[Epidemiological and therapeutic study on gonorrheal infection--study on NY-198 (lomefloxacin) single administration therapy. Sapporo Clinical Research Group for STD].

The efficacy of single administration of NY-198 in the treatment of gonococcal infections was studied employing male patients with gonorrheal urethritis and female patients with gonorrheal cervicitis. The clinical efficacy was evaluated on the basis of the efficacy rates which was estimated on each observation day for each patient. In the male patients, NY-198 was administered in a dose of 200 mg in 25 cases and a dose of 400 mg in 65 cases. The rate of eradication of N. gonorrhoeae (i.e. efficacy) on the 3rd day of administration was 96% in the 200 mg administration group and 100% in the 400 mg administration group. On the 7th day after administration, the eradication rate was 100% in both the 200 mg and 400 mg administration groups. When the eradication rate on the 7th day was statistically estimated for patients who returned to the hospital only on the 3rd day, it was 97.9% in the 200 mg administration group. In the female patients, NY-198 was administered in a dose of 200 mg to 4 cases and a dose of 400 mg to 5 cases. The eradication rate on each observation day was 100% in both dosage groups. In conclusion, single administration of NY-198 in a dose of 200 mg or 400 mg was highly effective, and considered to be a therapeutic method highly useful clinically.

The relationship between connective tissue and its microvasculature in the healthy dog gingiva.

The morphological consistency of subepithelial connective tissue and its vascular architecture was studied in the healthy gingiva of adult mongrel dogs. Corrosion-cast specimens of the vasculature and specimens of well-separated connective tissue from the lamina propria were observed by SEM. Specimens injected with india ink also were examined employing light microscopy. Summarizing the results: 1) Beneath the buccal gingival epithelium, short conical papillae were seen in the subepithelial connective tissue. The capillary structure throughout this region was basically of the loop type, the overall appearance of which replicated the papillae. The papilla disappeared at the muco-gingival junction, so that after the transition to the alveolar mucosa, no papillary formation was observed. The vasculature had transformed from loop to network structure. 2) Beneath the sulcular gingival epithelium, the subepithelial connective tissue displayed a smooth concavity with no papillae. In this region, the capillaries formed a flat, dense network in accordance with the morphology of the connective tissue. The capillary network suddenly disappeared at the terminal edge of the inner epithelium. The vessels of the lamina propria located immediately beneath the network reduced their lumina at the margin and formed the plexus of the periodontium, displaying a rather open network.

[Changes in microvascular patterns following the periodontal flap surgery].

Serial changes in the microvascular pattern beneath the inner epithelium and in the periodontal connective tissue attachment on the upper incisors in adult mongrel dogs, were studied by scanning electron microscopy to elucidate the process of reconstructing the vascular architecture following mucoperiosteal flap surgery. In the early stage of wound healing, capillaries beneath the epithelium in the marginal gingiva were transformed into glomeruli, in the course of the predominantly elongation type of angiogenesis. At the junctions, newly formed sinusoidal vessels were formed in the course of predominantly sprouting type of angiogenesis, and anastomosed with vasculatures between the flap and the periodontal membrane. In these newly formed vessels, vascular glomeruli in the marginal gingiva and newly formed sinusoidal vessels at the coronal side of the junctions were transformed into a capillary network beneath the inner epithelium accompanying the epithelization. At the apical side of the junctions, newly formed sinusoidal vessels showed a series of changes to cope with alterations of the tissue environment accompanying connective tissue attachment, but the vasculatures established after surgery showed a dense and irregular arrangement compared with the control side. These results show that the attachment mechanism obtained by wound healing differ from the original one.

[Clinical statistics on outpatients at the urological clinic of Sanjukai Hospital in 1988].

A statistical study was performed on new outpatients. The total number of new outpatients in 1988 was 8,389 (male: 5,284, female: 3,105) and the male to female ratio was 1.70:1. They had urogenital diseases definitely diagnosed (7,486), urogenital diseases indefinitely diagnosed (576), no diseases (260), and diseases other than urogenital (67). Thirty-five percent of the outpatients were referred to us by other sources. The number of operations on new outpatients was 139, circumcision, vasectomy and resection of condyloma were representative. The peak of the age distribution was in the thirties for males and in the fifties for females. A statistical study was made on new outpatients according to the international classification of disease. There were 146 malignant (urogenital) tumors (1.8%). The major diseases of the new outpatients were cystitis (acute or chronic: 20.7%), upper urinary tract stone (18.5%), prostatitis (13.0%), and benign prostatic hypertrophy (11.2%). In males the major diseases were prostatitis, upper urinary tract stone, benign prostatic hypertrophy, balanoposthitis, phimosis and in females they were cystitis, upper urinary tract stone, pyelonephritis, renoptosis, and neurogenic bladder. We conclude that our hospital plays a major role as a private urological hospital.

