Use of the nylon wool column method to separate lymphocytes results in preparations which, while fully viable, have grossly depleted cellular levels of phospholipid, cholesterol and sialic acid. These membrane effects, which relate mainly to T cells, indicate that the method is unsuitable for use in biochemical analysis of lymphocytes.

1. Rat and guinea-pig kidney cortex slices were made Na-rich by leaching in cold NaCl and then allowed to recover in Ringer solution at 30 or 25 degrees C. Net Na extrusion was systematically decreased by the use of re-immersion media with constant Na and diminishing K, or with constant K and increasing Na, and the energy requirements for Na extrusion under these conditions were calculated.2. The critical energy barrier at which no net Na loss can be achieved was somewhat lower in these tissues than found by other authors for frog sartorius muscle.3. Inclusion of pyruvate or insulin and lactate in the recovery fluid had no effect.4. Methylsulphate apparently enters renal cells during incubation and is therefore unsuitable for use with this tissue as a source of impermeant anion.
We prospectively studied 44 patients treated with the AO Unreamed Tibial Nail (UTN) at two orthopaedic trauma units. There were 15 open fractures: eight type I, four type II, one type IIIA, and two IIIB (Gustilo). The mean follow-up time was 13 months (range 6-24 months). There were 14 type A, 26 type B and four type C fractures (AO classification, Muller). All patients with the open fractures had surgery within eight hours of injury. Closed fractures were treated on the next available trauma list. The mean time to fracture union was 15.3 weeks (range 10-26). In the closed fracture group (n = 29), the mean time to union was 14.9 weeks compared to 18.6 weeks for the open fracture group. There were no non-unions and no infections. One patient had a malunion and breakage of locking bolts occurred in two cases. Our results indicate that the UTN provides excellent results in the treatment of both closed and open fractures of the tibial shaft, with union times comparable to reamed techniques.
We report the case of a 42-year-old woman with Cushing's disease and Nelson's syndrome. When she was 17 years old, transsphenoidal surgery was performed. A detailed morphologic study demonstrated nodular hyperplasia of corticotroph cells but no adenoma. Following a long-lasting remission (14 years), Cushing's disease recurred. After an unsuccessful second transsphenoidal surgery, Cushing's disease persisted and both adrenals were removed (at the age of 34). Subsequently the patient developed Nelson's syndrome. The pituitary tumor proved to be a corticotroph adenoma; it was removed by the transsphenoidal approach (at the age of 42). Although in most patients Cushing's disease is due to an ACTH-secreting pituitary corticotroph adenoma which precedes the manifestation of Nelson's syndrome, our case indicates not only that corticotroph hyperplasia may cause Cushing's disease but that it may exist before the development of Nelson's syndrome after the removal of both adrenals. Our study supports the view that protracted stimulation of corticotrophs resulting from the elimination of the negative inhibitory feedback effect by corticosteroids plays a role in adenoma initiation.
