The size of selected human skull foramina in relation to skull capacity.

An anatomical study was undertaken in order to investigate whether the sizes of selected human skull foramina with significant venous compartments correlated significantly with skull capacity. A total of 100 macerated human skulls were examined to determine the diameter of the foramina and the skull capacity. Measurements of the surface area of the foramina were made using a computerised digital analysis system. Only the size of the hypoglossal canal and jugular foramen were found to correlate significantly with the capacity of the skull. This correlation, together with the considerable size of the hypoglossal canal, indicated its important role in the venous drainage of the brain. There was considerable centralisation of venous outflow from the brain, with 60% of the area of all venous foramina of the skull occupied by jugular foramina. Asymmetry between the right and left jugular foramina was identified, with an average ratio of 1.6 (ranging between 1 and 3.47). In the case of right-sided domination the correlation between the skull capacity and the size of both jugular foramina was negative (the larger the skull cavity, the less the asymmetry), while in the case of left-sided domination the correlation was positive. Perhaps the left-sided domination is less advantageous for the haemodynamics of blood outflow, as the left brachiocephalic vein is longer and is often compressed by the sternum and aortic arch.

Correlations between risk factors for hearing impairment and TEOAE screening test outcome in neonates at risk for hearing loss.

The aim of the study was to find the correlation between specific risk factors for hearing impairment as well as between risk factors and TEOAE screening results in neonates at risk for hearing impairment. Seventy-one newborns at risk for hearing impairment have been included in the study. Strong correlations between specific risk factors were found. Investigation of the relationship between specific risk factors and TEOAE signal-to-noise ratio (SNR) showed that in children with genetic risk factors, TORCH (toxoplasmosis, rubella, CMV, herpes virus, others) infections and in those treated with ototoxic drugs, the values of SNRs were significantly lower than in children at risk who did not present those factors. In case of hyperbilirubinaemic children the values of SNR were significantly higher than in the controls. In our opinion these relationships may be partially explained by the positive predictive values of these risk factors, but other intrinsic factors may also be involved.

Results of total deafness treatment in young pre- and postlingually deafened children.

Developing a cochlear implant program is one of the most difficult problems we need to solve concerning young children. Application of cochlear implants to this group of patients gives hope for better speech and language development than using conventional hearing aids. Children comprise approximately 40% of all implanted patients at the Institute of Physiology and Pathology of Hearing in Warsaw. They are provided with different types of cochlear implants, 16 of which are included in the international comparative study EARS (Evaluation of Auditory Responses to Speech). In this paper results are presented of auditory speech perception in pre-, peri-and postlingual children using the multichannel cochlear implant systems COMBI 40 and 40+. All the children were regularly evaluated following the EARS procedure with a number of perception tests. Monitoring of auditory perception development is fundamental and allows prognosis of speech and language development in children.

Project of the countrywide data collecting system for neonatal hearing screening programme in Poland.

Neonatal hearing screening is becoming a standard of care in increasing number of hospitals and outpatient departments in Poland. A project of central data collecting system applicable to a neonatal hearing screening programme has been elaborated as a preparation to introducing a countrywide screening programme. The data collecting system will be based on the currently existing central system for the registration of neonatal screening tests for metabolic diseases. Data on risk factors for hearing loss and hearing screening test results will be collected. A central data collecting system for a neonatal hearing screening programme will increase the efficiency of the screening programme and facilitate epidemiological studies.

Correlation between Ki-67 index and some clinical aspects of acoustic neuromas (vestibular schwannomas).

Evaluation of the proliferation activity of neuromas has a practical meaning when there are doubts about the complete resection of the tumor. Evaluation of the clinical aspects connected with increased proliferation activity may have a much broader application. The aim of this study was to correlate selected clinical and radiologic aspects of vestibular schwannomas with the results of the Ki-67 index. The studied group included 23 males and 20 females. Unilateral neuromas were stated in 38 cases (mean age, 52.2 years) and bilateral tumors in 5 cases (mean age, 44.2 years). The immunohistochemical tests (Ki-67) were performed on the specimens preserved in formalin and stored in paraffin. The Ki-67 index was estimated in a semiquantitative study. The mean value of Ki-67 index was 1.86%. In case of unilateral neuromas (n = 38), the average Ki-67 index was 1.74%. In 5 cases of bilateral tumors, the index amounted to 2.79% (P = 0.278). No significant correlation was found by comparing the value of the Ki-67 index with the age of patients (P = 0.410: r = 0.128). Significant differences in the value of the Ki-67 index were noted in the sub-groups of tumors that were evaluated radiologically as growing and stable. The mean value of Ki-67 index was 3.17% in the first subgroup; in stable neuromas, it was significantly lower, amounting to 1.11% (P = 0.020). Such results may confirm that the growth rate of vestibular schwannomas varies and may explain the difficulties in estimating the growth of neuromas on the basis of clinical aspects only.