[Clinical studies of transurethral resection in patients with benign prostatic hypertrophy].

During the last 9 years, 1,892 patients underwent surgery of the prostate. Benign prostatic hypertrophy (BPH) accounted for 1,812 cases, and prostatic cancer 80 cases. In the case of BPH, transurethral resection was done in 1,645 cases, cryosurgery in 136 cases, and suprapubic prostatectomy in 31 cases. Latent cancer was detected in 54 (3.3%) of the BPH cases. The average patient age of the patients with latent cancer (73.6 +/- 6.9 years) was higher than that of those without latent cancer (69.5 +/- 7.7 years). This suggested a significant relationship between the age of the patient and the occurrence of latent cancer.

[Transurethral resection of prostate weighing over 100G].

Transurethral resection of prostate was done on 11 prostates weighing over 100g among 1664 cases seen during the last 9 years. The largest prostate weighed 180g and needed 140 minutes for resection. From 400 ml up to 1,600 ml, mean 890 ml of blood were transfused in these patients. No severe complication or death occurred. In conclusion, a prostate weighing less than 200 g can be operated by transurethral resection.

[Clinical experiences of renal and ureteral stones by extracorporeal shock-wave lithotripsy (ESWL). IV: 3-year clinical experience of cases treated with ESWL].

We report our 3-year experience with extracorporeal shock wave lithotripsy (ESWL) since we first used it for upper urinary tract stones on September 1st, 1984. A total of 1,225 patients (1,320 cases) underwent 1,647 sessions with ESWL; They consisted of 855 males (70%) and 370 females (30%). Treated stone locations were 593 renal stones, which contained 112 complete staghorn calculi, 504 ureteral stones, 110 renoureteral stones, and 1 bladder stone. ESWL monotherapy was performed on 90% of cases with renal and ureteral stones, and 46% of cases with complete staghorn calculi. In all the cases so far observed for more than 12 weeks after ESWL, 84.9% of the former showed complete discharge of the stones, and 0.7% showed no change. Only 48.9% of the later showed the complete discharge of the stones, 43.3% of which had residual stones, and 7.8% had fragments of the size of small beans. Complications, which were fever and pain, were noticed in 33.6% of the cases with renal and ureteral stones, and 64.3% of the cases with complete staghorn calculi. After ESWL, hematuria was noticed in almost cases, but the average volume of hemorrhage was 28 +/- 33 ml/day. The only contraindication of ESWL was severe obesity, and in the cases in which spontaneous stone discharge can be expected.

[A case-control study of testicular cancer].

A study on the possible association of maternal factors with testicular cancer was undertaken. Information was obtained from the mothers of testicular cancer cases (N = 37) and the mothers of men without the disease (N = 37). Comparisons were made between the two groups. The cases were collected from the records of nine hospitals, and the controls were selected from five Public Health Centers by individually matching the sex and the year of birth with the testicular cancer cases. The mothers of the cases had significantly fewer live births than the mothers of the controls (the adjusted odds ratio per live birth = 0.4, p = 0.02), when the five following variables had been simultaneously adjusted by the logistic regression analysis. Those five variables included the Quetelet's Index of mothers before pregnancy, the age at an index birth, the duration of breastfeeding for an index child, the birth order of an index child, and the number of induced abortions. The endogenous hormonal milieu of a mother may be associated with the occurrence of testicular cancer in her child.

[Clinical statistics on outpatients at the Urological Clinic of Sanjukai Hospital in 1987].

