[Value of using polyethylene glycol in the indirect antiglobulin test with erythrocytes].

A series of comparative investigations was carried out for establishing the value of the indirect antiglobulin test (IAT) in its first phase. The results showed that PEG, in relation to NaCl solution of low ionic strength, increased the reactivity of various antibodies of different specificity present in 72 studied sera. A significant superiority of IAT-PEG to IAT-BISS was noted in 45.8% of these sera. In search for antibodies in sera of 1160 patients IAT-PEG detected 7 antibodies not reacting in the IAT-LISS, and 5 of them had Rh system specificity, one was anti-K and one anti-Jka. Although the IAT-PEG was more sensitive than IAT-LISS in the detection of anti-Rh antibodies, it was less sensitive in this range than the enzymatic LEN test with which not only 5 of these antibodies were demonstrated but which detected also 8 antibodies not demonstrated in IAT-PEG or IAT-LISS. All 5 antibodies not detected by the IAT-PEG but demonstrated in the IAT-LISS were specific for the Lewis system. The per cent of reactions regarded as non-specific in the IAT-PEG was 1.3%. The IAT-PEG modification may be recommended as a sensitive, simple and specific method extending the possibilities of immunohematological diagnosis, especially in cases of post-transfusion reactions and in difficulties of interpretation connected with the occurrence of weakly positive reactions in the IAT-LISS.

Blood Group Antigens

[Dubious results of determining the presence of anti-HIV-1 antibodies in blood donors].

In 588 sera of blood donors the Western Blot test (WB) was done for confirming the presence of anti-HIV-1 antibodies. Negative results were obtained in 247 sera and positive in 90. In 251 cases the result was doubtful (42.7%). These results were obtained in 153 cases, however only 104 subjects came for control tests and had from 1 to 5 tests. Eight subjects were returned to the register of blood donors after obtaining of two negative WB results. In 5 cases in control test the serum reacted with the protein of HIV-1, and they were regarded as being carriers. In the remaining 91 subjects doubtful results persisted for from 3 to 12 months. The reactivity of the doubtful sera changed. Most of them reacted with the proteins opceded by one of three genes, they were mostly core proteins, among them p24 protein. The remaining sera reacted with proteins encoded by genes of the envelope and core. The possible causes of these doubtful results are discussed.

[HIV-1 infection in blood donors and blood recipients].

The results of the tests on HIV-1 infection in blood donors in the years 1985-1989 were presented. Out of 3.222.345 serum samples, in 66 donors HIV-1 antibodies were detected. In 1989 the significant raise of HIV-1 carriers, among whom there were 17 intravenous drug users, was noticed. The transmission of HIV-1 infection to 9 blood recipients was proved. Medical examination of 20 infected blood donors revealed in all of them the enlargement of peripheral lymph nodes. In 8 donors candidosis of oral cavity was diagnosed. The undertaken analysis showed the low efficacy of self deferral and pointed out that medical examination before blood donation should be more carefully done.

[Diagnosis of chimerism based on blood group determination].

The authors report the results of blood group determinations in a 9-year-old girl in whom two populations of erythrocytes were discovered. The larger population accounting for nearly 80% of erythrocytes contained the antigen A2, the smaller population had the B antigen. Besides that differences were observed in the antigens of Duffy and Lewis systems. In the saliva A and H antigens were revealed. Familial investigations indicated the participation of two gametes of maternal origin (mother--AB, father--O) although no evidence of twin pregnancy was available. The cause of this chimaerism has not been explained.

[Sera showing the ability of erythrocyte panagglutination].

The properties of a factor causing panagglutination of erythrocytes are described. The factor was found in the sera of three patients when blood was tested for transfusion. Panagglutination appeared in the indirect antiglobulin test when the erythrocytes were being washed with 0.15 M NaCl without addition of phosphate buffer. The method for cross testing in such cases was elaborated, with washing the cells with 0.15 M NaCl buffered with phosphates.

[Remarks on routine use of the enzyme test LEN (LISS Enzyme) in immunohematological studies].

