Mechanism of nasal secretion mediated via nerve reflex in guinea pigs and evaluation of antiallergic drugs.

In order to confirm the mechanism of nasal secretion mediated via a nerve reflex in guinea pigs, the secretory response from the contralateral side was studied which was induced by local application of various stimulators. There was no difference in the nasal secretion between the contralateral and the stimulated sides when the secretion was induced by allergen, histamine, and capsaicin at lower doses. Methacholine caused a nasal secretion only on the stimulated side. Pretreatment with local anesthetic and ganglionic blockers blocked the secretory response bilaterally which was induced by allergen, histamine, and capsaicin. Antihistaminics also blocked the secretory response induced by allergen and histamine on both sides, but not the capsaicin-induced nasal secretion. Unilateral pretreatment with local anticholinergics prevented all secretory responses only on the stimulated side. Thus, exogenous and endogenous histamine released by the allergen-antibody reaction may stimulate histamine H1 receptors located in the sensory nerve endings as trigger, resulting in the secretory response mediated via a nerve reflex, while methacholine may act directly on nasal glands. Ketotifen and azelastine, which are chemical mediators releasing inhibitor with antihistaminergic activity, prevented the nasal secretion induced by histamine and allergen. On the other hand, disodium cromoglycate, amlexanox, and tranilast had only a slight effect on the allergen-induced nasal secretion. The secretory response on the contralateral side induced by various stimulators would be useful in the in vivo evaluation of anti-allergic drugs to demonstrate the difference in their modes of action.

[Molecular biological study on multifocal infection of HPV in uterine cervix, vagina and vulva].

Vaginal and vulvar scrapes from 30 patients which have HPV types 16 or 18 positive cervical neoplasia (13 of mild dysplasia, 5 of moderate dysplasia, 4 of severe dysplasia, 3 of carcinoma in situ, and 5 of invasive carcinoma) were examined by the polymerase chain reaction (PCR), to detect amplified E7 gene of HPV types 16 and 18 DNA sequences. In 24 vaginal and 15 vulvar scrapes, the same type of HPV as in cervical scrapes was detected. The frequencies of HPV DNA in vaginal and vulvar scrapes for histological diagnosis of cervical neoplasia were 84.6% (11/13) and 53.8% (7/13) in mild dysplasias, both were 100% (5/5) in moderate dysplasias, 75% (3/4) and 25% (1/4) in severe dysplasias, 66.7% (2/3) and 33.3% (1/3) in CISs, and 60% (3/5) and 20% (1/5) in invasive carcinomas, respectively. The results of vulvar scrapes showed that the frequency of HPV DNA in mild and moderate dysplasias was significantly higher than that in severe dysplasias, CISs and invasive carcinomas (p < 0.05). We suggest that, in addition to the investigation for the natural history of HPV infection, cell dynamics, local immunological status and any other factors in the HPV-infected lesion must be investigated to clarify the carcinogenesis of HPV in the uterine cervix. Furthermore, HPVs detected in the vagina and vulva were thought to have disappeared without reaching an integrated form during the follow-up of cervical neoplasia with HPV infection over a long period.

Occurrence and expression of human papillomavirus type 16 genes in uterine cervical carcinomas.

The detection of human papillomavirus (HPV) type 16 early genes: E7, E5, and the late gene: L1 was attempted in 42 uterine cervical neoplasia (35 cervical carcinomas and 7 cervical dysplasias) using the polymerase chain reaction (PCR) method. Consequently, E7 gene was detected in 19 (54.3%) of 35 carcinomas and in 5 (71.4%) of 7 dysplasias, E5 gene was detected in 7 (20.0%) of 35 carcinomas and in 5 (71.4%) of 7 dysplasias, L1 gene was detected in 18 (51.4%) of 35 carcinomas and in 5 (71.4%) of 7 dysplasias, respectively. In order to elucidate the transcriptional pattern of HPV type 16 in each of the clinical stages, the expression of mRNA for E7, E5 and L1 genes was examined in HPV DNA positive cases using the reverse transcriptase polymerase chain reaction (RT-PCR) method. E7 gene mRNA was detected in 18 (94.7%) of 19 cervical carcinomas, whereas E5 and L1 genes mRNAs were detected in only 4 (57.1%) of 7 and in one (5.6%) of 18 carcinomas respectively. In cervical dysplasias, E7, E5 and L1 genes mRNA were detected in all cases. E7, E5 and L1 genes were transcriptionally active in all dysplasias, whereas E5 and L1 genes were not always transcriptionally active in carcinomas. These results suggest that the HPV type 16 early gene E7 is present preferentially as integrated form and transcriptionally active in the carcinoma cell, and plays an important role in the development of malignancy. On the other hand, E5 and L1 genes are present and transcribed in the dysplasia cell but their transcriptional activity is less frequent in the carcinoma cell.

