Synthesis and antiallergy activity of [1,3,4]thiadiazolo[3,2-a]-1,2,3-triazolo[4,5-d]pyrimidin-9 (3H)-one derivatives. I.

A series of 6-substituted [1,3,4]thiadiazolo[3,2-a]-1,2,3-triazolo[4,5-d]pyrimidin-9(3H)-one derivatives 4a--z were synthesized from 5-substituted 1,3,4-thiadiazol-2-amines 5 by the following consecutive reactions: pyrimidine ring closure with bis(2,4,6-trichlorophenyl) malonate, nitration, chlorination, amination, hydrogenation and diazotization. The structure of 4 was confirmed by an alternate synthesis of 4, involving reaction of 5-substituted 2-azido-1,3,4-thiadiazole 13 with ethyl cyanoacetate, followed by the Dimroth rearrangement and ring closure. The antiallergic activities (anti-passive peritoneal anaphylaxis, anti-passive cutaneous anaphylaxis and anti-slow reacting substance of anaphylaxis activities) of the products were evaluated.

Anaphylaxis

The effect of treatment with recombinant gamma-interferon on adjuvant-induced arthritis in rats.

We have investigated the effects of recombinant rat gamma-interferon (rIFN gamma) on adjuvant-induced arthritis (AA). Lewis rats, inoculated in the left hind-paw with adjuvant (day 0), were given 10(5) U/rat of rIFN gamma daily (days 0 to 20), subcutaneously and intramuscularly on alternate days. rIFN gamma suppressed the secondary phase of swelling of both hind-paw on and after day 18 without influencing the earlier phases, both primary and secondary, of swelling. rIFN gamma also reduced the hind-paw bone lesions, the degree of splenomegaly, and the increase in erythrocyte sedimentation rate and plasma fibrinogen. These results indicate a new aspect of the regulatory role of IFN gamma in chronic inflammation.

[Clinico-statistical study on pyometra in high-aged outpatients].

We clinico-statistically studied outpatients with pyometra aged 60 or older who visited our outpatient clinic during the past four years. 1. Pyometra was diagnosed in 48 cases (13.6%) of 353 in which the uterine cavity could be examined. The incidence became significantly higher with age (p less than 0.01). 2. Thirty-two patients with pyometra were found to require a wheelchair or to be in an immobilized state (bedridden). These patients with poor activity in daily living amounted to 76.2% of 42 cases which were not a complication of malignant uterine tumor or tuberculous endometritis. 3. Pyometra was diagnosed in 29 patients of 96 with urinary and/or rectal incontinence (30.2%). This incidence was significantly higher than that in our patients without incontinence (5.5%) (p less than 0.01). 4. Hematological and blood chemistry study revealed abnormality of the peripheral leucocyte count in 34.5% and CRP abnormality in 70.4% of all the cases of pyometra. 5. Mixed infection by both aerobic and anaerobic bacteria was observed in 16 cases of 36 in which strain could be determined by pus culture. Streptococcus sp. and Bacteroides sp. were predominant as aerobic and anaerobic bacteria, respectively. The present study on pyometra cases of aged people demonstrated that poor activity in daily living and incontinence were significant factors causing this disease. It also revealed the various features of pyometra in aged people.

The effect of treatment with interferon-gamma on type II collagen-induced arthritis.

We have investigated the effects of recombinant murine interferon-gamma (rIFN-gamma) on type II collagen-induced arthritis (CA) in DBA/l mice. Therapeutic as well as prophylactic treatment with subcutaneous rIFN-gamma, at 10(5) U/mouse six times a week, inhibited the development of CA without any obvious side effects. The accompanying suppression of anti-CII antibody responses may partly explain the inhibition of CA by rIFN-gamma. The possible role of the anti-inflammatory effect of systemic IFN-gamma in the inhibition of CA is discussed.

[Clinical applications of Brånemark system implant in the Nippon Dental University Hospital. A 5-year report].

