[Neck pain. Functional and radiological findings compared with topical pain descriptions].

Topical pain descriptions of the neck are summarized under the unspecific diagnosis "cervical syndrome" (CS). Neck pain localized in the cranial half of the cervical spine attended with occipital extension is defined as "cervicocephalic syndrome" (OCS). Pain concerning the caudal half with extention in both upper limbs or into the interscapular region is called "cervicobrachial syndrom" (UCS). The combination of both syndromes is described as "cervicocephalic and -brachial syndrom" (OUCS).The retrospective analyzed cohort of 75 patients showed a distribution of incidence of 1:20:17 of OCS : UCS : OUCS. Symptoms like headache, vertigo or tinnitus were reported in 34.7% of all cases, only 4% had MRI documented radicular lesions. Functional disturbances showed a maximum in the segments C2-3 (81.4%) and C3-4 (66.7%). Segmental blockades occurred 6 times more frequently than segmental hyper-mobility. Most of the radiological findings were localized in the vertebral segments C4-5 and C5-6, degenerative disc and joint diseases were predominant. The distribution of functional disturbances and radiological findings of the vertebral segments showed no significant coincidence. Therefore this study proved that there are no correlations between manual-medical findings and radiological results related to the subtypes of chronic neck pain.

[Results of a double-blind, randomized comparative study of Wobenzym-placebo in patients with cervical syndrome].

The results of this study are based on 30 patients per treatment group. The primary objective of this trial was evidence of a superior analgesic effect of the verum group under concomitant treatment with physicotherapeutic measures. This objective was achieved and showed a small, but statistically significant and beneficial effect of Wobenzym in the pain self ratings of the patients. The results of erythrocyte sedimentation rates (Westergreen), leukocyte counts and lymphocyte counts as secondary study parameters indicate, that the antiphlogistic effects of Wobenzym provide an additional beneficial effect for the patients suffering from cervical syndrome compared to placebo. In combination with the available safety data of this study, the final interpretation of the authors of this study leads to a superior benefit/risk ratio judgement for Wobenzym compared to placebo.

[Conservative therapy of shoulder pain].

Prerequisite to conservative therapy for shoulder pain is the diagnosis. Along with the topical diagnosis (omarthralgia) the structure analysis aims to identify the site and manner of disturbed structure. Destroyed structures are, in most cases, an indication for anatomical reconstruction, and disturbed structures an indication for conservative methods. Hereby acute symptoms require pain stimulus pacification and chronic symptoms target-specific irritant application. With disorders of the muscles their insertions and the gliding tissue, and in addition to physical methods such as thermotherapy and electrotherapy, the so-called needle treatments (local anesthesia, combination with corticoids, needling, and acupuncture) are to be mentioned. Manual therapeutic methods will also be discussed. With disturbances of the joint function use of intra-articular injections is to be considered, with movement restriction the mobilisation techniques of chirotherapy. Physiotherapy exercises are of particular importance here.

[Spinal diseases: vertebral complaints in relation to site and age].

Disorders of the vertebral column are one of the most common causes of pain in the human being. The nosological classification of various disorders is carried out according to many different criteria, e.g. topical pain descriptions such as cervical syndrome and lumbago in particular, but more often also according to purely speculative factors such as intervertebral disc lesions, muscular rheumatism etc. The analysis of each single structural disturbance which could come into question is a primary function of the anamnesis, and in particular of the clinical examination, which must seek, respectively recognize signs not only of functional disturbances but also of functional devastations, in order to then objectify the structural devastation in particular using a number of different methods. The therapeutic consequences are drawn from the actual diagnosis, which must, above all, distinguish between the acute and the chronic symptoms, in order to then influence those as well as these symptoms in the foreground. Vertebral complaints express themselves in different ways depend on the age of the patient, a fact which must be taken into consideration when forming a diagnosis.

[Prevention of spinal disorders].

The increasing number of people suffering from vertebral column disorders, despite great progress having been made in both diagnosis and treatment, shows the necessity of an effective prophylaxis. The therefore requisite recognition, generally and from case to case, of pathogenic factors, is of essential importance. Influencing prevailing physical conditions such as static, dynamic, muscular and other functional factors by appropriate, drilled exercises, takes priority. Later, correction and instruction for day to day living, such as standing, walking, sitting down, sitting, lifting, carrying and lying correctly, is of equal importance to taking into consideration the possible harmful effects at the place of work, and of sport and also psychosomatic mechanism. The problem with the prophylaxis is that the activities listed should be carried out all life long, but the effects, particularly by primary prevention, cannot be seen till later.

[Gonalgia: treatment with therapeutic local anesthesia. Results of a controlled comparative study versus diclofenac].

