Clinical and roentgenological studies on malalignment disorders of the patello-femoral joint. Part I: Classification of patello-femoral alignments using dynamic sky-line view arthrography with special consideration of the mechanism of the malalignment disorders.

Dynamic sky-line view arthrography at 30, 60, and 90 flexion degrees was carried out on 1,242 knees of 621 cases who claimed trouble with at least one knee. The patient's age range was from 15 to 35 years. Alignment of the patello-femoral joint was classified into four groups; Grade I: The patello-femoral joint is congruent in all the three views. Grade II: The view taken at 30 degrees shows the joint space open in the medial facet but closed in the view at 60 degrees. Grade III: The medial interspace is open at 60 degrees but closed at 90 degrees. Grade IV: The medial joint space is open in all three views. Grade distribution of all cases studied was: Grade I, 62.1%; Grade II, 25.8%; Grade III, 6.7%; and Grade IV, 5.4%. Clinical disorders thus occur at high percentages in the groups of Grades III and IV. Based on this observation, the mechanism of the malalignment disorders of the patello-femoral joint is discussed.

Approach to evaluation of impairment in rheumatoid arthritis--a study by multiple regression analysis.

We investigated the weight of various factors involved in the activity of rheumatoid arthritis (RA) and joint deformity in an attempt to establish a more objective method of evaluation of the impairment in RA using these factors as parameters. The subjects were 100 patients with RA. Multiple regression analysis was undertaken using Lansbury parameters and Stage as predictor variables and the classification of functional capacity (Class) as a criterion variable. The results of analysis showed that the weight was greatest, 32% for the time of appearance of fatigue, followed by 29% for the Stage, 16% for joint index, 10% for grasping power, 7% for ESR, 5% for morning stiffness and 0% for the dose of aspirin used. On the basis of these results, an original method of evaluation of impairment in RA was proposed. There was a close correlation (R = 0.82) between the Class and the total score obtained by this method.

[Leiomyosarcoma of the prostate: a case report].

A 39-year-old man with the chief complaints of gross hematuria and dysuria was referred to our hospital on June 6th, 1985. Physical examination revealed a tumor in the left lobe of the prostate. Prostatic needle biopsy showed the histological evidence of leiomyosarcoma. Pelvic exenteration with pelvic lymph adenectomy was performed on June 18th, 1985. Histological findings of the tumor revealed leiomyosarcoma and no lymph nodes metastasis. Two courses of vincristine-adriamycin-cyclophosphamide chemotherapy were given postoperatively after a clonogenic assay was done. He considerably improved and was discharged 3 months after operation. He shows no recurrence of tumor 17 months after operation. Thirty-three cases of leiomyosarcoma reported in Japan are reviewed.

[Differential diagnosis and staging of urological tumors by magnetic resonance imaging compared with computed tomography].

Magnetic resonance imaging (MRI) was performed on 49 urological tumors (11 renal cell carcinomas, 3 renal pelvic cancers, 2 renal angiomyolipomas, 1 renal leiomyosarcoma, 1 large renal cyst, 4 adrenal tumors, 11 bladder cancers, 2 bone metastasis from bladder cancer, 10 prostatic cancers, 1 prostatic sarcoma, 1 urethral cancer, 1 penile cancer and 1 perivesical granuloma) since October 1985 to September 1986. MRI was performed using a Signa (G.E.) with a 1.5T superconductive magnet and 3 images, including T1 weighted image, T2 weighted image, and proton density image, were obtained. In conclusion MRI is a noninvasive examination and gives more information than computed tomography despite its high cost. In renal cell carcinoma, the chemical shift in MRI and clear visualization of tumor thrombus enable accurate staging. Differential diagnosis from other renal mass lesions may be possible by the T2 weighted image. In adrenal disease, most of the adrenal masses can be differentiated, but in some cases it is impossible. In bladder cancer, wall invasion of tumor may be evaluated in T2 weighted image, and MRI is suitable for staging of locally advanced tumor. In prostatic cancer, visualization of periprostatic plexus and differentiation between internal and external gland may enable local staging and identification of low stage tumors.

