[Neurotransmitters of giant neurons in the African giant snail, Achatina fulica Férussac. IV. Supplemental results].

Following the previous works, we identified recently the twelve giant neurones in the ganglia of an African giant snail (Achatina fulica Férussac), by the pharmacological study of their sensitivities to putative neurotransmitters and derivatives, and by the morphological investigation of their axonal pathways due to the intracellular injection of Lucifer Yellow. The neurones studied were: TAN-2, TAN-3, BAPN, LPPN, LBPN and LAPN in the right parietal ganglion; RPeNLN and LPeNLN in the pedal ganglia; and d-LBAN, d-LBMN, d-LBCN and d-LBPN in the left buccal ganglion.

[Effects of biologically active peptides on the giant neurons of the giant African snail, Achatina fulica Férussac].

Effects of neuropeptides proposed as neurotransmitters of mammals and invertebrates and those of natural toxins, etc., on the identifiable giant neurones of an African giant snail (Achatina fulica Férussac) were examined. Oxytocin had a tendency to show the excitatory effects; this substance was excitatory on five neurones and inhibitory on one neurone. A substance related to oxytocin, vasotocin, also showed excitatory effects on the three neurones. On the other hand, FMR Famide was inhibitory on six neurones. Proctolin had excitatory effects on only one neurone.

[Clinical application of MRI for urological malignancy. 2: Usefulness of various imaging modalities for local staging of bladder cancer; a comparison between MRI, CT and transurethral ultrasonography].

MRI of bladder cancer was accomplished in 28 patients. All of the 28 patients were simultaneously studied by CT, and 22 were also studied by transurethral US before operation. MRI is not as useful as US for local staging of bladder cancer. However, MRI is more useful for diagnosing extravesical extension and covers the deficit of the overdiagnosing tendency of US.

Female wide urethra masquerading as a urethral diverticulum in association with ectopic ureterocele.

A rare case of wide urethra in a female infant mimicking as a urethral diverticulum in association with ectopic ureterocele is reported. The ureterocele terminated just below the bladder neck, but the diverticulum-like structure everted from the wide urethra, protruding towards the ureterocele. Weakness of the urethral wall, distal to the actual ureterocele may produce such an abnormal radiologic finding. The pertinent literature is reviewed.

[Laboratory and clinical studies of rokitamycin dry syrup in the field of pediatrics].

Laboratory and clinical studies on rokitamycin (RKM) dry syrup, a new macrolide antibiotic, were carried out in the field of pediatrics. The results are summarized as follows. 1. Plasma concentrations and urinary recovery rates after oral administration on fasting of RKM dry syrup at doses of 10 mg/kg and 20 mg/kg to 2 and 1 cases, respectively, were determined. Peak plasma levels were obtained in 30 minutes after administration of both dosages with half-lives of 1.5 to 2.2 hours. A clear-cut dose response was observed. Urinary recovery rates in the first 6 hours after administration ranged from 1.75 to 2.26%. 2. The MICs of RKM against 80 clinical isolates (Streptococcus pyogenes 9, Streptococcus pneumoniae 14, Branhamella catarrhalis 4, Haemophilus influenzae 27, Haemophilus parainfluenzae 9, Haemophilus haemolyticus 2, Haemophilus parahaemolyticus 14 and Campylobacter jejuni 1) were compared with MICs of midecamycin acetate (MOM), josamycin (JM) and erythromycin (EM). The antibacterial activity of RKM was superior to those of MOM and JM and slightly inferior to that of EM. 3. Twenty-eight pediatric patients with acute infectious diseases (acute tonsillitis 4, streptococcal infection 4, acute bronchitis 9, pneumonia 4, mycoplasmal pneumonia 2 and Campylobacter enteritis 5) were treated with RKM dry syrup at a daily dose of 12-42.9 mg/kg t.i.d. as a rule. Efficacy rates were 92.9% clinically and 58.6% bacteriologically. 4. No adverse reactions were observed. Abnormal laboratory findings were mild; thrombocytosis in 2 and eosinophilia in 1. 5. The taste and the odor of RKM dry syrup preparation were sufficiently tolerable for the pediatric patients to accept it.

[Urinary diversion with Kock pouch: clinical results in 75 cases focusing on late complications].

Internal urinary reservoir type urinary diversions have been getting popularity since Kock reported a continent ileal reservoir in 1982. From November 1984 through October 1987, we performed Kock pouch operation in 75 patients (male 64, female 11; from 24 to 82 years old, mean age 56 years old). The underlined diseases were mostly bladder cancer patients; bladder cancer 70, rectal cancer 2, prostatic sarcoma 1, vesical exstrophy 1 and neurogenic bladder 1 case. The end results for 71 evaluable cases, followed up more than 3 months, were excellent in 49 (69%), good 14 (20%), fair 6 (8.5%) and poor 2 (2.8%), with success rate 89%. The most common complication was the nipple malfunction; prolapse, including intermittent prolapse, in 7 patients, slippage or sliding in 1, and eversion in one patient. Stone formation occurred in 6 patients, mild acidosis in 2 and a entero-pouch fistula in a patient who was re-diverted from a ileal conduit. There were 2 deaths postoperatively. Late complications were prominently decreased due to several important modifications of the operative technique, such as usage of Cavitron Ultrasonic Surgical Aspirator (CUSA) for defatting the mesentery, and fixing the nipple to the pouch. In conclusion, Kock pouch can be a safe and sound operative modality for patients who need urinary bladder replacement.

