[Congenital bicuspid aortic valve with regurgitation--a rare case showing a fibrous band between the conjoined cusp and the ascending aorta].

It is quite unusual that the congenital bicuspid aortic valve shows aortic valve regurgitation except postinflammatory event. In a case of aortic regurgitation with bicuspid valve, a fibrous band was recognized between the conjoined cusp and the ascending aorta at the operation. Since a degenerative change alone was discerned histologically in the resected specimen, this abnormal band was supposed to be an embryogenic remnant of the aortic valve. Six cases were reported previously and all of them were male, and five were Japanese.

[A case of stage IVb ovarian carcinoma successfully treated with intra-arterial infusion chemotherapy using implantable reservoir system].

A patient with liver metastasis from ovarian cancer was treated by intra-arterial infusion chemotherapy (well differentiated adenocarcinoma--CDDP 50 mg/body, ADR 30 mg/body, CPA 500 mg/body (iv)). After the chemotherapy, the metastatic tumor appeared remarkably smaller and could be resected successfully at the second reduction surgery. This patient is active 30 months after the first appearance and enjoying a favorable quality of life.

[A case of aberrant left pulmonary basal aneurysm arising from the descending aorta].

A 65-year-old man was referred to our hospital because of fever and abnormal shadow in the left lung. An aortogram revealed a large artery arising from the descending aorta supplying the left basal segment and, which then flowed into the pulmonary vein. This artery had an aneurysm and 30 mm in diameter. Bronchography showed compression of the left basal branch. A clinical diagnosis of an aberrant left pulmonary basal aneurysm arising from descending aorta was made, and left lower lobectomy was performed, because of the danger apprehension of the aneurysmal rupture. The procedure was successful. There are only 15 case reports of a systemic artery supplying the lung with normal bronchial branch in Japan. Differences of this anomaly from pulmonary sequestration were discussed.

Antibiotic production by the marine photosynthetic bacterium Chromatium purpuratum NKPB 031704: localization of activity to the chromatophores.

Over 200 strains of marine purple photosynthetic bacteria were isolated. Two strains showed antibiotic activity towards Saccharomyces cerevisiae and were tentatively identified as Chromatium purpuratum. Crude antibiotic, prepared by solvent extraction, showed a broad antimicrobial spectrum. The highest activity was found in the chromatophore fraction. Chromatographic separation of purified light harvesting complex from one strain, NKPB 031704, showed the presence of two separate pigmented compounds which were responsible for antimicrobial activity. Our findings reveal the unexpected ability of photosynthetic bacteria to produce broad spectrum antibiotics. In addition, this is the first example of intracellular localization of antibiotic activity in a marine bacterium.

Induction of metallothionein and stimulation of calcification by dexamethasone in cultured clonal osteogenic cells, MC3T3-E1.

To clarify the effects of dexamethasone (Dex) on metallothionein (MT) synthesis and calcification in osteoblastic cells, a clonal osteogenic cell line (MC3T3-E1) was used in the present study. Under culture conditions designed not to cause calcification, MT synthesis of cells at 3 days after inoculation increased with increasing concentration of Dex (2.5-50 nM) for a 24-h culture period. Cells at 6 or 9 days after inoculation also synthesized MT by a 24-h exposure to Dex. These show that undifferentiated osteoblasts have the ability to synthesize MT by Dex. Under culture conditions designed to cause calcification, cells at 6 days after inoculation were cultured with 50 nM Dex in the presence of 7 mM beta-glycerophosphate (beta-GP) for 7 days. Ca content of Dex-treated cells was about 7.5 times as high as that of untreated cells. Dex-treated cells showed a high activity of alkaline phosphatase (ALP). The Zn content of the MT fraction in Dex-treated cells was about 8 times as high as that of untreated cells. These results show that Dex has the ability to induce MT synthesis in osteoblastic cells and to cause a high calcification which is due at least partly to an enhanced activity of ALP.

[Clinical study of centrifugal pump bypass as an auxiliary technique for surgical treatment of descending thoracic aorta].

