[Clinical results of repeated intraperitoneal administration of CDDP in patients with ovarian cancer].

CDDP was intraperitoneally administered to 45 ovarian cancer patients (9 cases in Stage Ic, 9 cases in Stage II, 19 cases in Stage III and 8 cases in Stage IV) as the first line chemotherapy to examine response rates and cumulative survival rates by Kaplan-Meier method. Among 18 cases with measurable tumor, there were 5 CR cases, 6 PR cases, 4 NC cases and 3 PD cases. Eleven out of 18 responded, for a response rate of 61.1%. The 3- and 5-year survival rates were 88.9% and 88.9% in Stage I, 46.7% and 0% in Stage II, 48.6% and 41.7% in Stage III, and 0% and 0% in Stage IV, respectively. The 3- and 5-year survival rates in patients with residual tumor smaller than 2 cm in diameter after the first surgery in Stage III or IV were 76.9% and 61.4% respectively, in contrast to 11.5% and 11.5% in patients with residual tumor of 2 cm or larger in diameter. Thus, a significantly better prognosis was demonstrated in patients with residual tumor smaller than 2 cm. These results indicate that intraperitoneal administration of CDDP demonstrated high response rates and proved to be useful as the first line chemotherapy, yet did not manage to improve the long-term prognosis of patients with progressive cancer.

[Multiple calcium oxalate stone formation in a patient with glycogen storage disease type I (von Gierke's disease) and renal tubular acidosis type I: a case report].

A case of multiple urinary stones in a patient with glycogen storage disease type 1 (GSD-1) is reported. In spite of the presence of hyperuricemia, these stones did not consist of uric acid, but mainly of calcium oxalate. Laboratory studies revealed distal renal tubular acidosis and hypocitraturia, but no significant abnormality in calcium metabolism. We discussed the mechanism of calcium stone formation in our case, and its prophylactic treatment by oral administration of citrate compound.

[Event-related potentials in progressive supranuclear palsy].

Abnormal event-related potential (ERP) has been reported in several types of dementing illness including Parkinson's disease (PD) and progressive supranuclear palsy (PSP). In this paper we report the result of EPR recordings obtained in normal control subjects (14 cases) and in patients with PSP (6 cases) and PD (32 cases). ERPs were recorded from 13 scalp electrodes, using the acoustic odd-ball paradigm. All PD patients were in early stage (Yahr stage 1 to 2), and all PSP patients were in the mild-to-moderate state. In control subjects and PD patients P3 was normal. In PSP patients, only one subject (17%) showed a normal P3 component, three cases (50%) demonstrated low amplitude and normal latency P3, and two cases (33%) featured lack of normal P3 sequence. The P3 abnormalities in PSP patients were not related with dementia, but with low amplitude P3 being observed in 2 of 3 non-demented patients, and abolished P3 in a patient with only mild dementia. The result suggests that the P3 changes in PSP patients develop earlier than the cognitive dysfunction, and they are related to the underlying subcortical lesion rather than dementia.

[Potentiation of cytotoxicity of anticancer drugs by flavone acetic acid].

We used B 16 melanoma cells both in vitro and in vivo to determine whether Flavone acetic acid (FAA) can increase the cytotoxicity of mitomycin C (MMC) under normothermic and hyperthermic (HT) conditions. The significantly increased cytotoxicity of MMC combined with FAA and HT seems to be mainly related to activation of alkylating species under FAA mediated hypoxic condition.

[A case of müellerian duct cyst with prostate cancer].

A case of müellerian duct cyst with prostate cancer is reported. The patient was a 64-year-old male, complaining of hemospermia. Prostate cancer was suspected by rectal examination and a cystic mass was detected between the ejaculatory ducts by computed tomography and magnetic resonance imaging. He underwent a transperineal needle aspiration of the cyst. The fluid did not contain spermatozoa. A adenocarcinoma was detected from the prostate by needle biopsy. Therefore, a radical prostatectomy with the resection of the cyst was performed. This is the first report about the resection of müellerian duct cyst with the organs around it in Japan.

[Experimental and clinical study in vitro chemosensitivity test for succinate dehydrogenase inhibition test].

The usefulness and problems in the succinate dehydrogenase inhibition (SDI) test as an in vivo chemosensitivity test were reviewed and discussed. SDI test is based on the correlation of succinate dehydrogenase activity with cell viability. The optimal condition of SDI test were evaluated with respect to concentration of anticancer drugs, incubation time, effect of sodium succinate on formazan production and enzymatic disaggregation of solid tumors. Analysis of the chemosensitivity of clinical tumors revealed that the origin of tumor, histological differentiation, DNA distribution patterns, estrogen receptor status in breast cancer, and difference of primary and metastatic lesions were critical for determining the chemosensitivity of tumor tissues. Correlation between in vitro sensitivity in the SDI test and in vivo clinical results of chemotherapy was evaluated. Since SDI test reflects cell viability and is a simple, rapid and inexpensive methods, it is appropriate both for clinical chemosensitivity testing and for the screening of anticancer drugs.