[Results of radical nephrectomy for renal cell carcinoma. Report 1. Analysis according to the TNM staging system of the general rule for clinical and pathological studies on renal cell carcinoma].

One hundred and six patients with renal cell carcinoma were treated with radical nephrectomy at our Department between 1970 to December, 1985. A retrospective analysis was performed with TNM staging system of The General Rule for Clinical and Pathological Studies on Renal Cell Carcinoma, which was established by the Japanese Urological Association in 1983. The 5-year survival rate according to pathological T-stage was 100% for 2 patients in pT1, 67.5% for 58 patients in pT2, 49.5% for 42 patients in pT3, 0% for 4 patients in pT4. Two patients in stage of pT1 had no venous involvement, lymph node metastasis, or distant metastasis. Twenty two patients had positive venous involvement (21%), 4 (7%) in stage of pT2, 16 (38%) in pT3, 2 (50%) in pT4. Twelve patients had positive lymph nodes (11%), 0 (0%) in stage of pT2, 10 (24%) in pT3, 2 (50%) in pT4. Twenty five patients, (24%) had distant metastasis at the time of nephrectomy, 8 (14%) in stage of pT2, 15 (38%) in pT3, 2 (50%) in pT4. The 5-year survival of 22 patients with venous involvement, 12 patients with lymph nodes metastasis, 25 patients with distant metastasis were 47%, 30%, 39% respectively. No significant difference of 5-year survival between 69% of 48 patients in T1 & 2VoNoMo (Robson-I) and 76% of 12 patients in T3VoNoMo (Robson-II) were considered to need the establishment of new classification for early stage of renal cell carcinoma. TNM staging system was thought to be better than Robson's Classification for analyzing the unique biological potential of renal cell carcinoma.

[A case of congenital penile curvature].

A case of congenital penile curvature is reported. A 19-year-old male with ventral curvature of the penis since adolescence was surgically repaired by Nesbit's technique. Marked curvature during erection and coital dysfunction was completely corrected by the operation. The definition of the clinical entity and the methods of surgical repair, emphasizing the superiority of Nesbit technique, was briefly summarized.

[Usefulness of the bladder assist device (BLAD)].

We investigated the effectiveness of the Bladder Assist Device on urinary tract infection of patients with indwelling catheters. The patients, except for those with cord bladder and vesicoureteral reflux, could urinate intermittently under the BLAD system. Irrigation of the bladder using BLAD made urine clear and decreased urinary bacteria. Because of intermittent urination in patients who had been using the BLAD system for two weeks, the urine became somewhat clear and the number of bacteria decreased. Combination of bladder irrigation and intermittent urination using this system may inhibit urinary tract infection of an indwelling catheter. BLAD is considered to be a useful device for patients with continuous indwelling catheters.

[Observation on fungi within urinary stones].

We examined the fungi within the urinary stones of two cases by light microscopy and scanning electron microscopy. In one case, fungal elements, hyphae and spores, were found in some layers within a bladder stone. This patient probably had a history of fungal urinary infection. In another case brown hyphae suggesting dematiaceous fungi were found in the surface and peripheral layers of a kidney stone. This maybe due to contamination of the fungi after removal of the stones. These fungi invaded into interstices of the apatite crystals within the stone as in the tissue.

Seadragon VI: a 7-day dry saturation dive at 31 ATA. II. Characteristics of diuresis and nocturia.