The effectiveness and tolerance of therapeutic local anesthesia (TLA) with lidocaine was compared with those of oral diclofenac in a controlled, open, randomized study involving 76 patients with gonalgia due to gonarthrosis or patellar chondropathy. During the course of the three-week (maximum) treatment, the summed score of pain at rest, pain on movement, and restricted mobility, taken as the target parameter of effectiveness, decreased significantly more markedly (p = 0.008) under TLA, than under diclofenac. Side effects involving mainly the gastrointestinal tract occurred significantly more frequently in the diclofenac group (p = 0.012). The results of this study indicate that in the treatment of localized joint pain of degenerative genesis, TLA with lidocaine is superior to NSAID treatment in terms of effectiveness and tolerance.

[Computerized documentation of the topicality of pain].

The exact and objective classification and documentation of pain is a difficult problem which is not easy to overcome. Therefore it seems necessary to reduce as for as possible all mistakes resulting from misunderstandings between pain information given by the patient and documentation carried out by the physician. Therefore a system should contribute to simplify the documentation of pain localisation. By means of dividing the human body into regions and numbering the pain-areas even computer documentation becomes feasible. Thus the standardization of pain area documentation permits its application as parameter in research projects with a large number of cases.

[Report on the results achieved over the past 10 years in the neuro-orthopaedic out-patient department (author's transl)].

A short abstract of the pathophysiology of pain and its conduction is followed by a discussion of the importance of neurological-orthopaedic cooperation for the diagnosis and the treatment of pain syndromes arising from the locomotor apparatus. The examination of the spine from the functional, i.e. segmental point of view is pointed out. The main reflex answer of the spine to irritation is the restriction of movement or hypermobility. Therefore, neuro-orthopaedic reflex treatment consists of so-called "test treatment" by means of manipulation to overcome the restriction of movement or by local curative anaesthesia, or by a combination of both methods. The results over the past decade of treatment in 1008 patients suffering from various pain syndromes are presented and differentiated.

[Osteopetrosis (Albers-Schönberg) (author's transl)].

Investigations of a patient with the benign adult form of osteopetrosis (Albers-Schönberg) revealed that the clinical symptoms correlated with a high bone metabolism, high calcium incorporation in bone, increased osteoclast activity and probably reactive increase of calcitonin in the blood.

[Clinical findings in thoracal functional disturbances (author's transl)].

Functional disorders of the thoracal motor structures show connections with visceral functions through the different forms of pain symptoms, but also not seldom without these, or else appear as their precursors. By means of conventional examination parameters, corresponding interconnection procedures could be demonstrated and the theoretical models for further discussion of the segmental reflector complexes are substantiated.

[Reasons for lumbar syndromes after disc operations (author's transl)].

This investigation presents the findings in 105 patients after disc operations who required renewed in-patient treatment mainly because of their resistance to therapy and/or recurrence of the lumbar syndrome. A number of factors were found to be implicated in providing a reason for unsatisfactory results after operations for prolapsed lumbar intervertebral discs. It must be postulated that interfering factors like ligamental pain, restricted movement, participation of the hip, psychological aspects, polyneuropathics and focal occurrence have to be taken into consideration already preoperatively in the diagnosis, indication for operation and therapy. Also with post-operative complaints special alteration must be paid during rehabilitation to those factors quoted above, as well as to direct post-operative complications like inflammation and scar.

[Influence of the weather on the course of lumbar syndromes].

Sensation of pain during lumbar syndromes is correlated with weather situations, the definition of which takes into consideration traditional as well as new methods of weather classification. To this end, it was necessary to develop an algesia index for the purpose of defining the subjective pain sensation by classifying it in different groups enabling the application of medico-meteorological statistics. The medical data were correlated with the following meteorological arametes: Weather-phase scheme according to Brezowsky-Ungeheuer; temperature-humidity environment; weather situations for Central Europe; synoptic index cycle; advention of temperature in medium altitudes of the atmosphere; vorticity for cyclogenesis. The following results were obtained: Pain sensation in the lumbar area shows a significant dependence on weather situations. Althoguh singular meteorological parameters are not important, the human body is affected primarily by the sum total of all weather elements.

[Radicular and pseudoradicular symptoms of the middle and lower cervical vertebral column (author's transl)].

The differential diagnostic classification of the "shoulder-hand-syndrome" according to etiology and pathophysiological principles is of major importance. The radicular syndrome, subsumed as the shoulder-hand syndrome following a local lesion, must be exactly defined. The pseudoradicular syndrome, which frequently appears clinically in the cervical region under the guise of middle and lower cervical syndrome, has its cause in disturbed function of the vertebral joints. But it can also arise as a mixed syndrome, radicular and pseudoradicular, through a substantial root lesion. The diagnosis of a referred pain in a "shoulder-hand syndrome" can lead to the discovery of internal organic lesions or lesions of the locomotor apparatus of the extremities.