The authors report one-year experience with the enzymatic test LEN (LISS-Enzym) replacing the one-step papain test in the compatibility testing, and in place of the two-step papain test in the detection and identification of red cells allo-antibodies. A wide range of investigations confirmed the usefulness of this test in the detection and antibodies specificity, mainly from the Rh system which are often undetectable by means of the indirect antiglobulin tests. It is concluded that in compatibility tests the LEN test should replace the low-sensitivity one-step papain test used as yet. It is stressed that in case of positive reactions only in the LEN test suggesting presence of autoantibodies the test should be repeated bringing to 37 degrees C the reacting components before their mixing. This makes possible elimination of clinically not significant cold autoantibodies.

[Unusual cis AB phenotype in ABO group system].

The authors present serologic characteristics of the "cis AB" phenotype in the ABO blood system on the basis of investigation of three families in Poland and data from the literature and genetic concepts explaining the mechanism of development of this phenotype.

[Autoimmune hemolytic anemia with presence of 3 types of autoantibodies to erythrocytes].

In a patient with chronic lymphatic leukaemia severe autoimmunohaemolytic anaemia (NAIH) was observed with coexistence of cold incomplete IgM, warm incomplete IgG and warm haemolysins active against papainized erythrocytes in low pH medium. The serologic and immunochemical characteristics of autoantibodies are presented and the mechanism of their action in vitro is tentatively explained.

[Rare erythrocyte phenotypes imitating weak variant of antigen A].

Two phenotypes with defect of sialic acid in the cell membrane of erythrocytes due to some unknown factor, imitating the weak variant of antigen A are reported. One of them found in a healthy candidate for blood donation was called Tn. The second one associated with thrombocytopenia and anaemia differed from all as yet classified ones in that it had simultaneously increased expression of antigen H and lack of reaction with Dolichos biflorus lectin. For demonstrating the diagnostic difficulties differentiation against the weak variant of antigen A approaching A3 is described.

[Anti-U antibodies in autoimmune hemolytic anemia].

The results of serological investigations in 23 patients with autoimmunohaemolyts anaemia (AIHA) and warm-type autoantibodies belonging to IgG immunoglobulins are presented. The serological specificity of autoantibodies was demonstrated in 8 cases, and in 4 of them specificity to single antigen of the Rh system was demonstrated, in the remaining 4 cases they were specific against U antigen. The method of identification of these autoantibodies and their characteristic features are described. Analysing the clinical course of AIHA in three cases with anti-U autoantibodies it was stated that was chronic, intermittently recurring and was not associated with proliferative disorders of the haematopoietic system.

[Monoclonal origin of autoantibodies of the warm type in hemolytic autoimmune anemia].

In 59 patients with warm-type autoimmunohaemolytic anaemia classes and types of immunoglobulins, presence of complement, serological specificity of autoantibodies, and in 14 selected cases also IgG subclasses were determined. It was found that warm autoantibodies differed from anti-Rh alloantibodies. In 27 patients only one type of L chain was found but, in the light of complex investigations, the authors reached the conclusion that this result cannot be sufficient for accepting autoantibodies as monoclonal immunoglobulins, since in some of them 2 or 3 IgG subclasses and more than one serologic specificity were demonstrated. These observations indicate the need for more precise investigation methods for explaining the problem of monoclonal origin of warm autoantibodies.

[Light chains of warm autoantibodies in patients with autoimmune hemolytic anemia. Long-term observation].

In 18 patients with autoimmunohaemolytic anaemia presence of light chains of IgG autoantibodies was south for by the method of direct antiglobulin reaction. In 11 patients variability was observed in the detection of light chains. The authors discuss the possible causes of this observation: variable participation of different cell lines in the production of autoantibodies and changes in the conformation of immunoglobulin molecule in the erythrocyte.

[Detection of defects in components of antigen I and its significance in transfusiology].

The authors present the results of family investigations carried out in a healthy man whose serum contained anti-IDBH alloantibodies. In the erythrocytes of the propositus and one of his brothers defects in ID and IS components were found. At the same time increased expression of IT component was observed in both these men and in their brother. In the erythrocytes of the remaining family members no abnormalities in the components of I complex was found. The authors stress the importance of detection of these defects in the practice of transfusiology in patients with autoimmunohaemolytic anaemia.