[Side effects of estrogen administration to prostatic cancer patients: clinical and statistical survey of 109 prostatic cancer cases of Kyoto University Hospital].

Since the introduction of hormonal treatment for prostatic cancer by Huggins and Hodges in 1941, severe side effects of synthetic estrogen, which have overcome its benefit, have been reported in the U.S.A. and in European countries. However, in Japan the adverse effects of estrogen have been reported to be milder than in western countries, and estrogen still has an important role in the treatment of prostatic cancer in Japan. In this communication, the side-effects of synthetic estrogen administered to 109 prostatic cancer patients, who were admitted to Kyoto University Hospital between 1980-1990 are reported. Fifty-three (48.6%) of the 109 patients suffered adverse side effects of the estrogen, specifically cardiac disease (20.2%), fluid retention (14.7%) and hypertension (13.8%). Five of these patients died. Among the risk factors analyzed, daily dose, past history of cardio-vascular disease and ECG abnormalities were significantly correlated with the appearance of adverse effects. The reasons why the frequency of lethal side-effects is lower in our cases compared to findings reported by the Veterans Administration group may be the lower daily dose and cessation of estrogen administration when mild adverse effects appear and some other unknown factors, although the background of the patients and method of analysis are not comparable among them. The overall frequency of side-effects in prostatic cancer patients administered estrogen in our cases is not necessarily lower than in western countries, but the severity of the side effects was milder in our cases. We must be a ware of the potential adverse effects of estrogen.

[Quality of life of the patients with continent urinary reservoir].

To improve the quality of life (QOL) of patients requiring urinary diversion, various surgical techniques including those employing a continent urinary reservoir (CUR), such as Kock pouch or Indiana pouch, and those using the conventional ileal conduit have been advocated. In this study we asked patients who had undergone a CUR operation to complete a questionnaire, consisting of 106 questions relating to the QOL during the disease-free period. A total of 81 questionnaires were sent out, and 60 (74.1%) CUR patients responded. The average age was 61 and the male to female ratio was 49 to 11. The time elapsed from the operation was 42 months. Twenty-nine questions addressed the patients physical condition and on the whole it was kept in good quality. Because they were unwilling to show their stoma, they were reluctant to bathe together with family members. However, 59% of them went to a public bathhouse (common in Japan). Other physical abilities related to exercise were not significantly hampered by this operation. Eighty per-cent of them were not bothered by having a stoma. Twenty four questions were related to social life. The operation had a bad influence in 20-30% of them concerning their interaction with friends and hobbies. Twenty two percent of the patients who had a job prior to the operation stopped working. Only a few patients abandoned travelling. Concerning their sex life (7 questions), males suffered impotence at a very high rate, but only 26% of the patients lost interest in sex. Symptoms related to the operation (41 questions) were mild. Overall, 94% of the patients would choose the same operation again if it were required.(ABSTRACT TRUNCATED AT 250 WORDS)

[Multifocal renal oncocytoma: a case report].

A case of multifocal renal oncocytoma in a 55-year-old man admitted for further examination following the detection of left renal tumors, is reported. Computerized tomographic (CT) scanning and magnetic resonance imaging (MRI) revealed 2 masses in the left kidney. A left radical nephrectomy was performed and two tumors were found. Both tumors were confirmed to be renal oncocytomas by light and electron microscopic examination. Including this patient, 42 cases of bilateral and multifocal renal oncocytoma have been reported in the literature to date. The clinical features of bilateral and multifocal renal oncocytomas are discussed accordingly.

The usefulness of Avitene for the control of oozing in laparoscopic cholecystectomy.

An efficient method for the laparoscopic application of Avitene is presented herein. During laparoscopic operation, we cannot staunch bleeding points directly. Given such circumstances, the spray application of Avitene is a very effective method of hemostasis that should make laparoscopic operations even safer.

[Fluorocytometric DNA analysis of intestinal metaplasia associated with gastric carcinoma].

Histopathologic studies and fluorocytometric DNA analysis were carried out in 39 patients with intestinal metaplasia (IM) of the mucosa associated with gastric carcinoma, in order to determine possible relationships. According to the histological degree of IM, 39 cases were divided into 16 (41%) with slight-to-moderate, and 23 (59%) with severe IM. The 39 tumors were histologically divided into 20 (51%) differentiated tumors and 19 (49%) undifferentiated tumors. The DNA patterns of tumors revealed 17 (43%) to be euploidy and 22 (57%) to be aneuploidy. The DNA content of the metaplastic epithelial cells was measured, and the percentage of the cells in S and G2 + M phases of the cell cycle was taken as the index of proliferative activity. DNA analysis of the non-metaplastic epithelium was performed in 10 as a control group. While the histological differentiation of the tumors and the degree of IM were not related to the proliferative activity of IM, the IM around the aneuploid tumors did have higher proliferative activity (14.01 +/- 6.16%) than that around euploid tumors (11.08 +/- 7.13%) or of normal mucosa (9.95 +/- 9.92%). This approach will help to determine cell kinetics of IM around the gastric tumor. Our study indicated a relationship between DNA ploidy and the proliferative activity of IM around gastric cancer.

