[Comparative diffusion of fusidic acid, oxacillin, and pristinamycin in dermal interstitial fluid after repeated oral administration].

OBJECTIVE: The aim of this study was to use the suction bullae technique to compare skin diffusion of 3 antibiotics commonly used for skin infections (fusidic acid, oxacillin, pristinamycin) and to estimate their potential activity at the site of skin infections. SUBJECTS AND METHODS: This comparative open study was conducted in 12 healthy volunteers using a repeated latin square experimental scheme. Antibiotic concentrations in serum and suction bullae fluid were measured by high performance liquid chromatography after 5.5 days of repeated oral administration of fusidic acid (1 g/d), oxacillin (2 g/d), and pristinamycin (2 g/d). RESULTS: Mean antibiotic concentrations in serum and interstitial fluid (suction bullae fluid) were highest for fusidic acid with a Cmax at 91.3 +/- 23.0 mg/l and 45.5 +/- 18.0 mg/l respectively (interstitial fluid/serum ratio=49 +/- 10 p. 100). For oxacillin, Cmax was 8.3 +/- 3.6 mg/l and 0.98 +/- 0.49 mg/l (ratio 13 +/- 5 p. 100). Pristinamycin concentrations were low with a Cmax at 0.51 +/- 0.40 and 0.26 +/- 0.15 mg/l (ratio 73 +/- 57 p. 100). Comparing the area under the interstitial fluid and the serum concentration-time curves showed that the best diffusion was obtained with pristinamycin (114 +/- 61 p. 100), followed by fusidic acid (57 +/- 13 p. 100) and oxacillin (48 +/- 25 p. 100). DISCUSSION: These data were used to calculate indicators of potential efficacy in the interstitial dermal fluid: inhibitor quotient (Cmax/MIC) and AUIC (ASC/MIC), indicator of the time antibiotic concentrations are maintained above the minimal inhibitor concentration (MIC). This showed that fusidic acid was potentially more active against all staphylococci. For streptococci, the observed interstitial concentrations of pristinamycin and of fusidic acid should theoretically inhibit streptococci A growth, but oxacillin was the most adapted antibiotic.

Anatomical differences in the psoas muscles in young black and white men.

The anatomy of the psoas major muscle (PMA) in young black and white men was studied during routine autopsies. The forensic autopsies included 44 fresh male cadavers (21 black, 23 white) with an age span of 14 to 25 y. The range for weight was 66-76 kg and for height 169-182 cm. The PMA was initially measured in its entire length before measuring the diameter and circumference at each segmental level (L1-S1). At each segmental level, the calculated anatomical cross-sectional area (ACSA) was more than 3 times greater in the black group compared with the white (P < 0.001). The psoas minor muscle (PMI) was absent in 91% of the black subjects, but only in 13% of the white subjects. These data show that the PMA is markedly larger in black than white subjects. The marked race specific difference in the size of the PMA may have implications for hip flexor strength, spine function and race specific incidence in low back pathology, and warrants further investigation.

Differences in the iliolumbar ligament and the transverse process of the L5 vertebra in young white and black people.

The anatomy of the iliolumbar ligament (ILL) and the spatial orientation of the transverse process of the L5 vertebra were studied in 62 young black (n = 29) and white (n = 33) men and women during routine autopsy. The aim of the study was to determine possible racial differences in the structure and attachments of the iliolumbar ligament. The present study also investigated the spatial orientation of the transverse process of the L5 vertebra since the ILL has been reported to attach to the transverse process. The measurements of the iliolumbar ligament were carried out with a digital vernier caliper while the transverse process angles were measured with an adjustable protractor. The ligament in black people was made up of a single, markedly longer band compared to white people, where the ligament was made up of two shorter bands. The ILL measured 61.8 +/- 1.3 mm in black and 33.2 +/- 1.5 mm white men, and in black women 61.3 +/- 0.9 mm versus 32.2 +/- 1.2 mm in white women (p <0.01). Further, the ILL was markedly wider in black than white subjects (p <0.01). The horizontal and vertical angle also varied greatly between black and white subjects (p <0.01). These are previously unrecognized observations. Albeit unsubstantiated, these findings may have implications for understanding the etiology of various low back stress problems.

Levels of fusidic acid in skin blister fluid and serum after repeated administration of two dosages (250 and 500 mg).