Four male divers were exposed to a dry, 31 ATA, He-O2 environment for 7 d (Seadragon VI). Urine was collected diurnally (0700-2200 h) and nocturnally (2200-0700 h) before (predive 1 ATA air), during, and after (decompression and postdive 1 ATA air) exposure to 31 ATA. A typical hyperbaric diuresis associated with a reduction in urine osmolality was observed in the face of a constant creatinine clearance. A significant increase (P less than 0.05) in osmolal clearance (COSM) was observed with concomitant decrease (P less than 0.05) in negative free water clearance, indicating that the diuresis has both osmotic and free water components. Although urine flow increased at pressure during both day and night, its magnitude was twofold greater at night as compared to daytime. Moreover, the diurnal diuresis was entirely due to an increase in free water excretion, whereas the nocturnal diuresis was largely due to an increased COSM. These results indicate that hyperbaric diuresis is induced by inhibition of tubular reabsorption of free water during daytime and of certain solutes during the night. Approximately 80% of the nocturnal increase in the excretion of osmotic substances was accounted for by Na, K, Cl, and urea.

Seadragon VI: a 7-day dry saturation dive at 31 ATA. IV. Circadian analysis of body temperature and renal functions.

Circadian rhythms of body temperature and daytime rhythms of urine flow and urinary excretion of electrolytes were investigated in 4 male subjects before, during, and after a 7-d stay in a dry heliox 31 ATA environment. The chamber temperature was maintained at about 28 degrees C during pre- and postdive 1 ATA periods and was raised to 31.5 degrees C at 31 ATA. The circadian rhythm of the rectal temperature, as analyzed by the cosinor fitting method, showed the same mesor (the mean level of fluctuation) and the amplitude at 31 and 1 ATA. However, a reversible phase shift was noted at 31 ATA in which the acrophase shifted to 1435 h at 31 ATA from 1540 h (predive) or 1610 h (postdive) at 1 ATA. This shift was attributed to an early rise of rectal temperature during night at 31 ATA. A similar phase shift was observed at 31 ATA for the skin temperature of the forehead, a region not covered by clothing or blanket. The daytime rhythms of urinary excretion of water, Na, Cl, urea, and total osmotic substances were similar, with the acrophase at 1300-1500 h at both 1 and 31 ATA. On the other hand, the daytime rhythm for urinary excretion of K, which was similar to the above at 1 ATA, disappeared at 31 ATA. The urinary excretion of endogenous creatinine remained constant during both daytime and nighttime at both 1 and 31 ATA. These results suggest that exposure to 31 ATA may alter the underlying pattern of circadian or daytime rhythms for thermoregulatory and certain renal functions.

Seadragon VI: a 7-day dry saturation dive at 31 ATA. VIII. Plasma enzyme profiles.

This report summarizes serum profiles of liver enzymes of divers during 2 dry saturation dives to 31 ATA. In both dives, serum glutamic pyruvic transaminase (SGPT), serum glutamic oxaloacetic transaminase (SGOT), and alkaline phosphatase were significantly elevated at 31 ATA when compared to predive control levels. SGPT and SGOT levels returned to control levels during the postdive period. These data provide strong evidence for compromised liver function at high pressure and are consistent with similar observations in other saturation dives. The reason for the parenchymal dysfunction at high pressure remains unknown.

Quantitative analysis of the mitochondrial cytochrome P-450-linked monooxygenase system: NADPH-hepatoredoxin reductase, hepatoredoxin, and cytochrome P-450s27 in livers of patients with cerebrotendinous xanthomatosis.

We studied the mitochondrial cytochrome P-450-linked monooxygenase system in livers of two patients with cerebrotendinous xanthomatosis (CTX). The three components of this system, which catalyzes steroid 27-hydroxylation, NADPH-hepatoredoxin reductase, hepatoredoxin, and cytochrome P-450s27, were stained on a nitrocellulose sheet with antibodies against NADPH-adrenodoxin reductase, adrenodoxin, and cytochrome P-450scc, respectively, from bovine adrenocortical mitochondria. The concentrations of hepatoredoxin in the patients were not significantly different from a control, but the level of NADPH-hepatoredoxin reductase was three times that of the control. Cytochrome P-450s27 was not detected in the patients, but it was present (22.8 pmol/mg of protein) in the control liver. This implies that a defect of mitochondrial cytochrome P-450s27 prevents steroid 27-hydroxylation of hepatic mitochondria in patients with CTX.

