Preparation of the renal function and imaging agent 99mTc-MAG3 starting from S-unprotected mercaptoacetyltriglycine.

A new approach that obviates inconvenient heating of the labelling solution when preparing the renal function and imaging agent 99mTc-MAG3 is described. The labelling procedure involves reduction of 99mTc-generator eluate with stannous chloride in alkaline solution in the presence of S-unprotected mercaptoacetyltriglycine (MAG3) and the co-ligand sodium tartrate, followed by neutralization with a phosphate buffer solution. After optimization of the kit preparation the radiochemical purity of the radiopharmaceutical amounts to greater than 98%. During the labelling process intermediate Tc-species occur.

[Main branch stenosis following aortic valve replacement].

Three patients developed left main stem stenosis within some months after aortic valve replacement. In all of them diagnosis was confirmed by angiography and bypass surgery was performed successfully. Left main stem stenosis is a rare complication of aortic valve replacement and is due to cannulation and perfusion of the coronary arteries. The mechanism is probably injury of the vessel wall due to the perfusion-catheter, followed by intimal hyperplasia. A similar mechanism is assumed for restenosis after transluminal coronary angioplasty.

Chemical and biological characterization of different Tc complexes of cysteine and cysteine derivatives.

The labeling of cysteine and its derivatives (penicillamine, N-acetylcysteine, cysteine ethyl ester) with 99Tc (99mTc) was studied as a model for the radiopharmaceutical preparation of Tc-99m mercaptide complexes. After the use of TcO4-, TcOCl52-, and TcBr62- as labeling agents for the Tc oxidation states VII, V, and IV, respectively, complexes of Tc(V) and Tc(IV) were prepared and characterized. The biologic behavior of these complexes was studied in rats. The substitutions in cysteine are responsible for substantial changes of net charge and lipophilicity in the Tc complexes, and the consequences on the excretion patterns in Wistar rats are discussed.

The results of a preliminary study of the cancer-Multistep-Therapy (C.M.T.) (so called "The Greifswald-Modell") are reported. This Greifswald-Therapy-Model has been developed by hitherto existing experiences in the clinical test of the C.M.T. 4 patients with carcinoma uteri T2bNOMO to T2bNOMO level epithelium-carcinomas histologically verified showed after a preoperative treatment with C.M.T. and cantactradiatio (60CO) under the employment of a reduced dose of radios (1/2 to 1/3 of the pretherapy used in this clinic or 1/4 to 1/5 of the tumor dose) a complete regression of the tumor-specific changes. In the surgical specimen won after 4-7 weeks after C.M.T., carcinoma could not be detected either macroscopically or microscopically. Patients with a good therapy -- compatibility show a liberty of relapse with an increase in weight and an excellent physical and psychical health two year after C.M.T. This preliminary study shows the capability of the well - chosen therapy-model for the clinical test of the therapy-effect of the C.M.T. and it urgently demands to carry on with adequate alternate rows by including other conventional therapy schemes and other kinds of tumor to estimate definitively the effect of the C.M.T. in the tumor therapy.
