Five cases of von Recklinghausen's disease were studied with regard to their association with chest diseases, especially pulmonary emphysematous and fibrotic changes. CT analysis revealed that four patients out of five had multiple pulmonary emphysematous lesions and that those lesions were seen in all segments. In two cases out of the four, it was difficult to detect their abnormalities by an ordinary chest roentgenogram, suggesting the possibility of higher incidence of emphysematous changes in von Recklinghausen's disease than has been expected. On the other hand, pulmonary fibrosis was observed in two cases. There were two patients who had no fibrosis but emphysematous lesions. TBLB was performed in one case, in whom no fibrotic changes were observed. Usually pulmonary fibrosis is associated with emphysematous bullae in patients with von Recklinghausen's disease. However, fibrotic and emphysematous changes may not always occur in combination in von Recklinghausen's disease.
Periodontal therapy for the maxillary anterior area requires careful consideration as to the choice of therapy, as esthetic results are just as important as eradication of disease. The extensive shrinkage or loss of the interdental papilla, especially between the two central incisors as a result of periodontal therapy, should be minimized by the proper selection of therapy. Thorough subgingival scaling and root planing without surgical flaps is the treatment of choice as the maxillary anterior area is compatible for this type of therapy from the anatomic and access standpoints. In the posterior dentition, the surgical, conventional flap approach is encouraged after initial scaling, because of the anatomy of the roots (concavities, furcations) and difficulty with access makes thorough root therapy difficult. Fortunately, esthetics is not a major concern in the posterior areas. If surgical therapy is necessary for the maxillary anterior area, the papilla preservation flap technique better preserves the papilla for esthetic purposes but allows good access to the roots for root planing and, if necessary, the placement of graft material.
A 32-year-old man was admitted to our hospital suffering from cough and bloody sputum. After the preoperative examination, the most suspected diagnosis was either a cardiac myxoma or a pulmonary embolism. At operation, the cavity of main pulmonary trunk was occupied with a tumor growing through the stalk from the outflow tract of right ventricle. The invasive tumor was not completely removed from the pulmonary tract. Rhabdomyosarcoma originating from the right ventricle was confirmed histologically. The patient died on the 199th postoperative day. A necropsy was not conducted. Diagnosis, operative indication and chemotherapy of cardiac rhabdomyosarcoma are discussed in this paper.
Erythrocytes were fractionated, by centrifugation, on the basis of cell density, which is proportional to cell age. Glycated hemoglobin (Hb A1c) in the fractionated cells was measured and compared with the Hb A1c in whole blood and with the fructosamine concentrations in plasma of normal and diabetic subjects. In normal subjects, Hb A1c increased according to the age of the erythrocytes, but hyperbolically rather than linearly, indicating the slow conversion of Hb A to Hb A1c with saturability. The greatest correlation of Hb A1c with plasma fructosamine was in younger middle-aged cells. A subgroup of subjects with impaired glucose tolerance had high Hb A1c in older cells. Hb A1c of the young cells reflects recent glycemic status, for monitoring the initial stage of the treatment of diabetics, whereas Hb A1c of the older middle-aged cells more specifically reflects the longer term glycemic status, for screening and for the long-term monitoring index for treatment of diabetes.
A 36-year-old Japanese woman with mixed connective tissue disease (MCTD) was treated with systemic Vitamin E and developed sudden myocardial infarction. The diagnosis of MCTD was based on clinical and serological findings. Since she developed pericarditis with high fever on the occasion of the attack of myocardial infarction, she was treated with systemic corticosteroid and recovered well. Although MCTD had been recognized as a disease with good response to corticosteroid and good prognosis, some cases suffer from interstitial pneumonia, pulmonary hypertension and myocardial infarction with poor prognosis.
In order to test the anti-atherosclerotic function of elastase, 44 Japanese quails, 40 d of age, were used in this study. An atherogenic diet contained 15% corn oil and 2% cholesterol. Elaszym was orally administered at a dose of 6,000 EL units per kg body weight 3 times a week for 3 months. After 3 months feeding the atherogenic diet was discontinued. Moderate hypercholesterolemia and marked lipid-rich aortic lesions were noted in the group which was fed the atherogenic diet for 3 months. The thickened intima was composed of fibroblasts and alpha-1-anti-trypsin, S-100 protein, calmodulin and elastase were strongly demonstrated. Withdrawal of the atherogenic diet resulted in marked improvement of the serum cholesterol level, and slight reduction of the degree of the intimal thickening of the thoracic aorta. Elastase treatment after the withdrawal of atherogenic diet induced significant regression of the aortic lesions of the thoracic aorta. These results suggest that Elaszym possesses the promotive effect on regression of atherosclerotic lesions.