Effects of catecholamine and monophenolamine agonists on identifiable giant neurones, sensitive to these amines, of an African giant snail (Achatina fulica Férussac).

The effects of 23 substances proposed as catecholamine (CA) and monophenolamine (MA) agonists were tested on the five CA-sensitive neurones, periodically oscillating neurone (PON), tonically autoactive neurone (TAN), left-visceral multiple spike neurone (1-VMN), dorsal-right pedal autoactive neurone (d-RPeAN) and visceral intermittently firing neurone (VIN), and the three MA-sensitive neurones, frequently autoactive neurone (FAN), dorsal-left pedal large neurone (d-LPeLN) and dorsal-left cerebral distinct neurone (d-LCDN), of an African giant snail (Achatina fulica Férussac). Of these neurones, PON, VIN and d-LPeLN were excited by the most effective catecholamine or monophenolamine, i.e. dopamine, epinine or DL-octopamine, whereas TAN, 1-VMN, d-RPeAN, FAN and d-LCDN were inhibited by the same substances. Of the five CA-sensitive neurones, PON and VIN were markedly excited by ergometrine (effective potency quotients (EPQs) of these substances as compared with the effective potency of the most effective catecholamine or monophenolamine for these neurones: 3.0 for PON, 1.0 for VIN), methylergometrine (EPQs: 3.0 for PON, 0.3 for VIN) and mescaline (EPQs: 0.3 for PON, 1.0 for VIN). These two neurones were excited slightly by DL-metaraminol (EPQs: 0.03 for both neurones) and DL-neosynephrine (EPQs: 0.03 for PON, 0.1 for VIN); VIN was also slightly sensitive to 3-methoxytyramine (EPQ: 0.03). TAN and 1-VMN were inhibited by ergometrine (EPQs: 1.0 for TAN, 0.3 for 1-VMN) and methylergometrine (EPQs: 0.3 for TAN, 0.1 for 1-VMN), whereas d-RPeAN was inhibited slightly only by DL-neosynephrine (EPQ: 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

Role of MLC serum inhibitory factors in high MLC-reactive kidney transplant recipients pretreated with donor-specific blood transfusion (DST).

In order to clarify the beneficial effect of donor-specific blood transfusions (DST) on kidney allograft survival, sera from 16 patients treated with DST were studied using the mixed lymphocyte culture (MLC) serum inhibition test. The results demonstrate that MLC inhibitory factors could be induced in the serum of the recipients after the completion of DST, and that these factors are directed against cells of the recipient but not against cell from the donor. Regarding the correlation with rejection episodes and clinical outcome, a significant improvement in renal transplant survival and reduction in rejection episodes was observed when MLC inhibitory factors were present in post-DST sera. These data suggest that such factors may contain antibodies directed against recognition sites on T lymphocytes, e.g., anti-idiotypic antibodies, and be associated with prolonged graft survival of living-related, high MLC-reactive one-haplotype-mismatched kidney.

Ultrasonic vocalizations and aggressive behavior in male rats.

The aim of the present study was to know whether 22-28-kHz vocalizations have any communicatory role in the regulation of aggressive behavior in male rats of the Wistar strain. In pairs of intact rats 22-28-kHz vocalizations showed a positive correlation with the extent of aggressive behavior. The pattern of aggressive behavior during ultrasonic vocalizations was different from that just before and just after the vocalizations. However, surgically deafened rats were less active in aggressive behavior and more active in ambulatory activity in the open field than the controls. Muted rats were not different from the controls in both aggressive behavior and ambulatory activity. The present result that the deprivation of ultrasonic signals failed to increase aggressive behavior does not support the classical hypothesis that ultrasonic vocalizations inhibit the initiation of aggressive behavior. It is concluded that ultrasounds emitted during aggressive encounters may have little communicative value in male rats.