The total number of new outpatients in 1987 was 8,062 (male 5,056, female: 3,006) and the male to female ratio was 1.68:1. They had urogenital diseases definitely diagnosed (7,197), urogenital diseases indefinitely diagnosed (536), no diseases (269), and diseases other than urogenital (60). Thirty percent of the outpatients were referred to us by other sources. The number of operations on new outpatients was 216, circumcision, resection of condyloma and vasectomy were representative. The peak of the age distribution was in the thirties for males and in the fifties for females. A statistical study was made on new outpatients according to the international classification of disease. There were 125 malignant (urogenital) tumors (1.6%). The major diseases of the new outpatients were cystitis (acute or chronic: 22.0%), upper urinary tract stone (15.9%), prostatitis (13.9%), and benign prostatic hypertrophy (11.9%). In males the major diseases were prostatitis, upper urinary tract stone, benign prostatic hypertrophy, balanoposthitis, phimosis and in females they were cystitis, upper urinary tract stone, pyelonephritis, renoptosis, and neurogenic bladder. We conclude that our hospital plays a major role as a private urological hospital.

[Clinical results of percutaneous nephroureterolithotripsy].

Thirty-five patients (37 renal units) were treated by PNL. Nine had complete staghorn calculi, 2 multiple reno-ureteral stones, 12 solitary renal stone, and 14 upper ureteral stone. The success rate was 94.6%. No severe complications were seen.

[Clinical statistics on outpatients at the Urological Clinic of Higashi Sapporo Sanjukai Hospital in 1985].

A statistical study was performed on new outpatients, the total number of which was 7,786 (male: 4,953, female: 2,833) in 1985. The male to female ratio was 1.75:1. They had urogenital diseases definitely diagnosed (6,786), tentatively diagnosed (600), normal (260), and diseases other than urogenital (140), and 36.2% of them had been referred to us by other sources. On these outpatients 206 operations had been performed circumcision, resection of condyloma and vasectomy were representative. The peak of the age distribution was in the thirties for males and in the twenties for females. For the first time in Japan, we treated renal and upper ureteral stones using extracorporeal shock wave lithotripsy (ESWL) on September 1, 1984. The results of ESWL at out hospital have been satisfactory. A statistical study was made on new outpatients according to the international disease classification. There were 152 malignant (urogenital) tumors (2.1%). The major diseases of the new outpatients were cystitis (acute or chronic: 20.6%), upper urinary tract stones (19.4%), prostatitis (13.5%), benign prostatic hypertrophy (10.7%). In males the major diseases were prostatitis, upper urinary tract stones, benign prostatic hypertrophy, balanoposthitis, and phimosis, and in females they were cystitis, upper urinary tract stone, pyelonephritis, and renoptosis. We conclude that our hospital plays a major role as a private urological hospital.

[Epidemiological and therapeutic studies on gonococcal infections--one shot therapy by spectinomycin--[Sapporo Clinical Research Group for STD]].

From January through March of 1985, the Sapporo Clinical Research Group for STD treated 69 cases of gonococcal infections (61 cases of male gonococcal urethritis and 8 cases of female gonococcal cervicitis) at its facilities in Sapporo City. The therapeutic efficacy of one shot therapy of Spectinomycin (SPCM) was investigated, and an epidemiological study on the cases and bacteriological studies on the isolated strains of Neisseria gonorrhoeae were made. The male patients were between 19 and 55 years old, with a peak age distribution in the younger half of the twenties. The female patients were between 18 and 40 years old. The major source of infections was a so-called special massage parlor which accounted for 36.1% of male cases. The isolation rate of PPNG were 16.7% (11/66). The MIC (inoculum size; 10(6) CFU/ml) of SPCM ranged from 3.13 to 25 micrograms/ml regardless of beta-lactamase production. In male patients, the eradication rate (efficacy rate) of N. gonorrhoeae by SPCM was 94.7% on the first day, 93.6% on the third day and 100% on the seventh day after 2 g one shot therapy. In female patients, the rate was 100% on the third and seventh day after 2 g one shot therapy, and 75% on the first day, 66.7% on third day and 100% on seventh day after 4 g one shot therapy. We considered that one shot therapy of SPCM was effective for gonococcal infection also in the present time. Especially SPCM was effective for infections by PPNG, since it was not resolved by beta-lactamase of N. gonorrhoeae. Positive rate of Chlamydia trachomatis was 16.3% in male gonococcal urethritis, and the serous discharge tended to remain longer in the positive patients than in the negative patients. There was only one side effect (1.4%), therefore SPCM was recognized to be a safely administrated antimicrobial agent.

