[The occurrence of Bacillus cereus in dairy products (authors transl)].

During one year 1390 samples of pasteurized milk products from five different dairies were examined for content of B. cereus. The examination was done on the day of packaging and after incubation at 17 degrees C for 24 hrs. as well. On examination on the day of packaging 8.1 % were positive. No difference could be demonstrated between low-temperature pasteurized and high-temperature pasteurized products. However, differences were found from one plant to another (2.1 %--14.5 %). After incubation B. cereus were isolated from 17.2 % of low-temperature pasteurized samples and from 22.8 % of high-temperature pasteurized products. The occurrence of B. cereus varied according to season. In July-September the level of contamination was 40 %. It is concluded that during summer months bacteriological examination of dairy products should be supplemented with an examination for B. cereus. This could provide a valuable tool in checking the efficiency of dairy cleaning.

Testicular femininization with intraperitoneal disseminated mesothelial cysts.

A case report is given of a patient suffering from testicular femininization, a 13.5 l mesothelial-lined cyst associated with the right gonad, and several small intraperitoneal mesothelial cysts. No malignant changes were found, and no remnats of the Mullerian duct system were detected.

Serum alpha-fetoprotein as a marker for the effect of post-operative radiation therapy and/or chemotherapy in eight cases of ovarian endodermal sinus tumour.

The clinical pathological findings of eight cases of ovarian endodermal sinus tumour (yolk sac tumour) are presented. Histological exmination in all eight cases showed a typical endodermal sinus tumour pattern, and in six of the patients other tumour elements such as dysgerminoma, choriocarcinoma, malignant teratoma, endometriosis, and a dermoid cyst were also found. Six patients had increased serum alpha-fetoprotein concentration in the post-operative period, and two patients had a normal concentration 27 and 35 days after operation, respectively. In all cases except one, a close correlation between serum alpha-fetoprotein and progression or regression of tumour was found. Serum alphafetoprotein was thus found to be a reliable parameter in post-operative radiation and/or chemotherapy (VAMBLE). In one patient who died 10 months after operation with widespread endodermal sinus tumour growth, only a small terminal increase in serum alpha-fetoprotein concentration was found. Four of the eight women are still alive with normal alpha-fetoprotein concentration, and without clinical evidence of tumour disease.

Accidental kidney biopsies in psoriasis.

Eight accidental kidney biopsies in psoriatics were obtained together with 429 liver biopsies. None of the biopsies showed signs of renal damage. All patients had severe psoriasis, six had been treated with methotrexate for 1-2-6 years, receiving total dosages from 660 to 1757 g.

Epidermoid carcinoma of the larynx. VI. Histologic grading in the clinical evaluation.

A multifactorial microscopic grading of malignancy was performed on a clinically well examined series of 129 patients with laryngeal carcinoma. The microscopic score was statistically significantly correlated to the frequency or regional lymph-node metastases and to the death rate. It afforded important supplementary information for the T-classification and for the prognosis. Microscopic grading is an essential supplement to the clinical evaluation of risk groups.

Detection of IgG aggregates or immune complexes using solid-phase C1q and protein A-rich Staphylococcus aureus as an indicator system.

A radioimmunoassay for detection of C1q-binding IgG aggregates and antigen-IgG antibody complexes is described. The assay makes use of solid-phase C1q and 32p-labelled protein A-rich Staphylococcus aureus as an indicator system. Both 19S and heavier IgG aggregates that fixed C1q were detected. The sensitivity of the assay permitted detection of heavy (19-25S) IgG aggregates at a concentration of 8 mug/ml or less. The results indicated that detection of IgG in this assay is dependent on the degree of IgG polymerization and the molar ratio between the solid-phase C1q and the IgG polymers. Albumin-anti-albumin complexes, preformed at equilibrium with antibody to antigen molar ratios of 2:1 to 3:1 and at antigen concentrations of 25 to 40 mug/ml, were also detectable using the described radioimmunoassay.

Epidermoid carcinoma of the lip. Histologic grading in the clinical evaluation.

A retrospective microscopic evaluation of primary biopsies was carried out in a material of 438 patients with carcinoma of the lip. A multifactorial grading system consisting of 8 parameters, each sub-divided into 4 grades, was used. The degree of malignancy showed a statistically significant correlation to the frequency of metastases, frequency of recurrence or residual tumour, mortality and T classification. This grading system may allow a separation of risk groups in which special treatment and follow-up should be applied.

Epidermoid carcinoma of the tongue. Histologic grading in the clinical evaluation.

In a series of 49 patients with epidermoic carcinoma of the tongue microscopy of the primary biopsy was performed, using a multifactorial grading system consisting of 8 parameters, each of 4 grades. The score value (degree of malignancy) revealed a statistically significant correlation with the frequency of regional metastases. As this was particularly marked in the T1 and T2 groups, it seems to be an important supplement to the clinical evaluation of risk groups.

Metastasizing basal cell carcinoma. Case report.

Three cases of metastasizing basal cell carcinoma are presented. Metastases to regional lymph nodes occurred in 2 cases, and to both the humerus and the brain in the third. In all 3 patients, local recurrence developed in spite of apparently radical surgical removal of the primary tumour, and histological examination revealed distinct aggressive growth. The presence of metastases provides further evidence of the existence of an aggressive variety of the basal cell carcinoma. In these cases it is of particular importance that initial radical treatment is performed, and that the follow-up of the patients includes a check of the regional lymph nodes.

Incidence of antibiotic resistance and transmissible R factors in the gram-negative bowel flora of hospital patients on admission.

Stool specimens obtained on admission of 95 patients to a male urological ward were examined quantitatively for antibiotic-resistant gram-negative aerobic bacteria. All patients excreted resistant strains of some kind; 67% resistant Escherichia coli strains. Organisms resistant to sulphonamides, tetracyclines, and streptomycin occurred at a frequency of 10(-3)-10(-4). Ampicillin resistance was encountered most frequently, i.e. greater than 10(-2), whereas organisms resistant to chloramphenicol were found less often (10(-6)). Transmissible R factors were demonstrated in 53/121 (44%) resistant E. coli strains. A connection between tetracycline resistance and ability to transfer resistance was demonstrated. Resistance to chloramphenicol did not occur as a single resistance trait but was transferred from all strains resistant to this antibiotic.