[Molecular biological study on the overexpression of c-myc gene in uterine cervical carcinomas].

The expression of mRNA for c-myc gene was investigated in 30 cases of human uterine cervical carcinoma by Northern blot hybridization, and c-myc gene amplification was also examined by Slot blot hybridization. Overexpression of c-myc gene was detected in one (7.7%) of 13 cases of carcinoma in situ (stage 0), one (10.0%) of 10 stage I and 4 (57.1%) of 7 stage II uterine carcinoma, and c-myc gene amplification was detected in 2 cases (6.7%) in which c-myc gene overexpression was also found. The patient whose c-myc mRNA level was more than 10 times as high as in normal uterine cervical tissue relapsed and died within 2 years after the first treatment. Our results suggest that the overexpression of c-myc gene occurred more frequently in advanced than in early uterine cervical carcinoma and that it might be useful in prognosing cervical cancer.

Evaluation of the usefulness of serum phosphatases and osteocalcin as serum markers in a calcium depletion-repletion rat model.

The present study examined the usefulness of the serum alkaline phosphatase (ALP) activity, osteocalcin, and tartrate-resistant acid phosphatase (TrACP) activity as bone turnover markers in a calcium depletion/repletion rat model. Weanling rats were fed calcium-deficient diet for 4 weeks, followed by 2 weeks of dietary calcium repletion. Serum phosphatases and osteocalcin were determined and compared with those of corresponding age-matched, pair-weighted controls. Rats were sacrificed at the end of each phase of the study, and bone phosphatase activities in tibiae and vertebrae were measured. During calcium depletion, rats developed hypocalcemia and lost significant bone calcium, which were reversed with dietary calcium repletion. During depletion when previously published histologic studies indicated a suppressed bone formation and stimulated bone resorption, serum ALP activity and osteocalcin levels were significantly elevated and serum TrACP activity reduced; at the same time, the bone ALP and TrACP activities were increased. Because the serum level of 1,25 dihydroxyvitamin D3 (1,25(OH)2D3) which has been shown to stimulate the synthesis of skeletal ALP and of osteocalcin, was also significantly increased during depletion, the increased serum ALP and osteocalcin level could be indirect consequences of the hypocalcemia-mediated elevation in 1,25(OH)2D3 level. These effects were reversed upon calcium repletion, during which previously published histologic studies demonstrated a stimulated bone formation and a suppressed resorption in these rats. In conclusion, although there is increasing evidence for the usefulness of these serum proteins as markers of bone metabolism in humans, a great deal more work is required before we can understand the significance of these assays. Until such is accomplished, these assays should not be assumed to be validated.

Expression of basic fibroblast growth factor in human gastric carcinomas.

The expression of mRNA for the basic fibroblast growth factor (FGF) gene was examined in seven human gastric carcinoma cell lines and in tissue from 29 gastric carcinomas together with the adjacent normal mucosa. Among the seven gastric carcinoma cell lines, the MKN45 cell line expressed mRNA for the basic FGF gene. Basic FGF protein production was confirmed by flow cytometric analysis and immunohistochemistry. Among the surgical specimens, 16 (55%) of 29 gastric carcinomas showed higher levels of basic FGF mRNA than the normal mucosa. Interestingly, in scirrhous gastric carcinomas characterized by their fibrous stroma and rapid growth, 9 (69%) of 13, samples examined revealed higher levels of basic FGF mRNA than normal mucosa, whereas only 3 (33%) of the 9 well differentiated adenocarcinomas studied produced similar results. Immunohistochemically, basic FGF protein was localized in tumor cells. These results suggest that basic FGF produced by tumor cells may play an important role in producing fibrosis and angiogenesis in gastric carcinomas.