A clinical review was performed on patients who requested implantation and visited the Nippon Dental University Hospital during the five years from April 1985 to June 1990. This report describes patient characteristics and treatment results. 1. The total number of new patients was 127 (68 males and 59 females). Their ages ranged from 20 to 80 years. 2. The number of patients who were diagnosed with an indication for implantation and completed the 1st stage operation was 39 (30.7%). An additional 23 patients (18.1%) entered preoperative treatment and are waiting for their operation. Overall, implantation was indicated in 48.8% of all new patients. 3. The ages of patients at fixture installation ranged from 20 to 79 years (mean 60.54, S.D. 11.21) among the 39 patients (20 males and 19 females) who completed the 1st stage operation. As for the number of jaws, implantation was indicated in 40 jaws (21 in male patients and 19 in females). Maxillary implants were indicated in 8 jaws and mandibular implants were indicated in 32 jaws (1 male patient had indications in both jaws). 4. The total number of implanted fixtures was 198, of which 39 were installed in the maxilla, and 159 in the mandible (1 female patient was 5 mandibular fixtures installed at the University of British Columbia, Canada). 5. Of the fixtures implanted in 40 jaws, 94.9% of maxillary fixtures and 98.1% of mandibular fixtures remained stable after the 1st stage operation.(ABSTRACT TRUNCATED AT 250 WORDS)

Antenatal diagnosis of congenital heart disease and fetal arrhythmia by ultrasound: prospective study.

Fetal echocardiographic and Doppler ultrasonographic prospective studies were performed in utero on 299 babies delivered at Hirata Municipal Hospital, Shimane, Japan, from May 1984 to June 1986. Two or three ultrasonographic examinations were performed on each fetus from 20 weeks of gestation to term. Three congenital heart anomalies and 12 fetal arrhythmias were diagnosed antenatally, but 3 heart anomalies (2 small ventricular septal defects and 1 moderate pulmonary stenosis) were not detected in utero. Routine echocardiographic screening appears to be a useful diagnostic tool to detect congenital heart anomalies and fetal arrhythmia antenatally.

Succenturiate placenta diagnosed by ultrasound.

The succenturiate placenta is a morphological abnormality, the antenatal recognition of which is important as vessels connecting the main placenta with the succenturiate placenta may rupture during labor and fetal death may ensue. In addition, retention of the placental material may lead to postpartum hemorrhage. We treated 5 patients with a succenturiate placenta and the antenatal ultrasonograms and related discussions are presented herein.

Fetal liver measurements by ultrasonography.

A total of 378 measurements of fetal liver size were made from 18 weeks gestation through term. Statistical analysis of the results showed a highly significant correlation between fetal liver measurements and gestational age, biparietal diameter, and fetal femur length (R = 0.93, R = 0.89, R = 0.89, respectively). Ultrasonic measurement of the fetal liver is a reliable indicator of fetal growth in the third trimester, as growth rates of the biparietal diameter and head circumference are blunt. Furthermore, consecutive measurements of fetal liver size enhance the detection of symmetrical, fetal growth, by acquisition of various ultrasonic parameters such as biparietal diameter, fetal femur length, fetal abdominal circumference and so forth.

Ultrasonographic measurement of the human fetal liver in utero.

A total of 232 measurements of the size of the human fetal liver were made from 19 weeks gestation through to term. The size of the liver increased linearly with the advance in pregnancy. Statistical analyses showed a highly significant correlation between liver circumference and area and gestational week or fetal abdominal circumference (r = 0.89, r = 0.90, r = 0.90, r = 0.85).

Ultrasonographic in utero identification and measurement of the normal fetal spleen.

Sixty-nine ultrasonographic examinations were performed to identify and measure the fetal spleen in utero on 49 women with regular menstrual cycles, at 18-41 weeks of gestation. The length (FS-L), width (FS-W), area (FS-A), and sectional area (length X width; FS-LW) of the fetal spleen correlated well with the gestational age. A very high correlation of FS-LW with FS-A was noted (r = 0.98; p less than 0.001). Ultrasonographic identification and measurement of the fetal spleen in utero is thus a useful indicator of fetal spleen growth and should facilitate detection of in utero splenomegaly, asplenia and other abnormalities.

Ultrasonic evaluation of trophoblastic disease and the response to chemotherapy.

Ultrasound is an established method of confirming the presence of a hydatidiform mole in utero. However, in agreement with other investigations, we have often noted that the appearance of this entity is less specific than originally reported. To elucidate this, the ultrasonic patterns of 26 patients with trophoblastic disease and 27 with other entities were reviewed. Since there is a paucity of literature with regard to the response of choriocarcinoma to chemotherapy, determined by ultrasound, we simultaneously studied the relationship between findings of the ultrasonograms and the levels of hCG-beta-subunit in sera. We noted variable ultrasonic features in trophoblastic disease, and the sonograms of the choriocarcinoma have occasionally been confused with those of hydatidiform mole. If special attention is directed to the thickness of the placenta-like echoes as well as the sonolucent areas within it, the diagnosis of partial hydatidiform mole may be feasible. We also noted a rough correlation between ultrasonic appearances and the hCG-beta-subunit value, determined during chemotherapy for choriocarcinoma.
