The pH changes of four different calcium hydroxide mixtures used for intracanal medication.

The purpose of this study was to determine the pH values of calcium hydroxide mixtures in combination with either distilled water, two different local anaesthetic solutions or physiological saline solution, which are commonly used as a dressing materials during endodontic treatment. The pH of each vehicle prior to mix with calcium hydroxide and subsequently each prepared combination was determined using a digital pH meter. The pH measurements were determined at 3 min, 10 min, 1 h, 24 h, 48 h and 7 days. The results of this study indicated that four different water-based vehicles demonstrated similar pH changes range within pH 11-12 when mixed with calcium hydroxide.

Pulmonary embolism due to cardiac hydatid disease: imaging findings of unusual complication of hydatid cyst.

We report two cases of primary cardiac hydatid cyst in which hydatid materials caused recurrent embolizations in pulmonary arteries and pulmonary parenchyma. Cardiac hydatid cysts may stay asymptomatic for a long time, until they reveal themselves being perforated into cardiac chambers and/or pulmonary artery or systemic circulation. The role of imaging techniques in diagnosis is discussed and the importance of dynamic enhanced CT, MR imaging, and enhanced MR angiography (MRA) is reported. Imaging findings were confirmed by surgery and pathology. Early diagnosis is essential because delayed treatment increases the morbidity and mortality rates.

Supravalvular aortic stenosis without Williams syndrome.

In this article, the case of a 30-years-old female patient with supravalvular aortic stenosis is reported. Her clinical picture was otherwise completely different from Williams syndrome. Since supravalvular aortic stenosis is rarely seen, its forms are discussed and it is concluded that supravalvular aortic stenosis can be present without Williams syndrome.

Empyema thoracis--a series of 230 cases.

A retrospective analysis of 230 cases of empyema thoracis which occurred in our clinic during the last 14 years have been presented. The causes, the pathogen organism, treatment with antibiotic are enumerated. The surgical treatment carried in resistant cases have been described Ten patients (4.34 percent) died due to respiratory failure and septic shock. Importance of early and appropriate treatment has been stressed.

Early decortication for postpneumonic empyema in children. Effect on pulmonary perfusion.

Early pulmonary decortication was performed on 66 of 137 children with postpneumonic empyema, while 71 received conventional treatment. The mean age of the 66 patients with decortication was 5.5 years (range 6 months-14 years). The empyema was left-sided in 34 and right-sided in 32. Decortication was performed when lung expansion was not obtained after 10-12 days of intercostal tube drainage, antibiotic therapy (guided by sensitivity tests of pleural fluid) and pleural irrigation. Scintigraphy showed loss of pulmonary perfusion on the side of empyema to be 65% +/- SD 20 (25-98)% before decortication in the 23 tested patients. In ten of them the test was repeated after surgery and showed significant (p < 0.001) diminution of the perfusion defect, from 57 +/- 6.8 (25-84)% to 4 +/- 2.6 (0-8)%. The hospital stay was significantly (p < 0.001) shorter for the surgically treated than for the classically managed patients, viz. 19.5 +/- 4 (13-36) days vs 73.6 +/- 14 (34-110) days. Early decortication thus had beneficial effects on pulmonary perfusion and hospital stay.

Lung hydatid cyst rupture and its surgery.

Hydatid cysts are common in Turkey. Authors have seen 248 cases of lung hydatid cysts; Fifty hydatid cyst of lung had ruptured. Their symptoms and detailed findings are reported. Rupture caused asphyxia in 3, haemoptysis in 20, lung abcess in 16, rupture in pleural cavity in 4 and anaphylactic reaction in one case.

[A case of bronchial leiomyoma].

Pulmonary leiomyomas are the least common benign tumors of the lung. These tumors originate from smooth muscles of the bronchial walls, peripheral small airways, or from perivascular smooth muscles. It is reported that the diagnosis of the tumor has been established bronchoscopically and proved with thoracotomy. The characteristics of the tumor and treatment modalities are discussed.

Dangerous pencils and a new technique for removal of foreign bodies.

Aspirated foreign bodies are important problems during childhood. Some instances may be fatal. Most of the foreign bodies are removed with use of classic instruments like rigid bronchoscopes and foreign body forceps. But sometimes we fail to remove them, particularly aspirated beads and spherical objects. In this case, a Fogarty catheter is helpful. Although we have had many experiences recently we failed to remove one aspirated foreign body which was a pencil cap. We succeeded in removing this pencil cap with a new technique that is explained in this article. We used a Storz transbronchial aspiration biopsy needle and a cotton-carrier stylet to remove the pencil cap.

Peripheral vascular injuries in children.