[Complications in urological laparoscopic surgery].

Laparoscopic surgery is often associated with complications which never occur in open conventional surgery. We investigated operative and postoperative complications in urological laparoscopic surgery. From February 1990 to November 1992, 91 laparoscopic operations were performed, including 62 varicocelectomies, 5 pelvic lymphadenectomies, 6 adrenalectomies, 5 simple nephrectomies, 2 radical nephrectomies, 1 staged Fowler-Stephens operation and 10 laparoscopies for cryptorchidism or other disorders. The patients ranged from 3 months to 77 years old. Nine (9.9%) of the 91 patients had complications associated with laparoscopic procedures and 4 (4.4%) of them had major complications. Pneumoextraperitoneum occurred in 3 patients. Two patients, who suffered from preperitoneal insufflation or pneumothorax associated with pneumomediastinum, failed to undergo laparoscopy. A small amount of scrotal emphysema developed in one patient, which subsided within 12 hours. Another patient, who underwent radical nephrectomy and para-aortic lymphadenectomy for left renal cell carcinoma, complained of symptoms due to persistent pneumoperitoneum and subcutaneous emphysema, which subsided 2 weeks after the operation. Respiratory acidosis caused by carbon dioxide absorption, which was difficult to be controlled, occurred in the other two patients; one was an adult male with reduced respiratory function and the other was a 1-year-and-8-month-old child. Bleeding during or after the procedure was another serious complication. Vena caval laceration occurred during right simple nephrectomy for pyonephrosis with renal calculi and was managed with laparotomy. Two patients, one after simple nephrectomy and the other after right adrenalectomy, suffered from postoperative bleeding about 300 ml through the penrose drain, which subsided following a bed rest.(ABSTRACT TRUNCATED AT 250 WORDS)

[Long-term complete response of multiple lung metastases from renal cell carcinoma induced by combination therapy with interferon alpha and UFT: a case report].

We report a case of long-term complete response of multiple lung metastases of renal cell carcinoma (RCC) by the combination therapy with interferon alpha (IFN alpha) and UFT. A 38 year-old man having left RCC with lung metastases underwent radical left nephrectomy and extended lymph node dissection, the pathological stage being pT2N2. Although metastatic lung tumors increased in size and number against intravenously admitted interferon gamma (IFN gamma) therapy after the surgery, they completely disappeared following the subsequent combination therapy with intramuscularly admitted interferon alpha (IFN alpha) and oral intake of UFT in about 2 years. The patient has been disease-free for 5 years after cessation of the treatment. Combination therapy with IFN alpha and UFT might be more effective on metastases of RCC than single use of IFN alpha or IFN gamma.

[Clinical study on efficacy of a Foley catheter coated with silver-protein in prevention of urinary tract infections].

We evaluated the clinical efficacy and safety of a Foley catheter coated with silver-protein (ProAg catheter) in the prevention of catheter-associated bacteriuria. ProAg catheter significantly reduced the incidence of extraluminal catheter-associated bacteriuria compared with usual latex Foley catheter although it did not inhibit intraluminal bacteriuria. There was no difference between ProAg catheter and latex catheter in the side effects such as urethral discharge, catheter-associated pain and allergic reaction. The ProAg catheter may be useful as an indwelling urethral catheter.

Raman spectroscopic study on the conformation of a peptide fragment representing the DNA-binding domain of filamentous virus Pf3 coat protein.

Raman spectra have been measured of a nonapeptide which has an amino acid sequence identical to that of the C-terminal region of the major coat protein subunit of filamentous bacteriophage Pf3. The peptide shows a strong tendency to form a beta-sheet structure in aqueous solution. The beta-sheet formation is significantly promoted by complexation with single-stranded DNA but not with double-stranded DNA. It is suggested that the C-terminal region of the Pf3 coat protein binds to the single-stranded DNA genome in the virion with a beta-sheet conformation, in sharp contrast with the alpha-helical binding in other filamentous bacteriophages.

Structural gene and complete amino acid sequence of Vibrio alginolyticus collagenase.

The DNA encoding the collagenase of Vibrio alginolyticus was cloned, and its complete nucleotide sequence was determined. When the cloned gene was ligated to pUC18, the Escherichia coli expression vector, bacteria carrying the gene exhibited both collagenase antigen and collagenase activity. The open reading frame from the ATG initiation codon was 2442 bp in length for the collagenase structural gene. The amino acid sequence, deduced from the nucleotide sequence, revealed that the mature collagenase consists of 739 amino acids with an Mr of 81875. The amino acid sequences of 20 polypeptide fragments were completely identical with the deduced amino acid sequences of the collagenase gene. The amino acid composition predicted from the DNA sequence was similar to the chemically determined composition of purified collagenase reported previously. The analyses of both the DNA and amino acid sequences of the collagenase gene were rigorously performed, but we could not detect any significant sequence similarity to other collagenases.
