Clinical and molecular genetic evaluation of patients with primary dystonia.

Primary dystonia is a movement disorder characterized by involuntary and sustained muscle contractions causing twisting or abnormal postures and mutations in several genes have been identified. Our goal was to investigate, whether the clinical presentation would differ between patients with a positive family history, and patients without. Furthermore, we have performed mutation analysis in the subgroup of patients with a positive family history. A total of 175 patients with primary dystonia were evaluated. Data on gender, presence and frequency of pain and tremor, age of onset, and the distribution of affected body parts were compared between patients with positive and negative family history. All exons of the torsion dystonia 1, GTP cyclohydrolase 1 and epsilon-sarcoglycan genes were examined in 40 patients by SSCP analysis of PCR products followed by sequencing of variant conformers. Dystonia patients with a positive family history of dystonia had an earlier age of onset and those with a positive family history of tremor more often associated tremor than those with a negative family history. Four new polymorphisms in the epsilon-sarcoglycan gene were found and others confirmed, but no known or new mutations could be detected. Our study supports the notion that primary dystonia is a genetically heterogeneous disease.

Outcome of tuberculosis retreatment in routine conditions in Cotonou, Benin.

SETTING: National Tuberculosis Programme (NTP), Cotonou, Benin. OBJECTIVE: To study the patient characteristics and outcome of tuberculosis retreatment cases in a well-functioning NTP. METHODS: A retrospective, register-based study of all smear-positive pulmonary tuberculosis cases put on retreatment (2SERHZ/1ERHZ/5R3H3E3) between 1992 and 2001 in Cotonou. For comparison, information on new smear-positive cases in Cotonou in 1999 was entered and analysed. RESULTS: Of 8103 tuberculosis patients registered, 642 were put on retreatment. The analysis is mainly based on the 236 patients whose initial treatment regimen records were available (113 relapses, 84 failures, 39 returns after default). Most of the relapse (57%) and return after default (72%) cases were put on retreatment within 12 months after stopping their initial treatment. Overall, the retreatment results were satisfactory (78% success) and comparable with those of new cases (82%); the failure rates were low (3%), as were those for initial treatment (1%). There were more defaulters from retreatment among those who had already defaulted from initial treatment (21%). Treatment success rates were better among women than men. CONCLUSION: The standardised retreatment regimen is effective in Cotonou, probably because the NTP is functioning well, there are no drug shortages, drug taking is strictly supervised, and a good treatment plan is followed.

EuroBionet: a pan-European biomonitoring network for urban air quality assessment.

EuroBionet, the 'European Network for the Assessment of Air Quality by the Use of Bioindicator Plants', is an EU-funded cooperative project currently consisting of public authorities and scientific institutes from 12 cities in 8 countries. In 2000, the bioindicator plants tobacco (Nicotiana tabacum Bel W3), poplar (Populus nigra 'Brandaris'), spiderwort (Tradescantia sp. clone 4430), Italian rye grass (Lolium multiflorum italicum) and curly kale (Brassica oleracea acephala) were exposed to ambient air at 90 monitoring sites according to standardised methods. Visible injuries and growth parameters were assessed and the accumulation of toxic substances in leaves determined. The exposure of tobacco resulted in a gradient with low levels of ozone-induced foliar injury in N and NW Europe, and medium to high values in the southern and central regions. The results of heavy metal and sulphur analyses in rye grass samples generally showed low to very low sulphur and low to medium heavy metal concentrations in leaves. In some cities, however, local hot spots of heavy metal contamination were detected. Analyses of the PAH contents in curly kale leaves gave low to medium values, with locally elevated levels at traffic-exposed sites.

[Genetic subtyping of HIV-1 in Liaoning province of China].

