[Malignant diffuse pleural mesothelioma: a comparison between computed tomography and the conventional chest x-ray].

The rate of incidence of malignant pleural mesothelioma is increasing although diagnosis of this disease may be very difficult. Computed tomography examinations and conventional chest x-rays of 30 patients with malignant pleural mesothelioma were reviewed independently, to evaluate the role of both modalities. CT offered the following advantages over conventional chest x-ray examinations: CT was more sensitive in detecting the smooth and nodular changes of malignant pleural mesothelioma, especially at the diaphragm and lower parts of the thorax, tumours could be better demonstrated at the pericardium and in the greater fissure, in some cases, where chest films showed just abnormal widening of the mediastinum, CT could differentiate between tumour involvement of the mediastinal pleura and local invasion of the mediastinum by the tumour, CT was more effective in detecting pleural calcifications and thickening of the contralateral pleura. CT proves more accurate in assessing the extent of the disease, and gives additional diagnostic help.

Effects of cholecystokinin-pancreozymin on bile flow and biliary bile salt secretion in the cholecystectomized animal.

The effects of an intravenous infusion of cholecystokinin-pancreozymin on bile secretion were studied in 13 unanesthetized and cholecystectomized piglets. The results demonstrate that CCK-PZ induces a significant increase of bile flow and bile salt excretion. The excretion patterns for bilirubin and Ioglycamide, however, showed a slight but not significant decrease. Peripheral hemodynamic parameters remained unchanged. A detailed analysis of these results suggests that CCK-PZ induces an increase of biliary bile salt secretion by stimulation of intestinal motility and enhancement of bile salt circulation within the enterohepatic circulation (EHC). In combination with previous studies on the effects of spasmolytics on biliary bile salt excretion these results therefore emphasize, that the intestinal motility and transit time might play a role in the physiology and pathophysiology of biliary bile salt secretion.

Inhibition of bile salt excretion and bile flow by spasmolytic drugs. An additional therapeutic effect in the treatment of biliary colics?

The influence of the spasmolytic drugs hyoscin-N-butylbromide (HBB, Buscopan) and Extractum opii (E. opii) on bile flow, bile salt and bilirubin excretion is investigated in the cholecystectomized, unanaesthetized pig and compared with the results after acute, quantitative interruption of the enterohepatic circulation (EHC) by biliary drainage. The results demonstrate that HBB and E. opii induce a significant decrease in bile flow and bile salt excretion. A further analysis suggests that this effect is due to an inhibition of intestinal motility and transit time, followed by a delay of the EHC of bile salts with a decrease in biliary bile salt excretion and bile salt dependent bile flow. The results point out a second mechanism that might be responsible for the effectiveness of spasmolytic drugs in the treatment of biliary colics: the lowering of intraluminal pressure by inhibition of bile salt dependent bile flow.

[Studies on the biliary excretion of the hepatotropic contrast media ioglycamate and iotroxinate in pigs and men (author's transl)].

The distribution and biliary excretion of two hepatotropic contrast media, Ioglycamate and Iotroxinate, has been studied in the anaesthetized pig in infusion experiments. The new substance Iotroxinate exceeds the Ioglycamate in two of the investigated parameters: it was excreted faster and reached a higher biliary concentration. The biliary transport maximum, however, was unchanged. On the basis of these experiments the new contrast medium Iotroxinate was tested in men.

The ultrasonically guided puncture of the gallbladder in amimals. A new methodological procedure for a simple and non-surgical collection of gallbladder bile.

In 4 piglets the gallbladder was punctured under sonographic control threetimes during a period of 46 days. Each puncture was proved by X-ray controls after application of 2-4 ml of a water soluble contrast medium. One day after the third puncture all animals were killed in order to look for macroscopic or microscopic lesions of the gallbladder, the adjacent liver tissue and the peritoneum resulting from the experimental procedure. The results indicate that the ultrasonically guided puncture of the gallbladder represents a procedure to obtain pure bile samples in the piglet or animals of similar size in a simple and non surgical and, therefore, most physiological manner.