Left heart bypass or arterial bypass using a centrifugal pump (Bio-pump bypass) with a H-PSD shunt tube was performed as an auxiliary technique for the treatment of descending thoracic aorta in 13 cases. Hemodynamic differences during aortic clamp were compared between cases using Bio-pump bypass and 4 cases of axillo-femoral temporary bypass which were carried out in the first term. Furthermore, in Bio-pump bypass cases, the bypass route was investigated from the point of view of operative complications in relation to bypass technique. Peripheral blood pressure and urinary output during aortic clamp were significantly increased in the Bio-pump bypass group. Those results indicated that the Bio-pump bypass was useful as an auxiliary technique especially for high risk patients with low renal function. In particular, left heart bypass was a beneficial technique which was not affected by modality or region of disorder in the descending thoracic aorta. However, this technique should be selected carefully for patients with cardiac disease, for instance severe left ventricular hypertrophy, because two cases of operative complications consisting of pericardial effusion related to the bypass technique were experienced in this study.

[Influence of ventriculoatrial conduction on hemodynamic consequences in patients with artificial pacing].

To assess the hemodynamic consequences associated with ventriculoatrial (VA) conduction following pacemaker implantation, 22 subjects with the sick sinus syndrome were studied using two-dimensional echocardiography (2DE) and Doppler techniques, including pulsed Doppler (PD). Pacemakers used were unipolar, programmable pulse generators which can operate in the DDD and VVI modes. A simultaneous strip chart recording of 2DE, phonocardiogram and ECG was obtained in each mode. Systolic time intervals (STI) and left ventricular diameters were measured using 2DE. Left ventricular ejection time (LVET) was determined from the aortic valve echo, which was the interval between the opening and closure of the cusp. Left ventricular pre-ejection period (LVEPEP) was the interval between the Q wave of the ECG and the opening of the aortic valve. Right ventricular (RV) STI were also measured using the pulmonary valve echo in a manner similar to that used with the aortic valve echo. Stroke volume and ejection fraction were calculated by conventional methodology. RVET was longer than LVET; RVPEP was shorter than LVPEP. These two STIs were shortened using an incremental pacing rate in each mode. Similarly, the stroke volume and ejection fraction decreased, depending on the pacing rate at a range of 50-110 b/min. RVET and LVET in DDD were longer than those in VVI. RVPEP and LVPEP in DDD were shorter than those in VVI. Stroke volume and ejection fraction in DDD were smaller than those in VVI. There was a significant difference (p less than 0.05) between DDD and VVI. These differences became significantly (p less than 0.01) greater between DDD without VA conduction and VVI with VA conduction.(ABSTRACT TRUNCATED AT 250 WORDS)

[A fundamental and clinical investigation of cancer antigen 130 (CA 130) in the field of obstetrics and gynecology].

Two murine monoclonal antibodies designated 130-22 and 145-9 have been recently established by immunizing mice with a pulmonary carcinoma cell line (PC-9). With use of these monoclonal antibodies a sensitive sandwich immunoradiometric assay (IRMA) for cancer antigen 130 (CA130) was developed in Daiichi Radioisotope Laboratories (Tokyo). Applying this IRMA kit CA130 concentrations were measured in various body fluids with special reference to obstetrics and gynecology. The results are as follows; 1) A CA130-IRMA showed excellent sensitivity, specificity, reproducibility and analytical recovery. The standard dose-response curve covered the range from 10 to 500 U/ml. 2) Serum CA130 levels measured by this assay system were closely correlated with serum CA125 levels, demonstrating quite a high correlation coefficient (r = 0.965). 3) In pregnancy maternal CA130 levels increased moderately (less than 300 U/ml) in the first trimester, and thereafter fell rapidly under normal upper limit (less than 35 U/ml). Immediately after deliveries maternal CA130 levels showed a rapid increase, reaching 269 U/ml (mean levels). In amniotic fluids CA130 concentrations were greatly elevated (3,236 U/ml mean levels), while the levels were almost within normal limit in the umbilical arterial and venous blood. Accordingly it is necessary to measure Schwangerschaftsprotein 1 or human chorionic gonadotropin simultaneously with CA130 to rule out possible pregnancy. 4) CA130 was clearly localized in the amniotic epithelium and umbilical sheath and other placental tissue components remained negative for this antigen. Taking the lower CA130 levels in the umbilical sera into consideration, these immunohistochemical results suggest CA130 is not oncofetal but oncoplacental. 5) Serum CA130 levels increased pretherapeutically beyond 35 U/ml in 87% of ovarian malignancies, and in 56% of endometrial carcinoma. The mean levels of serum CA130 reached 931 and 143 U/ml, respectively. These data indicate clinical usefulness of CA130 as a tumor marker for those diseases. By contrast serum CA130 levels were lower, and very few cases showed serum CA130 levels over 35 U/ml in cervical cancer. 6) In endometriosis 88% of the cases demonstrated increased serum CA130 levels, indicating its usefulness for monitoring therapeutic courses, just like CA125. In benign gynecologic diseases over 80% of cases showed increased serum CA130 levels while only slight increase in serum CA130 was found (mean levels less than 84.0 U/ml). This disadvantage could be lessened by a combination assay with CA72-4 and others whose serum levels were very low in the benign diseases.