The fusidic acid steady-state concentrations in serum (S) and skin suction blister fluid (SBF) after oral doses of 250 and 500 mg administered twice daily for 6 days, as film-coated tablets of sodium fusidate, were studied in eight healthy subjects. The mean peak serum concentrations after the 250- and 500- mg regimens were 39 +/- 5 and 102 +/- 11 mg/l, respectively, obtained between 2 and 3 h after drug intake. The corresponding values for SBF, obtained later, between 2 and 12 h after drug intake, were 21 +/- 5 and 79 +/- 11 mg/l. As measured by the ratio of area under the concentration vs. time curve (SBF/S), fusidic acid penetration is 69-75%, whatever the dose. With either regime, the fusidic acid SBF and serum levels exceed the minimal inhibitory concentration of bacteria usually found in skin infections, especially Staphylococcus species. A dose of 250 mg twice a day appears sufficient to treat these infections and could be tested in clinical studies.

Pharmacokinetics of sodium fusidate after single and repeated infusions and oral administration of a new formulation.

The pharmacokinetics of sodium fusidate were studied in eight healthy volunteers (five males and three females) aged 21 to 33 years (29.1 +/- 1.5), weight 46 to 79 kg (61.6 +/- 4.0 kg). First, the subjects were given 500 mg of sodium fusidate by infusion over two hours; secondly, one month later, the volunteers were given 500 mg of fusidate by infusion every eight hours for three days; thirdly, two 250 mg tablets of a new film coated formulation were administered as a single dose. Plasma concentrations of fusidate were measured by HPLC. Peak plasma concentrations reached at the end of the first and the last infusions were 52 +/- 5 mg/l and 123 +/- 12 mg/l respectively. The following mean pharmacokinetic parameters were obtained after single intravenous administration: elimination half-life 10 +/- 1 h, total clearance 22 +/- 2 ml/min and volume of distribution 0.30 +/- 0.04 l/kg. After repeated administration the half-life and the volume of distribution remained unchanged whereas total clearance was halved (11 +/- 1 ml/min). This leads to an experimental accumulation ratio (3.6 +/- 0.2) higher than the theoretical one (1.8 +/- 0.1; P less than 0.01). Consequently, mean trough and peak steady state plasma concentrations (81 +/- 9 and 123 +/- 12 mg/l respectively) are higher than those expected from the single dose kinetics (33 +/- 4 and 76 +/- 7 mg/l respectively). This dose regimen leads to concentrations well above the MIC for most sensitive strains.(ABSTRACT TRUNCATED AT 250 WORDS)

Perennial rhinitis in Saudi Arabia. A prospective study.

One hundred consecutive patients with perennial rhinitis were examined clinically and by means of a test battery of laboratory investigations in order to clarify the pathogenesis of the disease, and the clinical value of various tests in the assessment of the patients. In 54% of the cases allergy was proven. The most common allergen was Bermuda grass which is found widely distributed in the Eastern Province. In 46% of the cases, allergy could not be demonstrated. Twelve percent had eosinophilic rhinitis. In this study, investigations of blood eosinophils and IgE were found to be of value in the assessment of patients with perennial rhinitis. The possible connection between pollen allergy and perennial rhinitis in Saudi Arabia is discussed.

Longterm results of ultrasonic irradiation in Meniere's disease.

Thirty-seven patients with Ménière's disease were treated with ultrasonic irradiation to the lateral and superior semicircular canals. In a longterm follow-up study, lasting alleviation of the vertiginous attacks was found in 24 out of 33 patients (72%). In most of the cases, hearing remained unchanged. It is suggested that the therapeutic effect is associated with a change in the endolymphatic drainage dur to histological alterations in the labyrinth produced by ultrasound.

ABO blood groups in unipolar and bipolar manic-depressive patients.

The authors determined ABO blood groupings for 66 manic-depressive patients diagnosed and divided into bipolar and unipolar groups according to strict symptomatic and course criteria. A significantly higher percentage of bipolar patients than unipolar patients had blood group O, while a significantly higher percentage of unipolar patients than bipolar patients had blood group A. THese findings provide support for the validity of the unipolar-bipolar distinction and are consistent with the concept that vulnerability to manic-depressive disorders may be related to membrane disturbances.
