No one dies of old age by itself; death is always due to a disease of some kind. This means that safe and effective therapy for old people is a matter of great practical importance. From what has been said above it will be obvious that the effects of drugs in old people are similar to their effects in the young, but there may be differences in degree in their susceptibility to adverse reactions. The existing data are by no means abundant, sometimes contradictory and of limited evidential value. Looking at the picture as a whole, one cannot but agree with those authors who state that the available results are insufficient to substantiate the general opinion that old age in itself heightens the risk of all forms of pharmacotherapy. From the practical point of view it is nowadays essential that no drug should be prescribed for an elderly patient without strict scrutiny of the need for it, that such drugs should not be administered for any longer than is absolutely necessary, that treatment should normally be started with smaller doses than those usually given to younger adults and, lastly, that surveillance for possible side effects should be particularly close. The author states that it must not be forgotten that geriatric patients frequently consult specialists in addition to their family doctors and that they are perhaps overinclined to listen to advice on drug treatment from those about them. One of the duties of the family doctor is to check the assortment of drugs which his patient is consuming--often an alarming total--and to cut down their number to those which are really necessary.

Of three human neuroblastoma lines tested, IMR32K (and IMR32 parental line) was the only cell line that, after its exposure to a differentiation medium, consistently developed materials recognized immunocytochemically by a panel of antibodies against paired helical filaments (PHF). Ultrastructurally, these cells accumulated, at their perikarya and neuritic extensions, spatially discrete arrays of fibrils, which occasionally occurred in twisted pairs. When these fibrillar structures appeared as paired helices, they exhibited dimensions and configurations reminiscent of PHF found in affected Alzheimer neurons, although less compact. Immunoelectron microscope examinations of the fibrillar structures in these neuroblastoma cells with one of these anti-PHF immunoprobes revealed that only subsets of fibrillar structures that appeared thickened or aggregated to form bundles were selectively immunolabeled. Cultures of these immortal neuroblastoma lines may provide a convenient model for studying aspects of PHF formation that are hard to examine in Alzheimer brain obtained at autopsy.
372 children with suspected liver disease were examined for serum HBsAg. Six (three boys, three girls) were found to be positive (1.6%). Further studies of these patients for up to three and a half years revealed elimination of HBsAg in one case only. Biopsies were performed in five patients. Three showed mild chronic hepatitis (two chronic persistent hepatitis, one unspecific reactive hepatitis). Chronic aggressive hepatitis was diagnosed in one patient. One child seemed to be normal on light microscopy, but the findings on electron microscopy were abnormal, the liver cell nuclei being filled with core particles. Two thirds of the family contacts of these children showed hepatitis B marker. Two pregnancies were observed in HBsAg-positive mothers. An infection of the babies was not demonstrable.

The case of a nineteen-year-old women with the cerebral form of malaria tropica is reported. She showed hyperpyrexia, abdominal manifestations, haemolysis and disseminated intravascular coagulation. Cerebral symptoms amounting to grade IV encephalopathy occurred. The patient responded rapidly to the administration of chloroquine, anticonvulsants, dextran, corticosteroids, antipyretics, blood and antithrombin III and her symptoms had almost completely vanished one week after the onset of therapy.

In a 52-year-old female who was jaundiced since early childhood, marked conjugated hyperbilirubinaemia and a delay of bromsulfophthalein and indocyanine green plasma disappearance rates were observed. Liver histology, plasma bile acids and plasma clearance of 14-C-cholic acid were normal. There was a pronounced impairment of the relative storage capacity of bromsulfophthalein (S = 2,4 mg/mg%) and a moderate decrease of BSP-transport maximum.
Experiences are reported gained from 764 liver biopsies in children ad infants. Indications and contraindications are the same as in adults. The normal liver of infants and young children exhibits some particular differences as compared to the adult liver. Formation of giant cells is a striking pathological finding in various early childhood liver diseases. Needle biopsy is imperative in children with longstanding jaundice when it has to be decided, if surgery is to be done or not. Histology yielded the decisive diagnostic information in 80% of the cases in our hands. Needle biopsy may yield important or decisive information in the other liver diseases of childhood as well.

In extracts of Columbia Arabica Coffee the content of galactomannan precipitable with alcaline copper solution decreases slowly up to an extraction yield of 43.6%; it decreases somewhat more rapidly with higher extraction yields. An extract of Angola Robusta Coffee showed opposite behaviour. In this case the content of galactomannan increased with the extraction yield, although not evenly. Calculated on the basis of roasted coffee solids the extract of the Arabica coffee had a rather limited range precipitable polysaccharide, the Robusta coffee in contrast with rated increased amounts of galactomannan. The different behaviour of the two coffee varieties may be due to mote or less strong binding of this high-polymer carbohydrate to the cell wall.