Osteocompatibility of platinum-plated titanium assessed in vitro.

Suppression of dissolution is important to increase the biocompatibility of titanium implants. Therefore, the possibility of application of platinum-coated titanium as a biomaterial was explored in in vitro experiments using the MC3T3-E1 osteogenic cell line. The data obtained from long-term cultures indicated that pure platinum or titanium thickly coated with platinum inhibited calcification significantly, suggesting that the platinum ion fails to improve the osteocompatibility of titanium implants.

Development of a new system for evaluating the biocompatibility of implant materials using an osteogenic cell line (MC3T3-E1).

A new culture system was developed to clarify the biocompatibility of implant materials with bone tissue using the MC3T3-E1 osteogenic cell line. The cells were inoculated onto specimens such as aluminium oxide, titanium, dental casting silver-palladium alloy (PD), and a plastic coverslip. To study the effects of these materials on cell growth, differentiation, and calcification, DNA and protein content, alkaline phosphatase activity, and calcium content, respectively, were determined. The results from biochemical analysis suggest titanium and aluminum oxide to have adequate biocompatibility, while PD has an irritant effect on cell metabolism. It is clear that an objective view of the differentiation and calcification processes of osteogenic cells can be understood through such analysis. From the results of this study, our culture system appears suitable for evaluating the biocompatibility of implant materials with bone tissue.

Inhibitory effects of cadmium on in vitro calcification of a clonal osteogenic cell, MC3T3-E1.

To examine an inhibitory mechanism of Cd on bone formation, the effects of Cd on calcification were investigated in a culture of a clonal osteogenic cell line, MC3T3-E1. At 3 days after inoculation, Cd was added to the medium containing 7 mM beta-glycerophosphate, and culture was continued for 8 days. Cd at 1.78 microM and above caused a significant decrease in 45Ca accumulation. The decrease in mineralization by Cd was similar to that in collagen content or alkaline phosphatase (ALP) activity. Histologically, the cell density and the mineralization degree were lower than those of the controls. Ultrastructurally, degenerated cells were observed with undifferentiated cells which had fewer rough-surfaced endoplasmic reticulum and many mitochondria. This suggests that Cd may inhibit the differentiation into osteoblasts as well as the cell function. On the other hand, calcification of cells at 8 days after inoculation was inhibited by Cd at 1.78 microM and above. The decrease in collagen content and ALP activity by Cd was much lower than that in calcification. Cd-treated cells were well differentiated into osteoblasts morphologically, but the mineralization degree was lower than that of the controls. Ultrastructurally, cell damage was not recognized so strongly compared with long-term Cd treatment. The mineralization of osteoblasts was also inhibited by Zn levels which left both collagen content and ALP activity unaffected. From these results, it was suggested that the inhibitory effect of Cd on in vitro calcification of MC3T3-E1 cells may be due to both a depression of cell-mediated calcification and a decrease in physiochemical mineral deposition.

Response of osteoblastic clonal cell line (MC3T3-E1) to [Asu]eel calcitonin at a specific cell density or differentiation stage.

Clone MC3T3-E1 cells isolated from newborn mouse calvaria is an osteogenic cell line which retains an ability to differentiate into osteoblastic cell in vitro. The effect of [Asu]eel calcitonin (ECT) on clonal MC3T3-E1 cells was investigated at different stages of differentiation. ECT caused an increase in alkaline phosphatase (ALP) activity. The stimulative effect was demonstrated to be dependent upon cell density or differentiation stage. At a cell density of 1.18 X 10(5)/cm2 cells were incubated with ECT for 2 days. The treatment by ECT caused an increase in ALP activity. A specific response to ECT dependent on the cell density was observed in a narrow range of cell density. Moreover this range of cell density responsible to ECT was found to be a rapid differentiation stage of MC3T3-E1 cells. These results suggest that calcitonin stimulates differentiation of osteoblast. In addition to these results, cellular adenosine-3',5'-cyclic monophosphate (cAMP) level was raised by ECT treatment at a cell density of about 1.4 X 10(5) cell/cm2 and this response was also specific for cell density. At cell density lower or higher than this density no stimulative effect by ECT was observed. On the other hand, N6,O2-dibutyryl adenosine-3',5'-cyclic monophosphate (db-cAMP) and theophylline caused an increase in ALP activity in wide cell density range. These results indicate that an increase in ALP activity by ECT is mediated by intracellular cAMP and that the specific response to ECT dependent on the cell density is regulated in the process of cAMP formation and/or in the preceding process of cAMP formation.