A 13-year-old boy with partial anomalous pulmonary venous connection (PAPVC) having an intact atrial septum (IAS) is reported. He had open heart surgery 3 years ago at another hospital, but the cardiac septum was intact and PDA was ligated. After his discharge, cardiomegaly and increased right pulmonary vasculature were seen in chest roentgenograms. Our cardiac catheterization data and pulmonary angiograms showed that he had a PAPVC with IAS. Surgical correction was accomplished by utilizing a polytetrafluoroethylene graft with external ring support and interposing it between the right upper pulmonary vein and the left atrium during thoracotomy without the need for extracorporeal circulation. The postoperative course was uneventful and he was discharged on the 14th postoperative day. At 3 months postoperatively, radionuclide studies demonstrated no evidence of pulmonary congestion or graft obstruction. To our knowledge, this is the first clinical report of prosthetic replacement of the pulmonary venous pathway with graft patency demonstrated postoperatively.

Solid and granular porous hydroxylapatite implants were compared in the surgical treatment of angular interproximal periodontal defects in 10 subjects. After completion of initial therapy, presurgical measurements of pocket depth, attachment level, gingival recession, gingival fluid and tooth mobility were recorded. Six months after the surgery the measurements were repeated. The use of both forms of porous hydroxylapatite resulted in reduction in pocket depth, and probeable attachment level gains as well as gingival recession and reduction of gingival fluid and tooth mobility. These changes were similar for both granular and solid forms of porous hydroxylapatite.
Marked non-communicating hydrocephalus may rarely cause ventricular rupture producing either a dilated cystic cavity (ventricular diverticulum) or communication between ventricle system and subarachnoid space (spontaneous ventriculostomy). Ventricular diverticulum has been believed to be collection of cerebrospinal fluid which escaped beneath the pia mater after rupture of ependymal layers and cerebral parenchyma. We proposed herein to report a case of subtentorial ventricular diverticulum which accompanied with choroid plexus papilloma of the lateral ventricle. A nine year-old girl admitted to our hospital complaining of clumsiness of hands and walking, disability of reading, headache and vomiting. The neurological examination revealed alexia, papilledema, anisocoria, righ hemianopsia, weakness of right upper limb, and cerebellar ataxia. CT brain scan showed a large high density area at the trigone of the left lateral ventricle with non-communicating hydrocephalus and an extra-axial low density area in the posterior fossa. The medial space of the left trigone was especially ballooned and is just shifted above incisura tentoria by the tumor. The intraventricular tumor was totally removed by operation and proved to be benign choroid plexus papilloma microscopically. The subtentorial mass was confirmed to be a cyst contiguous to the medial trigone of the lateral ventricle, namely ventricular diverticulum. Three special features were recognized in this case. The first, this was the first example of ventricular rupture accompanied with tumor in the lateral ventricle reviewing all reports of both ventricular diverticulum and spontaneous ventriculostomy. The second, this ventricular diverticulum was not produced merely by hydrocephalus alone but mainly by direct effects of the tumor.(ABSTRACT TRUNCATED AT 250 WORDS)
The authors report a case of internal carotid artery aneurysm of the cavernous portion, which was treated by trapping of the internal carotid artery using a detachable balloon technique. In April 1984, 56-year-old female was admitted with chief complaint of ptosis of the left eyelid lasting for the past two years. A neurological examination on admission revealed the left oculomotor nerve palsy. Contrast CT scan and left carotid angiogram showed an aneurysm at the cavernous portion of the internal carotid artery. On June 11th, three days before the trapping, superficial temporal artery-middle cerebral artery anastomosis was performed using short vein graft. On June 14th, the internal carotid artery was trapped at the distal and proximal portions of the aneurysmal neck using two Debrun's No. 16 balloons. Digital subtraction angiography taken two weeks after the trapping showed complete occlusion of the left internal carotid artery with no visualization of the aneurysm. Contrast CT scan taken three months after the trapping highly indicated complete thrombosis of the lumen of the aneurysm. Following the intravascular surgery, the left oculomotor paresis was gradually improved. The authors emphasized that the trapping of the internal carotid artery using a detachable balloon technique was the most safe and beneficial method for the treatment of the aneurysm located in the cavernous portion of the internal carotid artery.

We evaluated whether any monosaccharides inhibit glycolysis in erythrocytes and discovered that D-mannose does. In the presence of D-mannose, glucose can be accurately measured by either the hexokinase procedure or the glucose oxidase procedure. In comparison studies with other glucose preservatives, we found that after 2 h at room temperature glucose decreased by 21 (SD 13) mg/L in D-mannose-treated blood, 93 (SD 10) mg/L in sodium fluoride-treated blood, 28 (SD 21) mg/L in ice-cooled blood, and 144 (SD 28) mg/L in control blood (no preservative treatment). Because D-mannose acted in the early phase of glycolysis, it was a more effective preservative than sodium fluoride; moreover, its use did not preclude measurement of sodium and potassium in the blood samples. D-Mannose did not interfere with other routine chemical tests except for the assay of creatine kinase involving coupled enzymes hexokinase/glucose-6-phosphate dehydrogenase. Creatine kinase could be correctly assayed in the presence of D-mannose by using glucokinase instead of hexokinase. D-Mannose can be used with or without anticoagulant and is compatible with most types of multi-channel automated analyzers.
