Hyperhomocysteinemia in high-aged subjects: relation of B-vitamins, folic acid, renal function and the methylenetetrahydrofolate reductase mutation.

Moderate hyperhomocysteinemia is an atherogenic risk factor and plays an important role in geriatrics. Here, we have investigated the role of hyperhomocysteinemia in two elderly groups: 104 longeval subjects of 85-102 years, 100 seniors aged 65-75 years, and 75 controls of 19-60 years. Elevated homocysteine levels were found in 58% of longeval subjects in comparison with 32% in seniors. The homocysteine level in serum correlated positively with age as well as serum creatinine, and inversely with serum folate, but there was no correlation with serum B-vitamins. The frequency of vitamin B deficiency in serum of longeval subjects compared to seniors was as follows: vitamin B6 43% vs. 22%, vitamin B12 20% vs. 8%, and folic acid 1% in both groups. Increased serum creatinine levels (> 1.1 mg/dl) were found in 63% of the longeval subjects and in 32% of seniors. The 677-missense mutation in the methylenetetrahydrofolate reductase (MTHFR) gene, responsible for moderate homocysteine elevation, was found in 35, 37 and 27% of alleles in longeval persons, senior subjects and younger controls, respectively, showing no significant difference in frequency distributions of the MTHFR gene mutation. It can be concluded that hyperhomocysteinemia is very common with increased age. Its importance as an atherogenic risk factor with advanced age has to be clarified.

Modulations of the effector function and cytokine production of human lymphocytes by secreted factors derived from colorectal-carcinoma cells.

We investigated the in vitro effects of factors secreted by 3 freshly explanted human colorectal-carcinoma (CRC) cell lines on lymphocyte proliferation, IL-2-receptor expression, LAK-cell generation and cytokine secretion. We found that the supernatants of all 3 CRC cell lines inhibited T-cell proliferation in a dose-dependent manner, due to the secretion of immunosuppressive factors (ISFs). In addition, the supernatants of 2 cell lines were able to inhibit LAK-cell generation and to depress IL-2R, but not HLA-DR expression, on PHA-activated T cells. Furthermore, the secretion of cytokines, i.e., IFN-gamma, IL-1beta, IL-2 and TNF-alpha, by peripheral-blood mononuclear cells (PBMC) was differently modulated by the tumor-cell supernatants, e.g., the production of IFN-gamma was reduced in normal PBMC stimulated with PHA. However, the effects induced by the supernatants were not identical: for example, factors from one CRC cell line (w25) influenced early and late events of T-cell activation and division, while 2 others (w19 and te6) contributed only to the inhibition of early events. Some biochemical properties of the ISFs were characterized. Our results suggest that colon-tumor cells can secrete ISFs, which may lead to the in vivo immunosuppression often observed in patients with these tumors.

[A modified Sarafoff suture for closure of the uterine wound in Cesarean section].

Using a retrospective investigation the authors demonstrate the advantage of the modified Sarafoff-suture (a one layer suture of the myometrium) in comparison with the two-layer suture. The results shows a lower rate of febrile courses, less postoperative complications and a shorter hospital stay. The first clinical experiences showed less scar complications in the courses of vaginal delivery in patients with a previous caesarean section. Therefore, the authors advise the modified Sarafoff-suture for wound closure in caesarean section.

[Urethral wall pulsation in pregnant patients, continent and stress incontinent females].

The amplitudes of urethral vascular pulsations in pregnant women, continent and urinary stress incontinent women were compared with each other. Significant differences have been found in the chi 2-Test. In pregnant women the mean pulsation amplitude was 1.81 kPa, in continent women it was 1.13 kPa and in urinary stress incontinent women 0.46 kPa. Low or missing urethral vascular phenomena seem to be connected with urinary stress incontinence. That is why the amplitude of urethral pulsation should be taken into consideration in the urodynamic investigation of urinary stress incontinence.

[Transient hyperphosphatasemia in childhood].

During hospitalisation four infants were found to have a marked increase in serum alkaline phosphatase, which could be attributed neither to clinical nor paraclinical diseases and is considered transitory hyperphosphatasaemia. This phenomenon is briefly discussed on the basis of the literature. The children should be protected from extensive diagnostical measures.

Accessory ovary.

A case is reported of an accessory ovary in a 46-year-old woman. Accessory ovaries are frequently small and may therefore be overlooked at laparotomy. The present case was presumably discovered due to gonadotropic hyperstimulation caused by the menopause or the previous bilateral ovarian resection.

[Intelligence tests in patients suffering from chronic rhinopathy (author's transl)].

In 100 patients referred to the allergy clinic of the First Otolaryngological University Hospital of Vienna because of chronic rhinopathy an exact allergy diagnosis was carried out, comprising anamneses, skin tests and laboratory tests (Rast, Prist). The test results have yielded two or three diagnosis groups: patients with rhinitis vasomotorica nonallergica, rhinitis vasomotorica allergica, and pollinosis. In addition, in all patients an intelligence test by means of WIP (a short form of HAWIE) was carried out. The IQ of all patients tested is, on the average, significantly above the average of the normal population, the variation in all three groups being significantly below the average of the normal population. The reason for the low degree of variation is that patients with low IQ values were missing. Earlier tests carried out by this clinic (allergy-screening test in 3500 school pupils) had shown that the percentage of allergy patients among pupils with low IQs (special schools) and other children (secondary-schools) cannot be assumed to be of a varying degree. This fact demonstrates that although allergy patients with below-average intellectual ability do exist, they obviously do not consult the respective special clinics. This paper tries to explain this phenomenon and suggests that the socially lower strata be increasingly informed about the problem of allergy.