[Clinical statistics on outpatients at the Urological Clinic of Sanjukai Hospital in 1986].

A statistical study was performed on new outpatients. The total number of new outpatients in 1986 was 8,174 (male: 5,177, female: 2,997) and the male to female ratio was 1.73:1. They had urogenital diseases definitely diagnosed (7,043), urogenital diseases undefinitely diagnosed (689), no diseases (357), and diseases other than urogenital (85). Thirty percent of the outpatients were referred to us by other sources. The number of operations on new outpatients was 182, circumcision, resection of condyloma and vasectomy were representative. The peak of the age distribution was in the thirties for males and in the twenties for females. A statistical study was made on new outpatients according to the international disease classification. There were 109 malignant (urogenital) tumors (1.4%). The major diseases of the new outpatients were cystitis (acute or chronic: 21.4%), upper urinary tract stone (16.8%), prostatitis (14.1%), and benign prostatic hypertrophy (11.3%). In males the major diseases were prostatitis, upper urinary tract stone, benign prostatic hypertrophy, balanoposthitis, phimosis and in females they were cystitis, upper urinary tract stone, pyelonephritis, renoptosis and neurogenic bladder. We conclude that our hospital plays a major role as a private urological hospital.

[Clinical experience with extracorporeal shock wave lithotripsy (ESWL) in renal and ureteral stones. The 3rd report: Treatment of 59 cases (64 renal stones) of complete staghorn calculi].

This report describes the treatment of 64 cases of complete staghorn calculi using extracorporeal shock wave lithotripsy (ESWL). Thirty cases (46.8%) were successfully treated using ESWL monotherapy and the rest of the cases (53.2%) required ESWL combined with an auxiliary procedure. Forty-four of the cases (68.7%) had symptoms such as fever and pain after ESWL. These cases were treated with chemotherapeutic agents and an auxiliary procedure. X-ray examination showed that the result of ESWL treatment was satisfactory in 97.4% of the 34 cases followed up for more than 12 weeks after ESWL. Based on the results of this study, the following plan of treatment for complete staghorn calculi has been adopted in our hospital. ESWL monotherapy is performed in cases without a dilated collecting system or stricture. ESWL combined with an auxiliary procedure is performed in cases with a dilated collecting system, stricture, ileal conduit and solitary kidney.

[Epidemiological and therapeutic studies of gonorrheal infection--clinical efficacy of sultamicillin].

We conducted an epidemiological study including analyses of background factors of 192 male and 13 female patients with gonorrheal infection in the Sapporo area and at the same time, investigated the therapeutic efficacy of sultamicillin, an ester linked prodrug of ampicillin and beta-lactamase inhibitor sulbactam in the treatment of these patients. The percentage of infections in Sapporo was rather high in the young generation, being as high as 13.5% in teen-age boys and 30.8% in teen-age girls, which were higher than the 6.1% and 6.3% of corresponding groups in Honshu island. The source of infections was so-called special public bath-ouse which accounted for about 31.8% of all cases which however, was lower than the 50% in Honshu island. By contrast, the percentage of their friends or so-called pick-up friends as a source of infection in Sapporo was as high as 46.9% which was significantly higher than the 19.9% in Honshu. Juveniles who had nonprostitutes of the other sex as a source of infection are a characteristic of the patients in Sapporo. The isolation rate of PPNG was 13.8%. The MIC (10(6) CFU/ml) of sultamicillin ranged from 0.05 to 0.39 micrograms/ml in beta-lactamase non-producing strains and from 0.20 to 1.56 micrograms/ml in beta-lactamase producing strains showing no trend of higher MIC against beta-lactamase producing strains. There was almost no difference in the efficacy of sultamicillin between a daily dose of 750 mg (2 tablets) and 1125 mg (3 tablets) nor in side effects. The eradication rate (efficacy rate) of gonococcus following a 3-day therapy was 96.2% (38.9% excellent cure rate) in male patients and 83.3% (8.3%) in female patients. In 31% of the male patients who underwent a 7-day therapy, residual serous secretion was found though some inaccuracy is involved in this data since dropouts were not counted. This suggests the need of concurrent therapy with other appropriate drugs in consideration of possible mixed infection involving Chlamydia trachomatis or other microorganisms.
