Reliability of histo-pathological diagnosis of squamous epithelial changes of the uterine cervix.

The reliability of histological diagnosis of squamous epithelial changes was tested by letting 13 pathologists read 1,001 consecutive cervical biopsies twice. Intra-observer and inter-observer agreement, variance, and deviation of diagnosis were determined. The diagnostic ability showed great individual variation and no significant correlation to experience in pathology. The diagnosis of invasive cancer had a high diagnostic specificity, and the diagnostic sensitivity of the diagnosis of no significant epithelial changes was high too. The reliability of the diagnosis of dysplasia and carcinoma in situ proved unsatisfactory.

Immunoproliferative alterations in lymphoid tissues and liver in mice stimulated with Corynebacterium parvum.

Morphological alterations in the spleen, liver, lymph nodes, thymus, bone marrow and blood were studied in DBA and Balb/C mice treated with Corynebacterium parvum. The spleen lymph nodes and liver increased in size and weight, whereas thymus showed decrease in weight compared to the control group. A marked, early proliferation of monocytes in the bone marrow preceded infiltrations by macrophages in lymph nodes, spleen and liver. These infiltrations were closely related to the vascular structures, became nodular and decreased around day 12 after the injection. A rapid increase in the number of monocytes and transformed lymphocytes of the peripheral blood were noted almost immediately after the administration of the vaccine. No relationship between the degree and duration of the histological changes, strain of mice, and number of inoculations was found. It is suggested that the increase in size and weight of the lymphoid organs and the liver is primarily caused by invasion of macrophages, perhaps secondary to a preceding proliferation of monocytes in the bone marrow.

Surface topography of the healthy and diseased transitional cell epithelium of the human urinary bladder.

51 biopsies from healthy and diseased epithelium from human urinary bladders were examined by scanning electron microscopy. The luminal surface of the cover cells was marked by microvilli and microplicae. The replacement of the cover cells takes place by simple desquamination. This exposes cover cells having numerous microvilli, and these later develop characteristic microplicae. The ability to form cover cells is lost by the transitional epithelium following tumour formation. No tumour-specific surface morphology was observed.

Focal nodular hyperplasia of the liver. Possible influence of female reproductive steroids on the histological picture.

In a series of 18 patients with focal nodular hyperplasia of the liver characteristic histological changes were found in the lesion of 5 patients receiving female reproductive steroids, e.g, four women taking oral contraceptives for several years, and one man treated with synthetic estrogenic compound for carcinoma of the prostate. These lesions contained young, connective tissue septa with abundant proliferation of bile ducts, piecemeal necroses and lymphocytic infiltration. Much less or no activity, was found in the lesion of 11 women and 2 men without hormone treatment. Possible mechanisms for this activity are discussed, and it is proposed that focal nodular hyperplasia represents a congenital malformation, possibly a hamartoma, the liver cells of which may suffer from enzymatic defects, which may render them especially vulnerable to female sex hormones.

The extent of carcinoma in situ in urinary bladders with primary carcinomas.

The occurrence of carcinoma in situ was examined in a consequtive series of cystectomy specimens from 43 patients. All patients were suffering from or had been suffering from primary bladder cancer. Carcinoma in situ was defined as a definitive polymorphia of enlarged nuclei with abnormal chromatin structure in non-tumour bearing areas. It was found in 26 bladders. By means of a systematic technique of cutting the specimens, the extent of the alterations was quantitied, relating the number of blocks containing the alteration to the total number of blocks in which the changes possibly might be present. The extent of carcinoma in situ ranged from 2 to 81% with an average of 20%. The distribution of the alterations was unpredictable, often strongly focal, most often in continuation of the tumour. In one specimen, the margins of resection were involved. The extent was largest in bladders with poorly differentiated tumours. A temporal relationship between in situ carcinoma and invasive carcinoma could not be shown, as regards tumour size and duration of clinical symptoms. The morphology of the changes is discussed seen in the light of variations in the normal epithelium lining the lower urinary tract. Minor degrees of atypia of the epithelium showed rather bad reproduction. The dominating occurrence of carcinoma in situ in bladders with poorly differentiated tumours may either be a manifestation of a biological difference between tumours of different grades of differentiation or that the morphological criteria used correspond to the in situ type of poorly differentiated tumours.

Types of metaplasia in forty urothelial bladder carcinomas. A systematic histological investigation.

In a systematic study of bladders from consecutively cystectomized patients, 40 primary urinary bladder carcinomas were examined with regard to the type of differentiation. Metaplastic areas were found in 24 of the tumours. A positive reaction of keratin to Kreybergs stain was required for the definition squamous metaplasia. Squamous areas were found in 16 of the tumours. In 4 tumours there was, in addition to squamos metaplasia, glandular metaplasia which was defined as definite glandular tissue with dysplasia of the epithelium. In 4 tumours, glandular metaplasia was also present in addition to the urothelial carcinoma. In 18 cases, metaplastic changes were present in the luminal portion of the tumour from which the biopsy material and desquaminated material originated. The occurrence of metaplasia was related to the degree of differentiation of the urothelial part of the tumour. Metaplastic changes were most frequent in the poorly differentiated tumours. The biopsies obtained pre-operatively permitted of the diagnosis metaplasia being made in 15 cases, while the cytological material suggested metaplastic changes in 4 cases only. The biological function of metaplasia is unknow. The possibility that these areas will react differently to radiation and chemotherapy is present and the frequency of such changes would suggest that their importance should be studied by their registration as mixed forms as stated by WHO.

Effects of cyclosporin A, gentamicin and furosemide on rat renal function: a lithium clearance study.

1. This study applied clearance methods of inulin, lithium, potassium, sodium and para-aminohippuric acid (PAH) for investigation of the effects of cyclosporin A (CyA), furosemide and gentamicin on rat (n = 92) renal function. The drugs were dosed for 2 weeks; CyA 12.5 mg/kg per day, gentamicin 32 mg/kg per day and furosemide 5 mg/kg per day. 2. The questions asked were: could these methods differentiate the effects of drugs with different sites of action, and would gentamicin or furosemide exaggerate the nephrotoxicity of CyA? 3. Furosemide increased sodium clearance (CNa) 74% and fractional sodium clearance (FENa) 105%, while fractional sodium reabsorption in the distal nephron (FDNR) was reduced, compared with placebo-treated controls. 4. Gentamicin reduced CPAH 29% and Cin 37%, while FENa increased 335%. Proximal fractional reabsorption (PFR) and absolute proximal reabsorption (APR) decreased. 5. CyA depressed CPAH 32% and lithium clearance (CLi) 56%, and increased PFR. 6. The effects of CyA and furosemide in reducing renal function were not additive. 7. CyA plus gentamicin reduced CPAH to 35% of the value in untreated controls, equal to 52% of the CPAH of CyA-treated rats; Cin was reduced to 46% of the Cin of CyA-treated rats. 8. Rats given CyA, furosemide and gentamicin had decreased Cin, CPAH and CLi compared with rats given either CyA plus furosemide or gentamicin plus furosemide. 9. Thus, in this investigation of drugs known to have different sites of actions, the differences in renal and tubular function were discernible with the lithium clearance method. 10. The nephrotoxicities of CyA and of gentamicin were additive, while furosemide did not aggravate CyA nephrotoxicity.