Decreases of action potential amplitudes, in sodium-free and calcium-free conditions, of identifiable giant neurons of an African giant snail (Achatina fulica Férussac)--I. The right parietal ganglion.

Decreases of the action potential amplitude in sodium- and calcium-free states were observed with respect to the four giant neurons, PON (periodically oscillating neuron), Tan (tonically autoactive neuron), RAPN (right anterior pallial neuron) and d-RPLN (dorsal-right parietal large neuron), identified in the right parietal ganglion of the suboesophageal ganglia of an African giant snail (Achatina fulica Férussac). The decrease of the PON action potential amplitude, caused in the sodium-free state, was observed to be 25.4 +/- 2.1% (23.0 +/- 2.0 mV), expressed by M +/- SE, while that of the calcium-free state was 35.0 +/- 2.1% (30.9 +/- 1.7 mV). Then, the two ionic dependencies of the PON action potential were estimated to be about 40-50% on sodium and 50-60% on calcium. The decrease of the TAN action potential in the sodium-free state, was observed to be 20.7 +/- 1.2% (18.8 +/- 1.3 mV), whereas that of the calcium-free state was 42.2 +/- 2.7% (39.0 +/- 2.2 mV), indicating that the two ionic dependencies were 30-40% on sodium and 60-70% on calcium. The decrease of the RAPN action potential in the sodium-free state, was 45.8 +/- 3.7% (40.3 +/- 3.1 mV), whereas that of the calcium-free state was 21.7 +/- 2.5% (17.8 +/- 2.0 mV), indicating that the two ionic dependencies were about 70% on sodium and about 30% on calcium. The decrease of the d-RPLN action potential in the sodium-free state was found to be 17.6 +/- 2.4% (15.2 +/- 1.8 mV), whereas that of the calcium-free state was 23.1 +/- 1.4% (20.8 +/- 1.4 mV), indicating that the two ionic dependencies were 40-50% on sodium and 50-60% on calcium. The action potential amplitudes of all the neurons tested were decreased in both sodium-free and calcium-free states. However, their ionic dependencies were estimated to vary from 70% on sodium (30% on calcium) to 30% on the sodium (70% on the calcium), according to the neurons tested.

Presence of TAN (tonically autoactive neuron) and its two analogous neurons, located in the right parietal ganglion of the suboesophageal ganglia of an African giant snail (Achatina fulica Ferussac). Morphological and electrophysiological studies.

Three identifiable giant neurons, which were morphologically and pharmacologically identical, named TAN (tonically autoactive neuron), TAN-2 and TAN-3, were found in line on the dorsal surface of the right parietal ganglion in the suboesophageal ganglia of an African giant snail (Achatina fulica Férussac). The diameters of these neurons were 150-200 microns. They showed regular spontaneous spike discharges at the rate of 30-40 per min. However, the spike discharges of the three neurons were not synchronized. For morphological studies of these neurons, Lucifer Yellow was injected into their somata by pressure. The axonal branches of the three neurons examined extended commonly into the following seven peripheral nerves: left anterior pallial, left posterior pallial, intestinal, anal, right posterior pallial, right anterior pallial and right anterior pallial accessory nerves. The sensitivities of the three neurons examined to the main neurotransmitter candidates and their derivatives were almost identical. These neurons were excited by 5-hydroxytryptamine [the minimum effective concentration (MEC): 10(-5)-10(-4) M] and histamine (MEC: 1-3 X 10(-4) M), and inhibited by dopamine (MEC: 1-3 X 10(-4) M), L-norepinephrine (MEC: 3 X 10(-4)-10(-3) M), L-epinephrine (MEC: 3 X 10(-4) M), GABA (MEC: 10(-5)-10(-4)), acetylcholine (MEC: 1-3 X 10(-4) M) and its derivatives.
