Acute promyelocytic leukemia associated with insulinoma: a case report.

We report a case of acute promyelocytic leukemia (APL) coexisting with insulinoma in a 61-year-old female. Two years before the onset of APL, she was diagnosed as having insulinoma and underwent resection of the body and tail of the pancreas, but no insulinoma was found in the resected pancreas. The clinical symptoms of hyperinsulinism, however, continued after the operation and were treated with diazoxide (100 mg daily, 37 g in total) for about two years until the onset of APL. During the induction therapy for APL, she died of hemorrhage and infection; autopsy revealed the presence of insulinoma (13 X 9 mm in size) in the residual pancreas head. This is the first case report of coexistence of APL and insulinoma.

[Eosinophil kinetics].

The kinetics of 111In-oxine labeled eosinophils were studied in 3 cases of reactive eosinophilia and were compared with those of neutrophils in patients with CML and neutrophilia. The disappearance curve of the labeled eosinophils consists of two exponential components. There was a slightly increase of radioactivity between these two components, suggesting the presence of recirculation. The disappearance rate of eosinophils was faster than that of neutrophils in CML and neutrophilia. In the cases of eosinophilia and CML, the marginal pool was larger than the circulating pool. In CML, the marginal pool was the largest among these three disorders. The granulocyte turnover rate in eosinophilia was less than in CML or chronic neutrophilia. It is suggested that the migration of eosinophils is less active than that of neutrophils.

[Acute promyelocytic leukemia with a history of RAEB in transformation and the 15/17 translocation].

A 34-year old female was admitted to our clinic because of fever and general fatigue on March 26, 1987. On admission, peripheral blood (PB) revealed pancytopenia. Bone marrow smears revealed 9. 0% of promyelocytic cells with or without Auer rods. Diagnosis of RAEB in transformation was made. Chromosome study of the bone marrow cells showed t(15; 17) in 3 out of 20 cells analysed. After 3 months, the leukemic cells were observed in PB and increased in number. Then the patient showed bleeding tendency and fibrin degradation products (FDP) increased up to 40 micrograms/ml. And the leukemic cells were over 30% in PB at the end of July, 1987. The diagnosis of APL with DIC was made. To our knowledge, this is the first case of APL with a history of MDS with t(15; 17).

[Hypereosinophilic syndrome associated with extensive asynergy of the left ventricular wall: a case report].

This is a report of a case of the hypereosinophilic syndrome associated with extensive asynergy of the left ventricular wall. This 41-year-old man's hospital admission in March, 1982 was for congestive heart failure, with marked eosinophilia (34%). Slight depression of the ST segments and flatness of the T waves were observed in electrocardiographic leads I, II, III, a VF, and V 5-6. Digitalis and diuretics were ineffective, but supplementary prednisone therapy resulted in a decrease in eosinophilia and improvement of congestive heart failure. He was transferred to our hospital for further examination in May, 1982. There were no signs of congestive heart failure, but a third heart sound was detected on admission. Marked ST segment depression and inverted T waves were observed in the left precordial leads. Echocardiography cardiography showed extensive asynergy of the left ventricular wall, and progressive enlargement of the left atrium and left ventricle. High density area near the apex of the left ventricle suggested the presence of mural thrombi. A left endocardial biopsy showed organized thrombi sparsely invaded by eosinophils. There were no signs nor symptoms of peripheral emboli. Tapering of the dose of prednisone was attempted twice, but each time eosinophilia resulted. The patient is now free of symptoms, and is being maintained on 25 mg of prednisone daily.