Organization of tinnitus management in Poland.

Spontaneous idiopathic tinnitus is a significant interdisciplinary therapeutic problem. Based on different programs of tinnitus treatment, we organized a team of physicians, psychologists and engineers in order to establish the needs for the first Tinnitus Clinic in Poland. At the same time, together with number of clinical centres, scientific societies and non-governmental organizations, we carried out training and an information campaign throughout the country and initiated the first epidemiological studies survey about tinnitus in Poland. Over a period of 2 years we have provided care for almost 1000 patients, including them in a IS 24-month therapeutic program at the clinic. As a method of choice, Tinnitus Retraining Therapy (TRT) based on a neurophysiological model of tinnitus origin is used. We present here epidemiological data on tinnitus and hyperacusis in Poland.

First auditory brainstem implantation in Poland: auditory perception results over 12 months.

Auditory brainstem implants (ABIs) are a modern method of treatment of total bilateral deafness in cases of extracochlear origin. In most cases therapy is applied in patients with neurofibromatosis type 2 (NF2). This paper presents the results of surgical treatment and rehabilitation in a 28-year-old woman with bilateral, multiple tumours of the central nervous system causing total deafness. Simultaneous removal of the tumours and implantation of ABI allowed treatment of the potentially lethal pathology and hearing restoration. Improving auditory skills and excellent tests results were noted in the year following implantation.

[Profound bilateral sensorineural hearing loss: a result of viral infections].

A patient is presented with profound bilateral sensorineural hearing loss, as a result of viral infections. The extensive clinical diagnosis was carried out. The findings of multiple audiological and immunological tests helped to fix the proper treatment and the period of its application. In consequence, the hearing in the right ear showed a significant improvement, but the left ear wasn't recovering. The inflammatory process occurred much earlier in the left ear and caused the indivertible hearing loss.

[Cochleostomy during the intracochlear implantation. Anatomical conditions in children and adults].

Cochleostomy is an opening of the perilymphatic spaces of the cochlea. It is the most important moment in intracochlear implant surgery. There are contradictory opinions in the available literature regarding this surgical intervention. Cochleostomy is performed through the round window, the oval window, the promontory or through the apical turn of the cochlea. A microanatomical study was performed to evaluate which kind of cochleostomy would be most effective and safe for the main structures of the temporal bone. The distances between round window and round window niche structures, the jugular bulb, internal acoustic meatus, posterior semicircular canal, internal carotid artery and cochlear canalicule were measured. It was stated that all the dimensions were smaller in children of up to four years of age and the differences were statistically significant. Cochleostomy through the promontory, especially using the "soft" technique of endosteum opening is safe for all the structures, and in children it also protects the cochlear canalicule.

[The external drainage in chronic exudative otitis media: distant results].

The usage of external drainage in the treatment of chronic exudative otitis media raised as much hope as it did doubts. However this method was not put to common use immediately. The doubts observed by laryngological mele delayed the usage of this method of treatment in many cases. In the Otolaryngological Clinic of the Surgery Institute in Warsaw, the external drainage in chronic exudative otitis media has been in use for several years. The authors of this paper present their own distant observations upon treated patients. They evaluate the efficiency of hearing examination and the lack of recurrence of inflammation of the middle ear.

[Carcinoma of the postcricoid area of the hypopharynx: treatment and results].

Squamous cell carcinoma arising in the postcricoid area of the hypopharynx is relatively late diagnose. Previous observation concerning patients treated in our Clinic and the data from literature confirmed very bad diagnosis in this cases. The results of the Clinical observation of 58 patients and pathomorphology findings in 35 preparates have been described. In that group we have found only 16 cases (27.6%) with 3 years long surviving rate. Morphological findings, relating lymphatic system, partially explained so bed prognosis.