[An architectural study of mixed uric acid and calcium oxalate stones by polarized light microscopy of thin section].

The architecture of mixed uric acid and calcium oxalate stones was studied by polarized light microscopy. Some uric acid stones contained calcium oxalate stones (whewellite or weddellite) as a nucleus. Other uric acid stones contained calcium oxalate crystals in the nucleus or inner portion. A uric acid stone covered with layers of calcium oxalate was also found. In the mixed stones, nuclei of randomly mixed fine grained whewellite and uric acid were seen and both crystals appeared in alternating sequence in the outer layers. Weddellite crystals dissolved layers of uric acid and invaded into them in the nuclear or peripheral zones. Whewellite may precipitate in the same or a similar condition as uric acid precipitates, whereas weddellite may precipitate in a different condition. We discussed the pathogenesis of mixed uric acid and calcium oxalate stones, heterogeneous nucleation and epitaxial growth based on a morphologic study of the mixed stones.

[Pharmacokinetic, bacteriological and clinical studies of sultamicillin fine granule in pediatric field].

Pharmacokinetic, bacteriological and clinical studies on sultamicillin (SBTPC) fine granule were carried out in the field of pediatrics. The results obtained are summarized as follows. 1. Antibacterial activities of SBTPC against clinically isolated strains of Haemophilus influenzae, Haemophilus parainfluenzae, Streptococcus pneumoniae, Streptococcus pyogenes, Streptococcus agalactiae, Staphylococcus aureus, Branhamella catarrhalis, and Escherichia coli were compared with those of ampicillin (ABPC). SBTPC was superior to ABPC especially against beta-lactamase producing H. influenzae, E. coli, S. aureus, and B. catarrhalis. 2. Serum concentrations and urinary excretion rates of sulbactam (SBT) and ABPC after administration of SBTPC fine granule at a dose level of 10 mg/kg in 2 cases were determined. Mean half-lives of SBT and ABPC in the serum following oral administration were about 1.33 and 1.61 hours respectively. Mean urinary recovery rates of SBT and ABPC in 6 hours after oral administration at a dose of 10 mg/kg were 58.7% and 49.6% respectively. 3. SBTPC fine granule was administered to 20 pediatric patients with various bacterial infections (pneumonia 8 cases, bronchitis 2, pharyngitis 2, tonsillitis 4, subcutaneous abscess 1 and urinary tract infection 3). The overall clinical efficacy rate was 100% and the overall bacteriological eradication rate was 75%. 4. No adverse reactions were observed except 1 case of loose stool. No abnormal laboratory test values were observed. These results indicate the usefulness of SBTPC fine granule in the treatment of bacterial infections in children.

[Intrapelvic cyst presenting with urinary retention: a case report].

A case of intrapelvic cyst is presented. A 74-year-old woman was admitted with the chief complaint of urinary retention. Intrapelvic cyst was found by computed tomography scan during evaluation for urinary retention. Surgical extirpation was performed and pathological examination revealed paraovarian cyst.

Right iliac metastasis occurring eight years after resection of invasive bladder cancer: a case report.

A case of transitional cell carcinoma of the bladder which metastasized to the right ilium eight years after resection of the original tumor without recurrence in the urinary tract is reported. Both the original and metastatic tumors were shown by immunoperoxidase technique to contain carcinoembryonic antigen (CEA). The patient received a combination of intra-arterial chemotherapy, irradiation and hyperthermia with partial response.

Bonding behavior of three types of hydroxyapatite with different sintering temperatures implanted in bone.

The present study suggests that bonding does not vary in hydroxyapatite (HA) with different sintering temperatures implanted in bone tissue. The apparent unimportance of strict temperature regulation greatly simplifies HA processing. Three samples of HA (15 mm x 10 mm x 2 mm, abraded with 2000 alumina powder), of which sintering temperatures and microporosity were 800 degrees (45%), 1000 degrees (4.8%), and 1200 degrees (0.4%), respectively, were prepared. They were implanted into the metaphysis of tibia of mature male rabbits. Segments of the tibia containing the HA were excised and prepared for the detaching test. The failure load, when an implant detached from the bone or the bone broke, was then measured. Failure loads in HA (800 degrees), HA (1000 degrees), and HA (1200 degrees) ten weeks after implantation did not differ. Those in HA (800 degrees), HA (1000 degrees), and HA (1200 degrees) 25 weeks after implantation were 2.58 +/- 1.88 kg, 6.46 +/- 1.43 kg, and 6.50 +/- 2.71 kg, respectively. The failure load in HA (800 degrees) versus HA (1200 degrees) and that in HA (800 degrees) versus HA (1000 degrees) differed significantly (p less than 0.05). The lower failure load of HA (800 degrees) 25 weeks after implantation can be explained by its weakened mechanical strength due to high porosity. Bonding between bone tissue and three types of HA did not vary as observed by contact microradiography, Giemsa surface staining, and scanning electron microanalyzer-energy dispersive X-ray microanalyzer.