A new method of the measurement of nasal secretion in guinea pigs.

A simple quantitative method to measure nasal secretion in guinea pigs is described. Nasal secretion was measured with a piece of cotton thread dyed with fluorescein at one end which was inserted into an anterior naris and kept there for 60 s. The stretch of color of a thread dyed with fluorescein was proportional to fluid volume and to increase in weight of a thread due to absorbed nasal secretion induced by nasal provocation. In addition, the stretch of color due to nasal secretion was associated with the score of rhinorrhea. Thus, it is considered that the amount of nasal secretion can be reflected to the length of the stretch of color. Each secretion on the ipsilateral and the contralateral sides induced by nasal provocation could be separately measured by this method. The amount of nasal secretion induced by allergen in passively sensitized guinea pigs could be reduced by pretreatment with ketotifen or flutropium. These results suggest that our method may serve as a quantitative test for nasal secretion in guinea pigs, which would be useful in the study of hypersecretory response in the allergic model or in evaluating the effect of antiallergic drugs on nasal allergy.

[In situ hybridization study on the detection of HPV DNA in adenocarcinoma and adenosquamous carcinoma of the uterine cervix].

Human papillomavirus (HPV) types 16 and 18 have been found closely associated with squamous cell carcinoma and related lesions of the uterine cervix. In order to investigate the relationship between HPV and adenocarcinoma and adenosquamous carcinoma of the uterine cervix, formalin-fixed, paraffinembedded tissues prepared from 38 cases consisting of 30 cases of adenocarcinoma and 8 cases of adenosquamous carcinoma were examined for the presence of HPV DNA by in situ hybridization with digoxigenin labeled HPV 6/11, 16, 18 DNA probes. HPV DNA was localized on the nuclei of the cancer cells in adenocarcinoma and adenosquamous carcinoma. HPV DNA was detected in 13 cases (43.3%) of adenocarcinoma and 4 cases (50.0%) of adenosquamous carcinoma, and HPV type 18 DNA was detected in 13 cases (34.2%) of adenocarcinoma and adenosquamous carcinoma. These findings suggest an association between HPV, especially HPV type 18, and adenocarcinoma and adenosquamous carcinoma of the uterine cervix.

Growth inhibition of BALB/c 3T3 cells by a high-molecular-weight mucin-like glycoprotein of human milk fat globule membrane.

PAS-O is a high-molecular-weight mucin-like glycoprotein of the human milk fat globule membrane, and originates from the lactating mammary epithelial cell membrane. The cell proliferation rate of BALB/c 3T3 cells was markedly reduced by addition of PAS-O to their culture medium, this inhibition being concentration-dependent (0-10 microgram/ml) and reversible. The inhibitory activity was removed by periodate or protease treatment of the glycoprotein. PAS-O may interact with 3T3 cells to mimic the effect of high cell density on growth. PAS-O, which has been recognized as a glycoprotein molecule carrying a differentiation antigen expressed on the mammary epithelial cell surface, may play a role in regulating the growth of the mammary cells.

Animals

[Study on the localization of human papillomavirus DNA in the vulvar disease by means of in situ hybridization].

We investigated human papillomavirus (HPV) DNA of vulvar diseases obtained from 18 cases of condyloma acuminatum, 3 cases of hyperplastic dystrophy without atypia, 4 cases of lichen sclerosus, 4 cases of Bowenoid papulosis, one case of Bowen disease and 8 cases of squamous cell cancer by in situ hybridization with biotinylated HPV 6, 11, 16 and 18 DNA probes. The results of in situ hybridization analysis showed that HPV 6/11 was positive in 94.4% (17/18) of condyloma acuminatum. In 17 cases of HPV 6/11 positive condyloma acuminatum, 3 cases were positive for HPV 16 and one for HPV 18, respectively. HPV 16 was positive in 75.0% (3/4) of Bowenoid papulosis and one case of Bowen disease was positive for HPV 18. In two cases of Bowenoid papulosis which were positive for HPV 16, cervical carcinoma in situ containing HPV 16 DNA sequences was also found during the follow up period. In 8 cases of squamous cell cancer, 2 cases were positive for HPV 18, one for HPV 16 and one for HPV 6/11. All cases of hyperplastic dystrophy without atypia and lichen sclerosus were negative for HPV DNA. Our results suggested HPV is closely associated with vulvar neoplasia.

