all other forms of therapy aimed at preventing or treating esophageal bleeding, since primary mortality seems to be lower and longterm results seem to be better.

[The detection of hyperlipidaemia during routine preventive medicine investigations. Serum lipid values in 3006 viennese adults (author's transl)].

1. 3006 adult volunteers (1001 women after an overnight fast of at least 12 hours and 2005 women and men, 6 to 8 hours after a light breakfast) were investigated in regard to serum cholesterol and triglyceride levels, body weight and height. 2. 73.8% of the men and 75.2% of the women were overweight. The degree of overweight increased with increasing age and only decreased in the advanced age groups. 3. Serum triglyceride and cholesterol values increased with increasing age and showed a decrease in the older groups. 4. Serum triglyceride and cholesterol levels increased with increasing weight, also within each single decade. 5. 21.7% of the 1001 fasting women showed values for TG over 200 mg/dl and/or Chol over 300 mg/dl.

[The diagnosis of cholestasis: lipoprotein X (LP-X) (author's transl)].

The diagnostic specificity of a new method to detect obstructive jaundice by determination of lipoprotein X (LP-X) was tested in 144 patients with different kinds of hepatic diseases and compared with the usual chemical "obstructive jaundice specific" tests, such as bilirubin, SGOT, SGPT, alkaline phosphatase, LAP and gamma-GT. The LP-X test was performed by using all-in test kit LP-X Rapidophor" low-voltage electrophoresis of Immuno AG/Wien. The results were correlated with the histological classification of the liver biopsy specimen. In 82% of the histologically verified cases of obstructive jaundice the result of the LP-X test was positive, whilst in 98.5% of the histologically negative cases the result of the LP-X test was negative. Hence, this LP-X method proved superior to chemical methods in providing a clear-cut positive or negative answer to the presence of cholestasis. Furthermore, the LP-X test was suitable for long-term follow-up investigation of patients with obstructive jaundice.