Augmentation of killer activity of murine spleen cells against allogeneic and syngeneic tumor cells by inoculation of alloantigen in vivo.

After C57BL/6 (B6) mice were inoculated with BALB/c spleen cells via tail vein, kinetics of cytotoxic activities in the B6 mice against sensitizing alloantigens (H-2d) and against syngeneic antigens were investigated using, as target cells, P815 mastocytoma cells (H-2d) and B16 melanoma cells (H-2b). Cytotoxic activity against P815 in the B6 spleen cells reached a peak 3 days after alloantigen inoculation, decreased drastically on day 5 and rose again thereafter. The profile of anti-B16 cytotoxic activity was similar to that of anti-P815 activity. The cytotoxic activity against P815 was inhibited partially by cold B16, but that against B16 was not inhibited by cold P815. Surface phenotype of cytotoxic cells against P815 was Lyt2+, Thy1+, Asialo GM1+ and that of cytotoxic cells against B16 was Lyt2-, Thy1+/-, and Asialo GM1+. The results indicate that inoculation of B6 mice with allogeneic BALB/c spleen cells induce two types of cytotoxic cells; one is similar to lymphokine-activated killer (LAK) cells and the other is activated natural killer cells.

MC3T3-G2/PA6 preadipocytes support in vitro proliferation of hemopoietic stem cells through a mechanism different from that of interleukin 3.

Both MC3T3-G2/PA6 preadipocytes and interleukin 3 (IL 3) can support in vitro proliferation of mouse hemopoietic stem cells (CFU-S). We examined whether MC3T3-G2/PA6 cells produce IL 3 and whether a common mechanism might underlie the action of both of these agents. We used cultured mast cells, DA-1 cells, and FDC-P2 cells as the targets of IL 3 and conditioned medium (CM) of WEHI-3 cells as a source of IL 3. MC3T3-G2/PA6 CM did not support the growth of the above cells. IL 3 mRNA was not detected in the preadipocytes. Since CM obtained from the cocultures of bone marrow cells and MC3T3-G2/PA6 cells did not have a significant effect on the growth of the IL 3-dependent cells, none of the bone marrow cells seem to produce IL 3 under the influence of the preadipocytes. When the factor-dependent cells were cocultured with MC3T3-G2/PA6 cells, the former did not survive, whereas mast cells and DA-1 cells intimately associated with the preadipocytes. Even when bone marrow cells, mast cells, and MC3T3-G2/PA6 cells were cocultured, the number of CFU-S increased, but not that of mast cells. These results seem to exclude the possibility of the action of IL 3 in the microenvironment provided by MC3T3-G2/PA6 preadipocytes.

The role of hepatic triglyceride lipase in the metabolism of intermediate-density lipoprotein--postheparin lipolytic activities determined by a sensitive, nonradioisotopic method in hyperlipidemic patients and normals.

With a view to elucidating the physiologic role of hepatic triglyceride lipase (H-TGL), we studied the relationship between the activity of H-TGL and the concentrations of the lipids of ultracentrifugally separated lipoprotein fractions in sera from 81 cases of primary hyperlipidemia, 5 of hypothyroidism, and 31 normal subjects. The activity of H-TGL in postheparin plasma was determined by the sensitive, nonradioisotopic method that was recently developed by us. In the entire group of subjects including the normals, the activity of H-TGL had a significant inverse correlation with the concentration of the cholesterol (r = -0.443, P less than 0.001) and phospholipid (r = -0.433, P less than 0.001) of intermediate density lipoprotein (IDL). When the patients were divided into subgroups according to the phenotype of hyperlipidemia, it was found that the correlation was more significant in type IIb (r = 0.695, P less than 0.001) or type IV + V (r = -0.664, P less than 0.0001). In the five cases of hypothyroidism, the mean IDL cholesterol level was high (28.3 +/- 12.3 mg/dL) and the H-TGL activity was very low (4.6 +/- 4.5 mumol/h/mL). The H-TGL activity was also significantly correlated with the ratio of high density lipoprotein-2 to high density lipoprotein-3 cholesterol (r = 0.351, P less than 0.001) in the entire group.(ABSTRACT TRUNCATED AT 250 WORDS)