OBJECTIVE: To study the prevalence of HIV-1 in Liaoning province of China. METHODS: Nuclear acids were extracted from blood samples of 16 HIV-1 infected individuals collected locally in Liaoning province of China from Jun. 1997 to Dec. 2000. The 0.7 kb or 1.2 kb segments of HIV-1 env gene were amplified using nested-PCR and the HIV-1 genetic subtypes were then assayed by heteroduplex mobility assay. RESULTS: Fifteen of 16 samples were positive by PCR amplification of HIV-1 env region and samples were found to be genetic subtype A,B',C,E. The proportion due to sexual transmission in all HIV infection was 31.25% (5/15), among which subtype B' (3/5) was the majority. A man who returned from Africa together with his spouse both had type A (2/5) infection. Intravenous drug users (IDUs) took up 31.25% (5/15) of all the HIV infections. Subtype C (2/4) and E were predominant among intravenous drug users. However, there was one IDU with subtype B or E. Nearly all blood recipients and blood donors were B' (4/5) except one with C. CONCLUSION: There have been several subtypes of HIV-1 existed in Liaoning province, demonstrating the complexity of HIV epidemology in Liaoning province and the difficulty conducting prevention and treatment.

Event-related brain potentials elicited by a number discrimination task.

To study the number cognition process, we recorded event-related potentials (ERPs) in 14 subjects when they were indicating whether a number pair was the same or not. The two numbers given in sequence were either the same (condition 1) or different (condition 2). After 270 ms following the onset of the second stimulus in condition 2, a negative component N270 was recorded on the scalp with the most negative amplitude at the central and occipital areas. Hemispheric asymmetry was not observed in the potential. This negative component is considered to reflect the mismatching process for numbers in the brain.

Early-latency somatosensory evoked potentials elicited by electrical acupuncture after needling acupoint LI-4.

The stimulating methods of prior studies on somatosensory evoked potentials (SEPs) elicited by acupoint stimulus had involved surface electrodes, while the clinical practice of acupuncture is mostly performed by inserting the acupuncture needle inside the body. Clinical observations show that there are often some special sensations when LI-4 is needled. To observe if the SEPs produced by acupoint acupuncture had a distinguishing property, we studied the SEPs elicited by electrical acupuncture after the acupuncture needle was inserted into LI-4 and its control point, and then mapped them with the 128-channel Electric Brain Signal Image system. We also compared this to SEPs by median nerve stimuli. Results showed that the most interesting finding was the marked differences of N1-P1 and N2-P2 amplitude between SEPs at LI-4 (SEP-LI) and its control point (SEP-CP), which were in the opposite direction. Marked differences were also found between latencies and amplitudes of the SEPs elicited by acupuncture and by median nerve stimulus (SEP-M). The differences between SEP-LI and SEP-CP might be due to the additional effects of the activation of nerve endings and muscle spindles in LI-4 to the SEPs formed by the activation of superficial and deep radial nerves during electrical acupuncture. The differences between SEPs to acupoint and median nerve stimuli might be mainly due to the different distances from the stimulated regions to the cerebral cortex, the diversity and the number of activated fibers.

[Cloning and expression of a resistant Culex gene in Escherichia coli].

Recombinant plasmid pRLB1 was constructed from detoxifying gene B1 of pesticide resistant Culex pipiens quinquefasciatus and from plasmid pRL439 contained the strong promoter PpsbA. The positive clone was identified by digestion and Southern analysis. Expression of recombinant plasmid containing esterase gene was detected. An engineered bacterium possessing high enzyme activity was obtained and immobilized. It can effectively degrade the specific substrate alpha-naphthyl acetate (alpha-NA) and beta-naphthyl of esterase enzyme. Assays showed that pesticide acetofenate (7504 an organic choride pesticide) was degraded by the immobilized cells within one hour.

Brain potentials during mental arithmetic-effects of problem difficulty on event-related brain potentials.

One addend '+' symbol and another addend were presented in sequence to subjects in a monitor, and event-related brain potentials (ERPs) were recorded at the same time to examine the effect of problem difficulty (with or without carrying in solution) on ERPs. After the presentation of the second addend, N1, P1, N2, late positive complex and slow waves were recorded. The P2 amplitude at F3 site for the difficult arithmetic problems between 168 and 184 ms is larger (more positive) than that for easy problems (P < 0.05). The mean latency of P2 at F7 and P3b at F3 and F4 is significantly longer for difficult problems than that for easy ones (P < 0.05). It is suggested that prefrontal activity may be involved in the arithmetic data retrieval process. ERPs is modified to different degrees by changing the difficulty of mental arithmetic.