[A case report of Campylobacter fetus subspecies fetus bacteremia].

An unusual case of Campylobacter fetus subspecies fetus bacteremia was presented. A twenty four year old male was admitted to our hospital due to abdominal pain, general malaise, diarrhea, high fever, and hemoptysis. He was alcoholic and fond of eating raw liver. He had a history of partial gastrectomy and disturbance of pancreatic function. He showed pulmonary empyema, pleuritis, thrombophlebitis of lower legs, jaundice, hepatomegaly, diarrhea, pneumothorax, and low T3 low T4 syndrome. C. fetus subsp. fetus was detected from the venus blood and pleural effusion on admission. He was successfully treated by gentamicin, chloramphenicol, and minocycline. This is the fourth case of C. fetus subsp. fetus bacteremia in the Japanese literature. This microanerophilic gram negative curved bacillus has been increasingly associated with human disease and relapsing in nature, so protracted antimicrobial therapy was recommended.

[Clinical evaluation of serum sialyl Lewis X-i (SLX) in diffuse panbronchiolitis].

Clinical comparison of SLX with other tumor-associated antigens such as CA19-9, CA125, and CEA, was made in benign respiratory diseases including diffuse panbronchiolitis (DPB), bronchiectasis (BE), bronchial asthma (BA), and pulmonary emphysema (PE). Sensitivities of each marker (greater than 38.0 U/ml) on DPB were 79.4% in SLX, 68.0% in CA19-1, 46.7% in CA125, and 35.7% in CEA. Serum levels of SLX in DPB and BE were significantly higher than those in BA and PE. There seemed to be no relationship between serum levels of SLX and CRP, ESR, and the volume of sputum. Immunohistochemical studies showed positive staining on the surface of respiratory bronchioles and alveolar walls in DPB and these findings were not observed in normal lung tissues. We consider that the high value of serum levels of SLX in DPB can be explained by these findings.

[Two cases of Mycoplasma pneumoniae pneumonia with obstructive ventilatory disturbance].

Mycoplasma pneumoniae (M. pneumoniae) pneumonia is usually considered as a benign disease, but in rare cases, the disease is fatal. Fatal cases requiring mechanical ventilation and many extrapulmonary complications have been reported. Cases accompanied with obliterative bronchiolitis also have been reported. Two cases of M. pneumoniae pneumonia with obstructive ventilatory disturbance were reported. In both cases clinical diagnosis of M. pneumoniae pneumonia was confirmed by a rise in the complement fixation antibody titer, a rise of the cold hemagglutinin titer and isolation of M. pneumoniae from sputum and pharynx. In one case we performed transbronchial lung biopsy (TBLB). The lung biopsy specimen showed bronchiolitis, so histological findings were thought to be compatible with ventilatory function. In both cases arterial oxygen pressure (PaO2) was below 60 Torr, by administration of erythromycin (EM) and adrenocorticosteroids, chest X-ray findings, atrial blood gas analysis findings and ventilatory function were improved.

[Clinical effectiveness of cefoperazone for respiratory infections of elderly patients, focusing on its safety on renal function].

Clinical effectiveness of cefoperazone (CPZ) was studied for 46 elderly patients with respiratory infections. 1. Clinical efficacy rate of CPZ was 80.6% (29/36). 2. Adverse reactions occurred in 2 patients as systemic rush and fever. Laboratory abnormalities were observed in 5 patients. These included elevation of transaminase value. 3. The serum concentration of CPZ at 12 hours after administration of 2 g CPZ was not reflected in creatinine clearance and total bilirubin, and those 2 values appeared to have little effects on CPZ excretion. 4. The administration of CPZ at 4 g/day for less than 3 weeks had no effects on the renal function. As a result of this study, CPZ was found to have little undesirable effect on renal functions and to be a useful drug for the treatment of respiratory infections in elderly patients.
