The growth of acoustic neuromas in volumetric radiologic assessment.

OBJECTIVE: The volumetric assessments of neuroma were applied for radiologic observation of tumor growth. The hypothesis that most of neuromas are stable or show only slight growth was tested. STUDY DESIGN: This was an observational study. SETTING: The study was performed in the university centers. PATIENTS: The study group included 27 patients with 15 unilateral tumors and 12 bilateral tumors. All patients had at least 2 magnetic resonance imaging (MRI) examinations, and the average interval between initial and control examinations was 11.4 months. MAIN OUTCOME MEASURE: Volume measurements were performed on T1- weighted MRI spin echo sequences after injection of gadolinium using special software. Growth of the tumors was estimated by comparison of the results of three measurements from the initial and control MRI examinations. RESULTS: The growth was confirmed in 17 of 27 tumors (63%). Growth was found in 10 of 12 neuromas of neurofibromatosis type 2 (83.7%). In 15 unilateral neuromas, growth was found in 7 (43%). Unilateral neuromas were observed for a shorter period of time (6.3 months) than bilateral tumors (14.7 months). The correlation between a neuroma volume gain and the follow-up period was statistically significant (p = 0.003, r = 0.544). CONCLUSIONS: The growth of tumors can be confirmed despite a short follow-up period.

Mastoid emissary in man. An anatomical-clinical study.

The purpose of the study was establishing differences in the situation of the external orifice of the emissary, its size and angle at which it leaves the skull. It is necessary for optimizaing of position for the internal coil of the speech processor of cochlear implant. As yet, the variability of external foramen of this emissary have not been sufficiently studied, especially in the clinical aspect with respect to the use of cochlear implants. The authors examined 200 skulls of adults of both sexes. The statistically significant differences were observed between sexes and between sides of the body, and they showed a definite regularity. The variability of the site of the emissary is presented in a form useful for clinical purposes.

Evaluation of performance with the COMBI40 cochlear implant in adults: a multicentric clinical study.

The present multicentric clinical study involves 19 centres, 16 of them in German-speaking countries, 1 British, 1 Polish and 1 Hungarian. 60 postlingually deafened adults with a mean age of 47.5 years (20-70) and mean duration of deafness of 5.3 years (0.5-20) have been evaluated with the MED-EL COMBI 40 cochlear implant which implements a high-rate continuous-interleaved-sampling strategy with 8 channels. Safety and effectiveness data have been collected. Speech perception tests include a 16-consonant, an 8-vowel, a sentence and a monosyllabic-word test in all languages and a 2-digit figure test in all languages but English. Test intervals are 1, 3, 6 months and 1 year after first fitting. 41 of the 60 postlingually deafened adult study patients have completed their 6-month evaluation. While their pre-operative monosyllabic-word score was 0%, their mean monosyllabic-word score 6 months after first fitting was 48% (8-90) with a median of 50%. The mean sentence understanding was 84% (24-100) with a median of 90%. The respective values for the 1-year evaluations with 25 patients are a mean of 50% (5-85), with a median of 60% for the monosyllables and a mean of 89% (30-100), with a median of 97%, for the sentences.
