Plate osteosynthesis in posttraumatic deformities of the femoral shaft.

The success of precisely indicated surgical treatment of posttraumatic deformities of the femur shaft depends on painstaking preoperative planning with exact assessment of the locus of correction, careful determination of the correction angle, and choice of an adequate operation method. In correction operations plate osteosynthesis is necessary, in contrast to the various possibilities for care of primary fractures of the femur. Therefore we concentrate in this paper mainly on the use of plate osteosynthesis, without forgetting that there are also indications for intramedullary nailing, especially of not yet consolidated fractures or aseptic wound healing impairments. Long X-rays of the leg are an essential part of the preoperative preparation. The success of all operative treatments does not depend only on the degree of disablement, the age of the patient, and realistic target. Particularly in view of the difficulty of plate osteosynthesis and the variety of methods of primary osteosynthesis, the surgeon has to consider the rules of biomechanics as they apply to the incorrectly set femur and to simulate the surgical treatment with preoperative planning in order to attain physiological axial conditions with the best operative method. This applies particularly to the knee joint. Axial deformities are very often combined with shortening, and surgical correction is therefore sometimes difficult.

[Catheterization of the umbilical vein, diagnostic and therapeutic relevance (author's transl)].

Catheterization of the umbilical vein does allow direct access to the portal system, thus yielding information about the pathophysiology of the hepatic and splanchnic vessel system. Mesurement of the portal vein pressure in conditions of portal hypertension can be performed in the portal vein. A special, three-compartment ballon catheter is available for mesurement of intrahepatic wedge pressure. Angiography of the umbilical vein allows earlier diagnosis of metastases within the liver than other diagnostic procedures. Intraportal application of cytostatic drugs using a long term indwelling umbilical vein catheter is feasible as well. Own experiences are as yet rather limited. Three patients have been treated up to now this way; evaluation of the therapeutic results is not yet possible, because of the short duration of therapy and because of the low number of patients. The technique of umbilical vein catheterization does not seem to present any problems. Complications are rare and did not occur in our series.

Implantation technique and complications of totally implanted arterial access systems for long-term therapy in patients with occlusive vascular disease.

Twenty-six totally implantable catheter systems for intra-arterial infusions were used in patients with peripheral arterial occlusive disease. A total of 606 short-term infusions were administered via the system. The patients were observed up to 22 months. There was an over-all rate of complications in 15.3%. The complications described were: one catheter occlusion, one dislocation, one case of local infection and one case of arterial thrombosis. The latter was successfully treated with local lysis therapy. The system is distinguished from conventional methods of arterial access by its long-term use and the low incidence of complications. The system was extremely well accepted by the patients.