Behavior of pythium Torulosum zoospores during their interaction with tobacco roots and Bacillus cereus.

Bacillus cereus UW85 suppresses seedling damping-off diseases caused by Oomycetes and produces antibiotics that inhibit development of Oomycetes in culture. The goal of this study was to determine how UW85 and its antibiotics affected the behavior of an Oomycete, Pythium torulosum, in its interaction with plant roots. We studied tobacco seedlings inoculated with zoospores of P. torulosum and UW85 culture, culture filtrate, washed cells, antibiotics (zwittermicin A or kanosamine), purified from cultures of UW85, and UW030, a mutant of UW85 that does not suppress disease and does not produce the antibiotics. Microscopic observation revealed that all of the treatments inhibited zoospore activity around roots and encystment on roots. Treatment with UW85 culture, culture filtrate, zwittermicin A, or kanosamine delayed cyst germination and the elongation rate of germ tubes, whereas treatment with UW030 or washed UW85 cells did not. In an in vitro seedling bioassay of disease suppression, the antibiotics, zwittermicin A and kanosamine, suppressed disease singly or together, although UW85 culture suppressed disease more effectively than did the antibiotics. The results show that B. cereus cultures affect zoospore behavior in the presence of roots, and B. cereus-produced antibiotics, zwittermicin A and kanosamine, contribute to disease suppression and inhibition of germ tube elongation in the presence of the plant root.

[The experimental treatment of laryngeal carcinoma with all-trans retinoic acid].

OBJECTIVE: To investigate whether all-trans retinoic acid (RA) has antitumor effect in vivo. METHODS: Sixteen nude mice were transplanted with human laryngeal cell carcinoma PHC3. Eight of them were injected with RA intraperitoneally and others with solvent as the control. Response was measured and tumor histopathological features were studied. RESULTS: The growth and weight of RA-treated tumors were significantly reduced in comparison with those of the control. The growth inhibition rate reached more than 50% in the RA group. Studies also showed that RA-treated cells became well-differentiated. There was a significant decrease of c-myc protein level in the RA treated tumors as compared with the control. CONCLUSION: It is suggested that RA could induce differentiation of human laryngeal carcinoma cells and suppress tumor growth in vivo. Regulation of c-myc gene or protein may be related to those happening.

Transmission of tuberculosis in the metropolitan area of Zurich: a 3 year survey based on DNA fingerprinting.

Between 1991 and 1993, 444 inhabitants of the metropolitan area of Zurich were reported as confirmed or suspected cases of tuberculosis (TB). Overall, isolates of Mycobacterium tuberculosis of 361 patients (90% of the bacteriologically confirmed cases) were available to study the frequency of transmission of the strains on a molecular level. Restriction fragment length polymorphism (RFLP) analysis was performed by using IS6110 and the polymorphic GC-rich sequence (PGRS) as genetic markers. Ninety nine isolates shared by 77 patients (21.3%) were associated with 28 IS6110-defined clusters. However, secondary typing of low copy number isolates decreased the number of clusters to 25, encompassing 81 isolates from 63 (17.5%) patients. By deoxyribonucleic acid (DNA) fingerprinting plus conventional contact tracing, definite transmission of TB was proven in only five patients (1.4%) and assumed in 20 patients (5.6%). In all other cluster-associated isolates, no epidemiological connections between the patients could be found using the clinical and sociodemographic data available. The present study demonstrates that in the time period studied only minor transmission occurred.

[Ultrastructures of a mycoplasma-like organism causing mulberry dwarf disease].

