[Kessler arthroplasty of the carpometacarpal joint of the thumb in arthrosis and arthritis].

Early results of the KESSLER-Implant in 11 patients are presented. 7 patients suffered from rheumatoid arthritis, 1 from psoriatic arthropathy. For technical reasons this endoprosthesis seems to be especially suitable for patients with rheumatoid disease. By remodelling the articular surface of the trapezium corresponding to the implant, it is possible to reestablish stability even in cases with severe destruction and subluxation of the CM joint.

[Medium-term results using the cement-free link endoprosthesis. Ribbed shaft V socket].

Since 3/84 340 primary implantations with the uncemented "Link" hip system (Rib-Type V socket) were done. The results with an average follow up of 3.2 years are published. Only 2.17% revision had to be suffered in the rib stem, while 0.77% socket revision had to be done. According to the score of Merle d'Aubigné the values of pain increased from 2.87% to 5.38%, walking ability from 3.68% to 5.27% and the range of motion from 133 degrees to 199 degrees. A high rate of good and satisfying results proved this anatomical implant system to be a good alternative in total hip replacement in younger patients.

[Behaviour of synovial blood vessels in semi--thin sections in progressive chronic polyarthritis].

The synovial blood vessels of 17 patients with clinically definite rheumatoid arthritis (RA) were studied histologically and ultramicroscopically. No differences between seronegative and seropositive RA subjects were found. The abnormalities observed concerned mainly the endothelial cells, which showed changes varying in degree from swelling to fibroblastic transformation. The destruction of endothelial cells seemed to be linked with a positive test for C-reactive protein as a consequence of an acute relapse. Areas of cellular infiltration and partial homogenization were found in the tunica media of the vessel wall. There was a striking hyperemia of the capillaries and venules.

[Synovial tissue levels in comparison with synovial fluid and serum concentrations following a single daily administration of 20 mg piroxicam (Felden)].

Serum, synovial fluid and synovial tissue was taken from a total of 25 patients of both sexes on whom a synovectomy had been performed, after a 1, 3, 5, 6 or 7 day treatment with a daily administration of 20 mg Piroxicam (5 patients in each group) and the concentration of Piroxicam (Felden) determined. Parallel to this a part of the synovial tissue taken was histologically examined and classified. The analytical results presented here show that in the mean appr. 33% of the Piroxicam passed from the serum into the synovia, appr. 26% into the non or partially florid synovial tissue and appr. 32% into the florid or highly florid synovial tissue. Felden 20 (Piroxicam) thus fulfills the requirement of a good acting, non-steroidal, antiinflammatory substance (NSAID), namely of being available at the location of the inflammatory process.

[Atlanto-dentoid dislocation in chronic polyarthritis. Correlation of clinical, roentgenological, neurological and electromyographical findings].

The involvement of the cervical spine in rheumatoid arthritis especially after prolonged steroid medication, is one of the most dangerous symptoms of this disease. 62 patients suffering from rheumatoid arthritis of the cervical spine underwent a critical examination regarding clinical signs and neurological symptoms. According to Larsen Theses of the radiological grading of the severity of rheumatoid arthritis, we tried to develop a classification of the involvement of the upper and the low cervical spine. Clinical and neurological signs are then found especially in stadium III and IV. The bony ankylosis (stadium V) must be considered as a spontaneous healing, for all clinical and neurological signs will have vanished. Electromyographical observations showed characteristic patterns for each stadium. According to this classification the conservative and the operative treatment can be chosen.

[Synovial hemangioma of the knee joint].

The making of the diagnosis and the histological morphology of the synovial capillary hemangioma of the knee joints is discussed. The cause of this rare condition which is accompanied by monarticular or oligarticular swellings and increased linear growth of an extremity, is a benign tumor-like change of the synovial vessels. It affects the capillary and/or the venous systems and occurs mainly in children and adolescents. The treatment of choice is an early as possible radical synovectomy. Two cases diagnosed late are reported. One of them was diagnosed 17 years after onset of symptoms, despite two previous arthrotomies. It resulted in severe joint destruction. According to what the morphological appearance is, radical early synovectomy, irradiation treatment or steroid treatment is the most appropriate therapy.
