Isolated capitate fracture diagnosed by computed tomography. Case report.

Isolated fractures of the capitate, though rare, are probably often missed entirely. As in scaphoid fractures, repeated radiographs, tomography, or bone scanning are necessary for the diagnosis. Reviewing the literature, we found that most capitate fractures reported were transverse or combined with other carpal injuries, including the scapho-capitate fracture syndrome. Our patient presented an isolated oblique intraarticular fracture of the capitate. An old fracture of the styloid process of the ulna was seen at the primary examination. This special type of capitate fracture has not been previously described. The fracture was missed on standard radiographs, but discovered by computed tomography (CT). The different types of fractures of the capitate and their treatment are discussed.

[Normal hysterosalpingography in tubal pregnancies].

We report on the case of a patient who had a laparotomy for an 8-week-old tubal ectopic pregnancy 11 days after a normal HSG. A recent normal HSG cannot exclude an extrauterine, tubal pregnancy in fertile patients.

Mesenteric lymphangioma diagnosed by ultrasonography.

An eight year old previously healthy girl presented with an increase in abdominal girth. A 22 x 23 cm multiloculated tumour was visualized by ultrasonography, and surgery revealed a lymphangioma of mesenteric origin. Microscopy confirmed the diagnosis of lymphangioma.

[Meckel's diverticulum---clinical picture, diagnosis and treatment].

Meckel's diverticulum is the most common anomaly of the gastrointestinal tract. It is usually asymptomatic, but it can cause rectal bleeding and bowel obstruction in both children and adults. Different investigative methods are used to diagnose Meckel's diverticulum. When a Meckel's diverticulum is suspected to cause rectal bleeding in children, the first choice of examination is the Meckel scan. Angiography is an alternative method in adults. Demonstration of a Meckel's diverticulum using barium meal examination is rare, but enteroclysis has been stated to be the most accurate method in detecting Meckel's diverticulum and other focal abnormalities of the small bowel.

Intussusception in children: plain abdominal radiograph and reduction by barium enema in relation to clinical features.

A retrospective study of 98 patients representing 104 instances of intussusception is reviewed. Criteria for inclusion in the study were: the patients primary admitted to the department of diagnostic radiology, plain abdominal radiographs performed prior to barium enema (BE) and/or surgery and diagnosis confirmed by BE or surgery. A success rate of BE reduction of 58.6% was achieved. The clinical features are described and the value of plain abdominal radiograph in ruling out the diagnosis of intussusception is discussed.

Follow-up investigation of fresh lunate bone fracture.

In order to evaluate the clinical and radiological aspects of fresh lunate fractures, a follow-up investigation was carried out. In the past thirty-one years (1956-1986), seventeen patients were found to have fresh lunate fractures. Eleven of these cases were clinically and radiologically reexamined, with observation intervals ranging from 1.25 to 31.33 years. The authors found no radiologic evidence of lunatomalacia, osteonecrosis, or osteoarthrosis. Two cases of chip-fractures had not healed, but the bone structure was normal. Clinically, none of the patients presented with pain, muscular atrophy, or reduced power of grip. None of the patients had changed occupation as a result of the fracture.

Classification of fresh fractures of the lunate.

The X-rays of 17 patients with fresh fractures of the lunate bone have been reviewed. The fractures were classified according to their radiological appearances and according to the vascular anatomy of the lunate. A long term X-ray follow-up examination was performed.

Ultrasonographic appearance of a simple ureterocele following surgical treatment.

A case history is presented, showing the ultrasonographic appearance of a simple ureterocele following surgery. The cystic appearance of the presurgical ureterocele and the gradual changes after surgery are demonstrated. Follow-up examination 17 months after surgical treatment revealed a solid tumour-like ultrasonographic image. Differential diagnosis to neoplasma of the urinary bladder is emphasized.

Radiological demonstration of neurofibromatosis of the ileum.

A 41-year old woman with abdominal pain and evidence of mechanical obstruction of the ileum, underwent surgery. Intussusception caused by a 15 x 1.5 cm tumor was found, additional numerous minor tumors were seen in the ileum. Microscopy revealed a neurofibroma. The patient showed no other symptoms or signs of neurofibromatosis. Barium meal examination and computed tomography were used to visualize the tumors.

Computed tomography in patients with dementia probably due to toxic encephalopathy.

Computed tomography (CT) was performed in 181 patients with dementia probably caused by organic solvents. No treatable causes of dementia were revealed. All but one of the patients had dementia symptoms for more than one year. Only three patients had focal neurologic signs. No indication for cranial CT was found in this group of patients, owing to the fact that no treatable causes of dementia were revealed.