We report 94 arterial injuries in 91 children treated at the Department of Thoracic and Cardiovascular Surgery, Medical School of Dicle University, between 1978 and 1988. The average age was 10.3 (range, 3 to 14) years. Stab wounds were seen in 37 patients (40%). Gunshot wounds were seen in 21 patients (23%), and two cases were caused by iatrogenesis. Vein damage was present in 39 patients (44.4%), and nerve loss was observed in 20 patients (21.2%). Twenty-one cases (23.3%) were associated with fractures. Diagnosis of arterial injury was made by clinical findings. Arteriography was used in 15 patients (16.5%) as the diagnostic method. The surgical interventions performed for arterial repair in our series were end-to-end anastomosis in 61 (64.5%), interposition of autogenous reversed saphenous vein grafts in 22 (23.4%), lateral suture in 8 (8.3%), and ligation in 3 (3.9%) patients. Four patients (5.3%) underwent limb amputation.

The hydatid cyst of the lung in children and results of surgical treatment.

Between 1976 and 1986 one hundred and ten children with pulmonary hydatid disease were treated surgically. Sixty five of the patients were males and 45 were females. We diagnosed hydatid cyst in 36 cases submitted in our clinic with hemoptysis. Thirty of the patients had cough, 16 had chest pain and dyspnea, 12 had purulent sputum and 8 cases had fever and anaphylactic phenomena. In most of the patients hydatid cysts were localised in the right lung. However, they had affinity for the lower lobes of both lungs, rather than the upper lobes. The radiological examination was found to be the most reliable diagnostic method. In the majority of our cases we performed thoracotomy + cystotomy + capitonnage, while in some cases we performed resection, like cystectomy, wedge resection and lobectomy. We did not encounter any serious operative and postoperative complications, except for one pleural empyema and a single case of mortality.

Injuries of the peripheral arteries and their surgical treatment.

196 cases of peripheral arterial injury presented to our clinic between 1976 and 1986 are summarised. Most of the cases were between 11 and 30 years of age and most of the injuries were by fire-arms and sharp weapons. The most common localisations were a. femoralis (29% cases), a. poplitea and branches (28%), a. brachialis injury (20%). The most common surgical measures were end-to-end anastomosis (43% cases) and venous orthograft replacement (17%). There were 5 mortalities. Factors affecting treatment success are discussed.

[Closed injury of the pericardium].

The closed rupture of the pericardium without accompanying intrathoracal injuries, is a rare lesion. These cases are diagnosed generally at autopsy. Authors describe a case, successfully treated by operation.

[In vitro investigation of Gutta-percha amounts in different root obturation techniques].

Several obturation techniques and obturation materials are developed in order to fill the root canals completely. It was necessary to fill the root canals with solid and semisolid materials due to the unproduction of an ideal root canal sealer. The well known material that is being used, for this purpose today is gutta-percha. Single cone techniques, lateral condensation techniques and thermoplastic gutta-percha enjection techniques are used with sealers and without sealers in single rooted 30 teeth. And than, the amount of gutta-percha were evaluated by mean of herizontally taken section from 3 different levels of root canals. Finally the gutta-percha enjection techniques found to contein more gutta-percha amount in all three section levels than the single cone techniques and laterally condancation technique.

Surgery in hydatid cyst of the lung. A report of 460 cases.

A series of 460 surgically treated cases of hydatid cyst of the lung is presented. Radiologic preoperative diagnosis was made in 90% of the patients. Cystotomy with subsequent capitonnage was performed in 395 cases, wedge resection in 27, lobectomy in 15 and segmental resection in 23 cases. After careful hemostasis, two drains were placed in the thorax and the thoracic cavity was closed. There were three postoperative deaths (0.6%) and two recurrences of cyst (0.4%) during follow-up investigation for periods up to 10 years.

Methods of treatment of childhood empyema in a Turkish university hospital.

In the last ten years, 120 patients were admitted to our clinic with empyema. Empyema and effusion have been investigated for pathogenesis. Microbiologic studies of pleural aspiration fluid showed that Staphylococcus aureus was the most common pathogen, found in 44 patients (36.7 percent). Other pathogens found were streptococci in 23 (19.2 percent) and Pneumococcus in 18 (15 percent) in sequence. No production occurred in cultures of 19 (15.8 percent) patients. In 16 (13.3 percent), Pseudomonas and Enterobacteria such as E coli, Klebsiella and Proteus occurred. Surgical intervention has been carried out depending on clinical conditions. In the 24 (20 percent) patients, pleural aspiration and lavage, decortication in 23 (19.2 percent), pulmonary resection (segmentectomy, lobectomy) in seven (5.8 percent), and only drainage and thoracic lavage in 66 (52.5 percent) have been carried out. Three patients (2.5 percent) died due to respiratory failure and septic shock, and the others have been discharged with healing.
