HLA and rheumatoid arthritis. A study of five families.

Five families with two or more members ill with rheumatoid arthritis were investigated clinically. Tissue typing for HLA A, B, C and D/DR antigens was performed. We could not define may HLA haplotype associated with disease within these families. However, the prevalence of the haplotypes containing HLA-DR4 was high, suggesting a direct relationship between DR4 and increased risk of contracting rheumatoid arthritis.

Leiomyomatosis peritonealis disseminata.

Leiomyomatosis peritonealis disseminata (LPD) is a rare disease characterized by the development of multiple subperitoneal leiomyomas, which are widespread in the peritoneal cavity. Of the previously reported LPD cases, nine patients were pregnant, two were on oral contraceptives and one had a granulosa-cell tumor. The thirteenth documented case is presented here (this patient had been on oral contraceptives for 13 years). Thus, hormonal factors might be involved in the pathogenesis of LPD. Controversy prevails as regards the cellular origin of LPD. The disorder has a grossly malignant appearance, but has a benign histology and a favorable prognosis; so a correct diagnosis is imperative. In almost all the previously reported LPD patients, hysterectomy and bilateral salpingo-oophorectomy have been performed, but salpingo-oophorectomy is probably unnecessary, at least in premenopausal patients.

Vaginal smear history in patients with invasive cervical carcinoma.

In 23 (about 43%) patients cytologically screended within 4 years before the diagnosis invasive cervical cancer 11 had a benign smear; when reviewed, two were borderline cases and three were misinterpreted. Twelve patients had different degrees of squamous cell dysplasia. In three of these the cancer was detected during the follow up procedures. Four did not respond to calls for follow up. Five were checked without any active measure but sampling new smears. Atypical smears must be clinically evaluated and treated in order to detect invasive carcinomas. The therapeutic results, however, may be controlled by repeat smears.

Appendicitis followed by reactive arthritis in an HLA B27-positive man after infection with Yersinia enterocolitica, diagnosed by serotype specific antibodies and antibodies to Yersinia outer membrane proteins.

A case of appendicitis followed by reactive arthritis in an HLA B27-positive, 29-year-old man after infection with Yersinia enterocolitica is reported. Infection with Y. enterocolitica was diagnosed by determination of serotype specific antibodies and antibodies to Yersinia outer membrane proteins. Bacteriological cultures from the appendix were not made. Although reactive arthritis is a well-known complication of Yersinia-associated enteric disease, there are only few reports of patients with Y. enterocolitica pseudo-appendicitis complicated by arthritis during follow-up.