The pathogen causing the dwarf disease of mulberry in Tongxiang, Zhejiang Province was investigated based on transmission electronic microscopy of the leaf vein of diseased plants collected in the field. A mycoplasma-like organism was identified in the sieve tube, companion, and parenchyma cells of the phloem tissues infected. The organism was spheric or oval in shape, ranging from 50 nm to 160 nm in diameter, and with bilamina unit membrane of 8-10 nm in thickness, and was filled with nuclear strands or fibrous material. Some nuclei of mesophyll cells in the diseased leaves were found disrupted as their nuclear membrane was broken, followed by of nucleo-cytoplasmic matrix and dispersive disappearance of nucleoli. Furthermore, other ultrastructural aberration including depletion of chloroplast stroma, disruption of chloroplast grana, increase of mitochondria and rough endoplasmic reticulum, and discomposition of some mitochondrial cristae were also observed associated with the pathological changes caused by the mycoplasma-like organism. In contrast, this type of organism was not found in healthy plants examined. Thus, the mycoplasma-like organism is likely to be a pathogen attributable to the mulberry dwarf disease.

[Antianoxic effects of Dracocephalum tanguticum on brain of mice].

The aqueous extracts i.p. of Dracocephalum tanguticum Maxim (DtM), a Tibetan medicine, prolong sighificantly decapitaion-induced gasping duration in mice and the survival time of mice ligated bilateral carotid (P > 0.01). DtM prolong also the primal gasping in mice after injecting the air into the coccygeal vein (P > 0.01) and the survival time in mice poisoned by Lidocaine (P > 0.05). In addition, DtM is possessed of protecting against the pathologic lession of brain in rats under stimulating high altitude hypoxic condition. The results suggest that DtM has a antianoxic effects on mice.

[Tuberculosis therapy in canton Zurich 1991-1993: what are the causes for recurrence and therapy failure?].

A retrospective cohort analysis of TB patients in the Canton of Zurich was undertaken to evaluate the effectiveness of a primary care physician-based tuberculosis treatment program in the Canton of Zurich (1991-1993) and compare it with treatment results in selected patient groups in controlled prospective studies. Of the 440 TB patients (62.5% men, 51% foreign born, 9.6% with HIV infection) 65% were initially hospitalized. The total treatment duration was 210 (+/-108) days on average. Only 75% of the patients who were treated by a total of 410 hospital and primary care physicians were treated with standard regimens according to guidelines. 3 patients were treatment failures; 13 relapsed (5.1% of the 257 patients reexamined in 1995). Possible causes for relapses were poor patient compliance due to alcoholism, homelessness, drug addiction, psychiatric disorders; prescription errors with inadequate treatment durations, concurrent AIDS disease, or use of a three-drug combination despite primary drug resistance. In 4 of the relapse patients and in all 3 treatment failures, secondary drug resistance developed under treatment. 5 of these 7 patients were treated initially only with a fixed three-drug regimen (Rifater). The relatively high INH resistance rate (8.6%) and relapse rate (5.1%) in the Canton of Zurich renders the use of a four-drug combination therapy necessary. In the outpatient maintenance phase, more effort must be devoted to ensuring patient compliance and adherence to a proper treatment duration. This should be documented by a standardized questionnaire at 6 or 12 months after initiation of therapy. In high risk patients in particular, home care visits by a TB nurse in addition to consultations by primary care physicians are necessary to ensure directly observed therapy (DOT) in order to prevent relapses and treatment failures.

AIDS-Related Opportunistic Infections↗

[Tuberculosis in Switzerland].

In Switzerland, in 1992, 957 persons suffered from tuberculosis; 52.3% were Swiss, 47.7% foreigners. Most of the swiss TB patients were more than 65 years old, whereas the foreigners generally were young patients originating from countries with high TB-infection rates. Asylum seekers had much higher TB-case rates (131 cases per 100,000) than other foreigners (27 cases per 100,000) or Swiss (9 cases per 100,000). Special refuge reception centers have been set up in Switzerland, in charge of tuberculosis screening procedures in this high-risk group on arrival to this country. Although HIV and AIDS patients bear a much higher risk of developing tuberculosis once infected, the HIV epidemic did not lead to an increase of tuberculosis in Switzerland so far, since young Swiss are rarely infected with tuberculosis. HIV-infected, drug addicts, homeless persons and alcoholics run a higher risk of contracting tuberculosis only when congregating with a person suffering from active tuberculosis not yet diagnosed or improperly treated. In order to maintain low levels of tuberculosis in Switzerland DOT (directly observed therapy) must be implemented in all patients with uncertain compliance, especially as cultural and social backgrounds become increasingly complex.
